However, there is no consensus on the optimal protocol and number of trials for screening sarcopenia in older adults with cognitive impairment.</p></div><div><h3>Objective</h3><p>To investigate the use of the first trial (FT), the mean of three trials (MT), and the highest value (HT) from three trials of the HGS test to screen for sarcopenia in older adults with cognitive impairment. Additionally, to analyze the consistency, agreement, and measurement error in the diagnosis of muscle weakness.</p></div><div><h3>Methods</h3><p>176 older adults with cognitive impairment were evaluated. The HGS test was repeated three times. Analyses were performed using the Friedman repeated measures test with Wilcoxon post-hoc, intraclass correlation coefficient (ICC), Standard Error of Measurement (SEM), Minimal Detectable Change (MDC95), and Kappa index tests.</p></div><div><h3>Results</h3><p>There was no significant difference between the first trial (FT) and the mean of three trials (MT) (<em>d</em> = 0.17 [95 % CI: −0.08, 0.42]), but both differed significantly from the highest value (HT) (<em>p</em> &lt; 0.001). The ICC indicated a reliability of 0.97 (95 % CI: 0.95, 0.98) across all participants, while the kappa index demonstrated over 80 % agreement. The SEM for the first measure of HGS ranged from 0.59 to 2.12 kgf. The MDC<sub>95</sub> ranged from 1.64 to 5.87 kgf.</p></div><div><h3>Conclusion</h3><p>For HGS testing, there was excellent consistency between the FM and MT. All three testing methods demonstrated excellent agreement in diagnosing muscle weakness. Views of people with FAI syndrome regarding such interventions are currently unknown, including perceptions of potential barriers and facilitators to participation and adherence to exercise programs.</p></div><div><h3>Objectives</h3><p>To explore participant perceptions of physical therapist-led programs for FAI syndrome, including barriers and facilitators for accessing physical therapy, and adhering to a rehabilitation program.</p></div><div><h3>Methods</h3><p>Our qualitative study used semi-structured interviews to explore the perceptions of patients with FAI syndrome undertaking physical therapy-led treatment, where treatment was ceased due to Coronavirus Disease 2019 (COVID-19). The interview topic guide was informed by the Theoretical Domain Framework. Interviews were transcribed verbatim and data categories were developed using inductive thematic analysis. Themes were discussed between researchers until consensus was reached.</p></div><div><h3>Results</h3><p>Fourteen participants (mean age: 30 years) with a range of physical activity backgrounds undertook interviews. We identified four key themes, 1) Patients believed their hip pain was caused by structural damage worsened through exercise; 2) Barriers and facilitators on the feasibility of physical therapist-led programs; 3) Participants held beliefs regarding the importance of adjunct treatments to exercise; and 4) Impact of FAI syndrome on physical activity participation.</p></div><div><h3>Conclusion</h3><p>People with FAI syndrome believe they have structural damage which leads to their hip pain and are often afraid to exercise due to fear of causing more damage to their hip. Choosing the best diagnostic test to apply in a particular situation can be difficult. The choice of diagnostic test should be informed, at least in part, by evidence of test accuracy. Finding evidence of diagnostic test accuracy has, until recently, been challenging. Ideally, there would exist a database that comprehensively indexes evidence on diagnostic tests relevant to physical therapy practice, is free to access, and is easy to use.</p></div><div><h3>Objective</h3><p>This Masterclass will describe the DiTA (<em>Di</em>agnostic <em>T</em>est <em>A</em>ccuracy) database (dita.org.au) including its development and search interface, and provide advice on how to search and retrieve records.</p></div><div><h3>Discussion</h3><p>DiTA indexes more than 2400 primary studies and systematic reviews of diagnostic test accuracy relevant to physical therapy practice. Users can search DiTA using text fields and dropdown lists to find evidence of diagnostic test accuracy. The database is freely accessible on the internet. However, no common framework and language exist to describe the stages of sports participation.</p></div><div><h3>Objective</h3><p>To present the background to the SPORTS Participation Framework, and how it can be used to illuminate the path that people with disability may travel to enter into, participate in, and enjoy and excel at all levels of sport.</p></div><div><h3>Method</h3><p>The SPORTS Participation Framework includes six stages drawn from mainstream sports pathways and models used to classify barriers to sports participation for people with disabilities: (S) Screening, goal setting and individual preparation, (P) Practitioner led, peer-group sports interventions, (O) Organised junior entry-point sports programs, (R) Recreational sport (non-competitive), (T) Team competition (school/club representation), and (S) State, National, and International competition.</p></div><div><h3>Results</h3><p>For each stage, this paper describes the content of sports activities, the context in which they are performed, key stakeholders, barriers to participation, available evidence, and case studies.</p></div><div><h3>Conclusions</h3><p>The SPORTS Participation Framework presents a structure to navigate the stages of introducing and promoting lifelong sports participation for people with disabilities. Confidence intervals indicate the precision of the results and describe the strength and direction of a treatment effect measure.</p></div><div><h3>Objectives</h3><p>To determine the prevalence of reporting of confidence intervals, achievement of intended sample size, and adjustment for multiple primary outcomes in randomised trials of physical therapy interventions.</p></div><div><h3>Methods</h3><p>We randomly selected 100 trials published in 2021 and indexed on the Physiotherapy Evidence Database. Two independent reviewers extracted the number of participants, any sample size calculation, and any adjustments for multiple primary outcomes. We extracted whether at least one between-group comparison was reported with a 95 % confidence interval and whether any confidence intervals were interpreted.</p></div><div><h3>Results</h3><p>The prevalence of use of confidence intervals was 47 % (95 % CI 38, 57). Only 6 % of trials (95 % CI: 3, 12) both reported and interpreted a confidence interval. Among the 100 trials, 59 (95 % CI: 49, 68) calculated and achieved the required sample size. Among the 100 trials, 19 % (95 % CI: 13, 28) had a problem with unadjusted multiplicity on the primary outcomes.</p></div><div><h3>Conclusions</h3><p>Around half of trials of physical therapy interventions published in 2021 reported confidence intervals around between-group differences. This represents an increase of 5 % from five years earlier. Very few trials interpreted the confidence intervals. Most trials reported a sample size calculation, and among these most achieved that sample size. The plethora of terms and the lack of evidence to support widely used pelvic floor muscle function (PFMF) measurements hinder diagnostic labels.</p></div><div><h3>Objective</h3><p>To structure an examination of PFMF using visual observation and digital palpation and terms consistent with the ICF terminology, and to test its intra and interrater reliability/agreement.</p></div><div><h3>Methods</h3><p>A panel of 9 physical therapists applied Delphi method to structure the PFMF exam under ICF terminology and to verify its measurements reproducibility. For reliability and agreement, a convenience sample of women aged 51.2 ± 14.7 years had the sensitivity to pressure, pain, muscle tone, involuntary movement reaction, control of voluntary movement (contraction/relaxation), coordination, strength, and endurance examined by two raters, in the same day for interrater (<em>n</em> = 40), and one week apart, for intrarater reliability (<em>n</em> = 25). Percent agreement, linear weighted kappa, intraclass correlation coefficient, and Bland-Altman's limits of agreement were calculated (alpha = 0.05).</p></div><div><h3>Results</h3><p>Four round Delphi discussion structured the PFMF exam, named EFSMAP (<em>Exame das Funções Sensoriais e Motoras do Assoalho Pélvico/Examination of Pelvic Floor Sensory and Motor Functions</em>), set a list of concepts and instructions targeted at reproducibility and established PFMF diagnostic labels. Reliability, except for pain and tone, were moderate to excellent (Kw= 0.67–1.0 and ICC=0.48–0.82). Agreement was substantial for most PFMF features (0.64–1.00), except pain.</p></div><div><h3>Conclusions</h3><p>The EFSMAP was successfully developed as a valid and reliable exam to be used in research and clinical practice; it provides labels for the diagnosis of pelvic floor muscle impairments. {"title":"Brazilian classification of physical therapy diagnosis","authors":"Bruno Prata Martinez ,&nbsp;Roberto Mattar Cepeda ,&nbsp;Fernando Mauro Muniz Ferreira ,&nbsp;Francimar Ferrari Ramos ,&nbsp;Flavia Massa Cipriani ,&nbsp;Sibele de Andrade Melo Knaut ,&nbsp;Elaine Caldeira de Oliveira Guirro ,&nbsp;Cristine Homsi Jorge","doi":"10.1016/j.bjpt.2024.101066","DOIUrl":"10.1016/j.bjpt.2024.101066","url":null,"abstract":"<div><h3>Background</h3><p>The Brazilian Classification of Physical Therapy Diagnosis, developed by the Federal Council of Physiotherapy and Occupational Therapy (COFFITO), has the constitutional objectives of standardizing ethical, scientific and social aspects of the Physical Therapy profession.</p></div><div><h3>Objective</h3><p>To describe the work process, rationale and proposal for standardization of a physical therapy diagnosis classification system in Brazil.</p></div><div><h3>Methods</h3><p>A working group was created to propose a standardized classification for the description and codification of physical therapy diagnoses. Some terminologies common to the International Classification of Functioning (ICF) were used to make the nomenclature of diagnoses compatible with the outcomes inherent in the field of physical therapy.</p></div><div><h3>Results</h3><p>The Brazilian Classification of Physical Therapy Diagnosis project culminated in a physical therapy diagnosis model consisting of terms grouped by organic systems and identifying codes. In addition, an application was developed to allow professionals to use the standardized diagnostic classification in an online system.</p></div><div><h3>Conclusion</h3><p>The diagnostic classification system is expected to advance the Physical Therapy profession allowing the identification of structural and/or functional alterations in a simplified and standardised manner. The potential of these technologies for injury prediction, performance analysis, personalized training, and treatment comes with challenges related to the complexity of sports dynamics and the multidimensional aspects of athletic performance.</p></div><div><h3>Objectives</h3><p>We aimed to present the current state of AI and ML applications in sports science, specifically in the areas of injury prediction, performance enhancement, and rehabilitation. We also examine the challenges of incorporating AI and ML into sports and suggest directions for future research.</p></div><div><h3>Method</h3><p>We conducted a comprehensive literature review, focusing on publications related to AI and ML applications in sports. This review encompassed studies on injury prediction, performance analysis, and personalized training, emphasizing the AI and ML models applied in sports.</p></div><div><h3>Results</h3><p>The findings highlight significant advancements in injury prediction accuracy, performance analysis precision, and the customization of training programs through AI and ML. However, future studies need to address challenges such as ethical considerations, data quality, interpretability of ML models, and the integration of complex data.</p></div><div><h3>Conclusion</h3><p>AI and ML may be useful for the prevention, detection, diagnosis, and treatment of health conditions.
