Elcim","doi":"10.5336/medsci.2022-90110","DOIUrl":"https://doi.org/10.5336/medsci.2022-90110","url":null,"abstract":"ABS TRACT Objective: Active breathing control (ABC) system ser- ves as a viable technique for the management of respiratory motion by providing reproducible breath-holding. Herein, we assess physical lung dose-volume parameters of V20 (total lung volume receiving ≥ 20 Gy) and mean total lung dose (MLD) in a series of patients receiving split course radiation therapy (SCRT) for advanced-stage non-small cell lung cancer (NSCLC) with the incorporation of the ABC system. Material and Methods: A series of 22 patients receiving ABC-guided SCRT for advanced-stage NSCLC were evaluated for physical lung dose-volume parameters of V20 and MLD. Two different radiation therapy plans have been generated for every patient with and without the incorpora- tion of the ABC system to investigate the dosimetric impact. Results: Mean V20 and MLD were 32.3% and 2,104 cGy, respectively at free- breathing. Mean V20 and MLD were 24.7% and 1,697 cGy, respectively with the incorporation of the ABC system. The decrease in V20 and MLD was 23.53% and 19.34%, respectively. Conclusion: Our study shows improved normal lung tissue sparing by incorporation of ABC system in SCRT of advanced-stage NSCLC. The reduction in V20 and MLD may have clinical implications. Improved normal lung tissue sparing by use of the ABC system may result in an improved toxicity profile of radiation delivery and thereby enable dose escalation in selected patients. In humans, SIRTs fam i ly i s compo-sed of seven members; SIRT1-SIRT7. The major i ty of SIRTs show NAD + -dependent deacetylase act i v i ty. SIRT4 and SIRT6 funct i on as mono-adenos i ne tr i phosphate-r i bosyl transferase. SIRT5 bes i des deacetylat i on, i t can also generate demalonylat i on and desucc i nylat i on. The presence of SIRTs has been reported to be present i n var i ous compartments i n the cell for example; SIRT1, SIRT6, and SIRT7 was lo-cated i n the nucleus, SIRT3, SIRT4, and SIRT5 i n the m i tochondr i a and SIRT2 i n the cytosol. Recents i nvest i gat i ons demonstrated act i ve i nvolvement of family of nicotinamide adenine dinucleotide (NAD+)-dependent deacetylases and is able to cat-alyze post-translational modifications of proteins. It has been documented that, SIRTs respond to metabolic challenges, inflammation molecules, and oxidative stress. Many studies have linked SIRTs with longevity and having anti-aging activity. Our knowledge of SIRTs in reproduction has grown increasingly over the last few years. The majority of these research is carried out primarily on the effects on SIRT1 on female reproduction. It has been demonstarted that down-regulating SIRT1 trigers the reduc- tion of ovarian reserve. Many research has demonstrated that, SIRT1 regulates proliferation and apoptosis in granulosa cells. Activity studies of SIRTs opened the discoveries of the functional aspects of different types of SIRTs in improving the overall quality of in vitro oocytes in humans and animal models. It has been documented that, SIRT1, SIRT2 and SIRT3 protect oocytes against postovulatory aging. The relationship be- tween derangement of SIRT signaling and the imbalance of reactive oxygen species and antioxidant defenses in female reproductive organs has also been documented. The present review aims to put forward informa-tion on the mechanism and cellular role of SIRTs and give an update of sirtuin research in female reproduction under physiological and patho-logical conditions. This study was carried out to evaluate new/late-detected congenital/ acquired scrotal pathologies during examination in children who applied for circumcision by making an appointment with a single physician. Material and Methods: Patients who applied for circumcision by making an appointment with a single physician from pediatric surgery outpatient clinic of University of Health Sciences, Zeynep Kamil Maternity and Children's Diseases Health Training and Research Center between January-September 2021 were evaluated retrospectively. Cases with new-detected scrotal pathology during the examination were included in this study. The data were uploaded to 20.0 SPSS statistical program, their descriptive values were determined. Results: The mean age of 150 children who applied for circumcision during the 8-month period was 7.3±3.1 (minimum: 1 month, maximum: 12 years) years. Scrotal pathology was detected in 15 (10%). Retractile testis was detected in 7 (4.7%), undescended testis in 6 (4%), hydrocele in 1 (0.7%), and in 1 (0.7%) testicle was lowered taut into the scrotum. The mean age was 6.83±2.32 (minimum: 3, maximum: 10) years for un-descended testis, 5±3.06 (minimum: 1, maximum: 11) years for retractile testis. The patient with hydrocele was 3 years old. Ultrasonography (USG)/Doppler USG was performed in 15 patients with scrotal pathology. Scrotal pathologies were on right-side in 10 (67%), on left-side in 4 (27%), bilateral in 1 (6%). Seven patients (46.67%, n=15) with undescended testis and hydrocele were operated;8 patients (53.33%, n=15) were followed up, circumcision operations were recommended to be performed after the pandemic. Conclusion: Scrotal pathology was noticed in 10% of the cases who applied for circumcision;4.7% required an operation.
