{"title":"The role of prostate inflammation in the pathogenesis of urethral strictures occurring after transurethral resections","authors":"Arif Aydın , Pembe Oltulu , Mehmet Balasar , Mehmet Giray Sönmez , Hakan Hakkı Taşkapu , Mehmet Serkan Özkent , Fahriye Kılınç","doi":"10.1016/j.androl.2020.10.009","DOIUrl":"10.1016/j.androl.2020.10.009","url":null,"abstract":"<div><h3>Objective</h3><p>To make an extensive evaluation about the effects of histopathological findings acquired from the resection materials of patients who underwent transurethral resection of prostate (TUR-P) due to benign prostate hyperplasia (BPH) on postoperative urethral stricture formation.</p></div><div><h3>Materials and methods</h3><p>Among patients who had TUR-P due to BPH and were followed up for minimum 6 months, 51 patients detected to have urethral stricture based on endoscopic imaging were included in the urethral stricture group (Group 1) and 52 patients without urethral stricture were included in the control group (Group 2). The relation between histopathological findings of TURP materials and postoperative stricture occurrence was investigated.</p></div><div><h3>Results</h3><p>No difference in age, prostate volume, operation time and postoperative catheterization time was detected among the groups (<em>p</em> <!-->=<!--> <!-->0.86, <em>p</em> <!-->=<!--> <!-->0.13, <em>p</em> <!-->=<!--> <!-->0.06, <em>p</em> <!-->=<!--> <!-->0.32, respectively). Average time until the urethral stricture diagnosis in the group with urethral stricture was measured as 57.9<!--> <!-->±<!--> <!-->27.2 days. In our study, inflammation intensity in peri-urethral, stromal and periglandular areas and intraglandular destruction ratios were higher in urethral stricture group (Group 1) (<em>p</em> <!-->=<!--> <!-->0.048, <em>p</em> <!-->=<!--> <!-->0.3, <em>p</em> <!-->=<!--> <!-->0.03, <em>p</em> <!-->=<!--> <!-->0.01, respectively). Again, it was detected that neutrophil, plasmocyte and eosinophil cell ratios were higher in peri-urethral, stromal and periglandular areas and lymphocyte values were lower compared to the control group.</p></div><div><h3>Conclusion</h3><p>Acquired data has shown that acute inflammatory attacks may be related to urethral stricture with a mostly chronic inflammation background in the prostate. Since the beginning of HIV infection in our country, prevention campaigns have been developed by the Health Services regarding sexual and reproductive health. Several authors warn about the poor evaluation of these campaigns.</p></div><div><h3>Objective</h3><p>To evaluate the design and evaluation strategies of the sexual health campaigns developed in Spain from 1987 to 2016.</p></div><div><h3>Methods</h3><p>Observational epidemiological study based on a detailed retrospective collection of data obtained from the National AIDS Plan, official agencies and Health Services about the design and evaluation of developed sexual health campaigns. Statistical analysis was performed using UNAIDS indicators system.</p></div><div><h3>Results</h3><p>82 campaigns have been developed since 1987, 27 have been aimed at young people. In 100% of campaigns aimed at young people, general information about HIV infection and the promotion of condom use has been addressed; however, other issues about risky sexual behavior have virtually not been included. The prevention of pregnancy in young people is present in less than 25% of campaigns. Reconstructive surgery with testicular prosthesis might mitigate this burden. We conducted a questionnaire-based study aiming to evaluate our patients’ satisfaction with testicular prosthesis. Overall satisfaction was rated as excellent or good in 97.7%. The main complaints were related to the prosthesis’ inappropriate texture (45.5%), size (18.1%) or position (15.9%). Among men interviewed, 59% considered that having a normal looking </span>scrotum was either extremely important or important for their self-esteem. The majority (88.2%) stated they would make the same decision again, and nearly all patients would recommend it to other men with testicular cancer. After removing the prosthesis, we wash the cavities with four dilutions. The 1st to 50% of hydrogen peroxide; the 2nd to 50% of povidone iodine; the 3rd with 1 g of cefazolin and 40 mg of tobramycin, the 4th with 80 mg of gentamicin and 500 mg of vancomycin. In the act we implanted a malleable prosthesis bathed in antibiotic solutions.</p><p>The postoperative period was satisfactory. Androgen deprivation therapy (ADT), which is the central component of advanced PCa treatment, causes side effects. The goal of this study was to examine the metabolic changes and bioelectrical impedance analysis differences in PCa patients who received ADT.</p></div><div><h3>Materials and methods</h3><p>After age-related match-pair analysis, a total of 519 patients with PCa and control group who had benign disease were enrolled in the study. Biochemical blood parameters and TANITA measurements were recorded for all patients. Patients were categorized into three groups, ADT group (Group 1, <em>n</em> <!-->=<!--> <!-->124) and non-ADT group (Group 2, <em>n</em> <!-->=<!--> <!-->248), control group (Group 3, <em>n</em> <!-->=<!--> <!-->147).</p></div><div><h3>Results</h3><p>The mean age of groups was similar. Body mass index, waist circumference, body fat mass and fat ratio, which were among the TANITA parameters, were higher in group 1 (<em>p</em> <!--><<!--> <!-->0.05). Total cholesterol, high density lipoprotein, non- high density lipoprotein, triglycerids and fasting blood glucose values were also higher in group 1 (<em>p</em> <!--><<!--> <!-->0.05). Myocardial infarction and metabolic syndrome rates were also higher in this group.</p></div><div><h3>Conclusions</h3><p>While the use of ADT is manifested by an increase in fat mass and fat ratio in body composition, it negatively affects waist circumference measurements. It is associated with metabolically unfit body composition changes that predispose to diabetes mellitus and may increase cardio-vascular disease. All the patients underwent transrectal and scrotal ultrasonography. The ultrasonographic features were observed of testis, epididymis, scrotal segment of vas deferens, seminal vesicle, ejaculatory duct and prostate. Eighty cases with normal semen were taken as control group.</p></div><div><h3>Results</h3><p><span>Among the 92 cases of acquired obstructive azoospermia, 28 cases were prostate midline cyst, 32 cases were stone or calcification of the ejaculation duct and 21 cases were chronic seminal vesicle inflammation, which were found through transrectal ultrasonography; 27 cases were vas deferens dilation, 30 cases had abnormal mass of epididymis tail, 31 cases were epididymis thickness with reticular change, 8 cases were cystic dilatation of rete testis, which were found through scrotal ultrasonography. Compared with the control group, ultrasound examination increased the detection rate of distal seminal duct lesions and epididymal lesions in acquired obstructive azoospermia patients (</span><em>p</em> <!--><<!--> <!-->.01). Comparison of testicular volume between case group and control group did not reveal significant difference (<em>p</em> <!-->><!--> <!-->.05). The thickness of the head, body and tail of epididymis in case group was significantly bigger than that in control group (<em>p</em> <!--><<!--> <!-->.01).</p></div><div><h3>Conclusion</h3><p>Transrectal and scrotal ultrasonography can find lesions in different parts of the seminal passage. Acquired obstructive azoospermia patients have increased rate of distal seminal duct lesions, epididymal lesions and epididymis volume. The interaction between the pudendal nerve and Onuf's nucleus may be important for developing orgasmic information. The study aims to investigate whether ischemic damage to Onuf's nucleus-pudendal network following spinal subarachnoid hemorrhage (SAH) causes taste bud degeneration or not.</p></div><div><h3>Methods</h3><p>The study was conducted on 22 fertile male rabbits who were divided into three groups: control (GI; <em>n</em> <!-->=<!--> <!-->5), SHAM (GII; <em>n</em> <!-->=<!--> <!-->5) and study (GIII; <em>n</em> <!-->=<!--> <!-->12). Isotonic solution, .7<!--> <!-->cm<sup>3</sup>, for the SHAM, and .7<!--> <!-->cm<sup>3</sup> homologous blood was injected into spinal subarachnoid spaces at S2 level of the study group. Two weeks later, Onuf's nucleus, pudendal ganglia and the taste bud-like structures of the penile urethra were examined histopathologically. Spinal SAH induced neurodegeneration in Onuf's nucleus, pudendal ganglia and taste bud atrophy was significantly different between GI/GII (<em>p</em> <!--><<!--> <!-->.005); GII/GIII (<em>p</em> <!--><<!--> <!-->.0005) and GI/GIII (<em>p</em> <!--><<!--> <!-->.0001).</p></div><div><h3>Conclusion</h3><p>Ischemic neuronal degenerations of Onuf's nucleus and pudendal ganglia following spinal SAH lead to genital taste bud-like structure atrophy. This mechanism may be responsible for sexual anhedonia and sterility in cases with spinal cord injury, which has not been documented so far.
