Recent years, biochemical mechanisms leads to male infertility are started to research. Redox regulation and </span>oxidative stress (OS) show importance in the pathogenesis of infertility in male. </span>Malondialdehyde<span><span> (MDA) and Glutathione (GSH) are biochemical indicatives of sperm damage by </span>reactive oxygen species<span> (ROS). In addition, sperm are coated with a thick glycocalyx rich in sialic acids. It is aimed to determine and evaluate the differences between normozoospermic and oligozoospermic individuals according to sialic acid, MDA and GSH concentrations and correlations between spermyogram and these parameters.</span></span></p></div><div><h3>Material and methods</h3><p><span><span>This study was carried out on seminal plasma of individuals who admitted to Selcuk University Faculty of Medicine IVF Unit </span>Andrology Laboratory. The groups were divided into two as normozoospermics (</span><em>n</em> <!-->=<!--> <!-->30, sperm concentration<!--> <!-->≥<!--> <!-->15<!--> <!-->million/mL), and oligozoospermics (<em>n</em> <!-->=<!--> <!-->30, sperm concentration<!--> <!-->&lt;<!--> <!-->15<!--> <span>million/mL). Spermyogram were evaluated regarding WHO (2010) Kruger criteria. GSH, MDA and sialic acid concentrations were analyzed in seminal plasma. Diagnostic performance of sialic acid has been determined with ROC curve analysis.</span></p></div><div><h3>Results</h3><p>Sialic acid levels were significantly lower in Normozoospermic than Oligozoospermic individuals (<em>p</em> <!-->&lt;<!--> <!-->0.0001), MDA and GSH levels were not differ significantly in both groups (<em>p</em> <!-->&gt;<!--> <!-->0.05). Sialic acid correlated significantly with most of the spermyogram findings. This study was conducted to determine whether the expression of fertilin β after intrauterine insemination (IUI) in donors with normal parameters after standard semen analysis is related to low success rate or failure of fertilization.</p></div><div><h3>Methods</h3><p>We examined the sperm of 30 male donors who have normal as controls, oligozoospermia, and unexplained infertility as the clinically indication for IUI. Fertilin β has been labeled with the ADAM2 antibody by indirect immunofluorescence (IF) assay. To evaluate the reproducibility of the test, we selected four sperm samples scale of 0 to +++ according to the distribution of fluorescence label.</p></div><div><h3>Results</h3><p>The results were highly correlated with the corrected total cell fluorescence (CTCF) (<em>R</em>_<em>p</em> <!-->=<!--> <!-->0.9972, <em>P</em> <!-->&lt;<!--> <span>0.05). We suggest that the relationship between infertility and fertilin β may be due to the distribution of this protein on the sperm surface. {"title":"Corporoplasty under local anesthesia: Cost-effective improvement of the surgical treatment of Peyronie's disease","authors":"Morales Martínez Ana,&nbsp;Funes Padilla Carlos,&nbsp;Melgarejo Segura Mª Teresa,&nbsp;López Carmona Pintado Fernando,&nbsp;Arrabal Martín Miguel","doi":"10.1016/j.androl.2021.02.007","DOIUrl":"10.1016/j.androl.2021.02.007","url":null,"abstract":"<div><h3>Introduction and objectives</h3><p>Peyronie's disease produces penile curvature that leads, in certain cases, to difficulties in having satisfactory sexual relations.</p><p>The objective of this study is to evaluate the functional and cost-effectiveness results obtained in the surgical treatment of penile curvature due to Peyronie's disease under local anesthesia, comparing them with those performed under a general–spinal anesthesia regimen.</p></div><div><h3>Material and methods</h3><p>Two groups of patients undergoing corporoplasty are compared according to the type of anesthesia used. Group 1 consists of 32 patients who underwent major outpatient surgery and under local anesthesia from June 2016 to June 2019. Their data are collected prospectively. Group 2 consists of 30 patients who underwent surgery under general/spinal anesthesia with hospital admission, from January 2013 to December 2015, with their data collected retrospectively.</p><p>Anesthetic and surgical procedure, postoperative functional results, degree of satisfaction and hospital costs between both groups are analyzed, considering <em>p</em> <!-->≤<!--> <!-->0.05 as statistical significance and analyzing the results with the SPSS 20.0 program.</p></div><div><h3>Results</h3><p>Of the 32 patients included in group 1, none required admission for intra or postoperative complications. In both groups, an improvement of the PDQ-test was observed without statistically significant differences, with the degree of global satisfaction above 95% in both groups. There were also no differences in the appearance of complications secondary to the anesthetic procedure or the hospital regime. We observed differences in hospital costs, being 44% lower for the group performed with local anesthesia.</p></div><div><h3>Conclusions</h3><p>Surgical treatment of penile curvature under local anesthesia improves the cost-effectiveness ratio with the same quality of care, degree of satisfaction and postoperative functional results, maintaining a similar rate of intra/postoperative complications. {"title":"The effects of wireless devices on male reproductive health: A literature overview","authors":"Lanceford Sterling ,&nbsp;Louis-Ray Harris ,&nbsp;Kamali Carroll","doi":"10.1016/j.androl.2020.10.004","DOIUrl":"10.1016/j.androl.2020.10.004","url":null,"abstract":"<div><h3>Purpose</h3><p>The aim of this study was to summarize the evidence of radiofrequency electromagnetic radiation (RF-EMR) exposure from wireless devices on total motile sperm count (TMSC) and identify gaps in the literature that could help clarify this link.</p></div><div><h3>Materials and methods</h3><p>A literature search was conducted using PubMed/MEDLINE to find relevant studies examining the effects of EMR on male fertility, with a specific focus on TMSC, published from 2000 to 2019. R was used for data analyses.</p></div><div><h3>Results</h3><p>Motility was identified as the parameter linked to TMSC that was most negatively impacted by EMR exposure. Many gaps were found including geographic and lack of standardization with EMR factors such as exposure time and operating frequency.</p></div><div><h3>Conclusion</h3><p><span>The EMR emitted by wireless devices may negatively affect TMSC, which is one of the better predictors of achieving pregnancies and impairs male fertility. In general, it is considered a self-limited disease that usually resolves with conservative management and very rarely requires surgical intervention. We report the case of a 41-year-old patient, who presented PMD which persists after medical treatment with </span></span>nonsteroidal antiinflammatory drug and </span>low molecular weight heparin. Surgery was decided and </span>thrombectomy plus resection of the superficial penile vein was performed with satisfactory results. All children were operated on using the incisional plication technique and were divided into two groups: Absorbable-polyglactine (PLG, <em>n</em> <!-->=<!--> <!-->23, 48.93%) and nonabsorbable-polypropilen (PP, <em>n</em> <!-->=<!--> <span>24, 51.06%) according to the suture material used for plication. Surgical outcomes were compared between groups.</span></p></div><div><h3>Results</h3><p>Mean follow-up period was 19.02<!--> <!-->±<!--> <!-->4.66 months. There was no significant difference between the mean age of the children in two groups included in the study (PLG<!--> <!-->=<!--> <!-->41.39<!--> <!-->±<!--> <!-->34.63 months vs PP<!--> <!-->=<!--> <!-->53.66<!--> <!-->±<!--> <!-->37.42 months, <em>p</em> <!-->=<!--> <!-->0.250). There was no significant difference in penile straightening degree between the two groups in the postoperative follow-up (PLG<!--> <!-->=<!--> <!-->27.39<!--> <!-->±<!--> <!-->6.88 vs PP<!--> <!-->=<!--> <!-->31.08<!--> <!-->±<!--> <!-->6.38, <em>p</em> <!-->=<!--> <!-->0.06). Similarly, there was no significant difference between two groups in terms of postoperative curvature recurrence (<em>p</em> <!-->=<!--> <span>0.681). There is a very rare relationship between the use of </span>alpha blockers<span> and the development of priapism. Here, we describe 2 cases of alpha blocker induced priapism and a literature review. One of these cases is related to the use of silodosin<span> and the other is related to the use of tamsulosin<span>. So far, 18 alpha blocker induced priapism cases have been reported. We are presenting the first case of silodosin induced priapism and the eighth case of priapism secondary to tamsulosin. Despite silodosin having a much greater affinity for the α1-a receptor than the α1-b receptor, as represented in this case it can cause this rare side effect. However, little is known about their impact on male and female gametes. Our study aimed to investigate the relationships between lipids levels in serum, follicular fluid and seminal plasma with ovarian response and sperm concentration regardless of age and body mass index (BMI).</p></div><div><h3>Methods</h3><p>51 follicular fluid and serum samples of IVF-ICSI cycles and 52 seminal plasma and serum samples of males in the infertility study were analyzed for cholesterol, triglycerides, and non-esterified fatty acids. The parameters used to assess gonadal response were number of mature oocytes in metaphase II and total motile sperm. Differences between groups were studied by means Principal Component Analysis, Kolmogorov–Smirnov test, Pearson correlation, Student's <em>T</em>, and multivariate linear regression.</p></div><div><h3>Results</h3><p>Using a multivariate linear regression model to exclude the effect of the age and BMI, we found that the lipid profile in follicular fluid and plasma influence inversely and significantly on ovarian response and the number of matured oocytes recovered. The aim of the study was to examine the psychometric properties of Massachusetts General Hospital-Sexual Functioning Questionnaire (MGH-SFQ) in that population.</p></div><div><h3>Materials and methods</h3><p>The sample was 634 Northeast Mexican elderly, selected by a non-probabilistic procedure. {"title":"Medium-long-term outcomes of saphenous vein graft in Peyronie surgery: Is there a need for new graft material?","authors":"Yavuz Güler","doi":"10.1016/j.androl.2020.12.009","DOIUrl":"10.1016/j.androl.2020.12.009","url":null,"abstract":"<div><h3>Aim</h3><p>We present our results of tunical incision and saphenous graft surgery for men with short penis and who cannot have sexual relations due to curvature linked to chronic Peyronie plaques.</p></div><div><h3>Introduction</h3><p>Peyronie disease is a chronic process where fibrotic plaques form in the tunica albuginea. The plaques cause pain in the acute period and cause the penis to bend after inflammation ends. Surgical procedures are required for curvature<!--> <!-->&gt;<!--> <!-->30°. Saphenous vein graft (SVG) replacement is an alternative method for tunical plaque incision defect.</p></div><div><h3>Material-method</h3><p>The outcomes for 71 patients with TI+SVG surgery were retrospectively assessed. Plaque features (curvature angle, location, direction of curve and tunical defect diameter), operative and postoperative properties (surgery duration, recurrent curvature, penis shortening, glans hypoesthesia, satisfaction score, follow-up duration), preoperative–postoperative penis length and IIEF-5 score differences were assessed.</p></div><div><h3>Results</h3><p>Mean age was 61.12<!--> <!-->±<!--> <!-->7.9 (45–75) years, mean curvature angle was 62.0<!--> <!-->±<!--> <!-->14.7 (50–90), location was mid-penile for 46 (65%), with angulation direction toward the dorsal for 37 (52%), and the defect area after tunical incision was 15.5<!--> <!-->±<!--> <!-->3.9 (10–23) cm². With mean follow-up duration of 37<!--> <!-->±<!--> <!-->9.4 months, 9 patients had re-curvature (13.2%), 8 had short penis (11.3%) and 7 had ED identified (9.4%).
