Cognitive function was assessed using the Montreal Cognitive Assessment (MoCA), with CI defined as a MoCA score < 26. IAH was identified using validated questionnaires (Clarke scale, Gold scale and Hypoglycaemia Awareness Questionnaire).

Results: The median MoCA score was 26 (23-28). Higher insulin dose (β = +2.92 and p = 0.012) was independently associated with better cognitive performance. In the interaction model, IAH (β = -1.54 and p = 0.003) and DPN (β = -1.18, p = 0.040) were independently associated with approximately 1.2-1.5-point lower MoCA scores.

Conclusions: In individuals with T1D, DPN and IAH, a history of SH, poorer metabolic control, older age and male sex are associated with CI. Overactivation of the mineralocorticoid receptor (MR) plays a crucial role in kidney damage by promoting inflammation, fibrosis, and functional deterioration. Finerenone, a third-generation highly selective non-steroidal MR antagonist (MRA), has emerged as a promising therapeutic option for CKD. In this review, we discuss the mechanisms through which MR overactivation contributes to renal damage, the structural and pharmacokinetic characteristics of finerenone, and compare it with first- and second-generation MRAs. We also focus on the safety and efficacy of finerenone in clinical applications for CKD. Notably, finerenone demonstrated a reduction in the risk of hyperkalemia during treatment, with its high selectivity minimising treatment-related hormonal imbalances, offering a novel perspective. Furthermore, we explore the promising future of finerenone in both diabetic kidney disease (DKD) and non-diabetic CKD, presenting updates from recent clinical trials and new insights into its potential role in these populations. In clinical practice, combining finerenone with sodium-glucose cotransporter-2 (SGLT-2) inhibitors and renin-angiotensin system (RAS) inhibitors has demonstrated strong cardiorenal protective effects. However, further clinical evidence is needed regarding direct combinations of finerenone with RAS inhibitors. Future studies on their synergistic effects may provide valuable insights. Ultimately, we hope this review offers new perspectives on finerenone's clinical applications and its future therapeutic prospects.
","PeriodicalId":48934,"journal":{"name":"Diabetes-Metabolism Research and Reviews","volume":"41 6","pages":"e70075"},"PeriodicalIF":6.0,"publicationDate":"2025-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144795835","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":2,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Notably, we observed significantly higher levels of serum estrone, whereas lower levels of corticosterone and estrone glucuronide as the course of T1DM prolonged; Glycated haemoglobin and fasting blood glucose levels were positively correlated with estrone, and negatively correlated with corticosterone and estrone glucuronide. This study aimed to explore an efficient method for early diagnosis of DPN by establishing a quantitative Neuropad.

Methods: We recruited 518 patients with type 2 diabetes. Neuropathy Symptoms Score (NSS) combined with Neuropathy Disability Score (NDS) was used to assess distal symmetrical peripheral neuropathy (DSPN). The area under the ROC curve (AUROC), sensitivity, and specificity were used to compare the diagnostic efficacy of quantitative Neuropad (the change rate of the chromatic aberration value per minute) and two types of visual Neuropad (visual Neuropad A: whether the time to complete colour change within 10 min, visual Neuropad B: the time to complete colour change) for DPN.

Results: We did not observe very good diagnostic efficacy of Neuropad (visual Neuropad A and B: 0.59 and 0.64, quantitative Neuropad AUROC: 0.62-0.64) when using standard DSPN diagnostic criteria (NDS 6-12 or NDS 3-5 combined with NSS 5-9). When DPN was assessed by NSS + NDS ≥ 4, visual Neuropad B improved the specificity (AUROC 0.72, 67.00%, specificity 71.70%) by extending the detection time compared with visual Neuropad A (AUROC 0.62, sensitivity 81.80%, specificity 41.70%). Quantitative Neuropad significantly improved the diagnostic effect (AUROC 0.81, sensitivity 80.0%, specificity 76.3%) and reduced the detection time (4 min).

Conclusions: This study provides a new quantitative Neuropad, which has great potential to be an extremely useful diagnostic tool for early screening of sudomotor dysfunction in the clinical practice.
","PeriodicalId":48934,"journal":{"name":"Diabetes-Metabolism Research and Reviews","volume":"40 8","pages":"e70010"},"PeriodicalIF":8.0,"publicationDate":"2024-11-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"142668848","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":2,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
