The aim of this review was to summarize the literature regarding the definition of \"symptomatic\" considering also new diagnostic tools recently available.Methods: A systematic search was conducted according to PRISMA using PubMed, Cochrane Library and Scopus databases for the terms: \"symptomatic flat foot and children\". Reported symptoms at clinical examination and evaluation methods were extracted.Results: A total of 15 different symptoms were reported: pain (n = 24), dysfunction (n = 16), functional impairment (n = 8), reduced quality of life (n = 6), altered kinetic chain (n = 5), daily activities limitation (n = 5), fatigue (n = 4), sport limitation (n = 3), skin lesions (n = 3), general pain and fatigue (n = 3), tight Achilles tendon (n = 2), gait abnormalities (n = 2), others (n = 3). Eleven reported new different diagnostic tools: gait analysis (n = 7), pedobarography (n = 3), podoscopy (n = 2).Conclusions: Beyond pain, fourteen additional clinical features have been described. Patient reported outcome measures (PROMs) collected were Patient-Reported Outcomes Measurement Information System (PROMIS) Pain Interference, Physical Function, Global Health, and the Foot and Ankle Disability Index. Patients who completed surveys at all timepoints (preoperatively and 6- and 12-months postoperatively) were included. Generalized estimating equations estimated mean changes in PROM scores; multivariable models assessed the impact of clinical variables.Results: Twenty-seven patients met inclusion criteria with median age of 59 years and median body mass index (BMI) of 36. Patients demonstrated a significant decrease in Pain Interference at 6 months, but this improvement was not sustained at 12 months. All underwent diagnostic ankle arthroscopy and open stabilization based on standardized intraoperative criteria for lateral, syndesmotic, and deep deltoid instability; combined patterns were classified. PROs and complications were recorded.Results: Twenty-nine patients (30 ankles; 36.1 years, BMI 31.2 kg/m2) after RAI were included, mean 45.2-month follow-up. Instability patterns were multidirectional (lateral/deltoid/syndesmotic) in 60%, rotational (lateral/deltoid) in 20%, anterolateral (lateral/syndesmotic) in 13%, and anteromedial (deltoid/syndesmotic) in 3.5%; isolated lateral occurred in 3.5%. Collected PROs showed improvement (ps<0.04) and three complications occurred (10%).Conclusion: In symptomatic patients after RAI, multi-ligament ankle instability predominated, most commonly multidirectional or rotational patterns. Intraoperative-guided comprehensive stabilization yielded PRO improvements with a low complication rate.Study design: Level II. As inflammation is a key driver of pain in accessory navicular syndrome (ANS), TAME may be a therapeutic option for patients refractory to conservative management.Methods: We retrospectively analyzed 75 patients (36 females; mean age, 23 ± 15 years) with refractory ANS treated with TAME (Sep 2020-Aug 2024). Adverse events were recorded. Outcomes were assessed using Numeric Rating Scale (NRS) and European Foot and Ankle Society (EFAS) scores.Results: No major complications were observed. Mean NRS improved from 6.9 ± 1.5 at baseline to 1.6 ± 2.1 at 12 months and 1.4 ± 1.9 at final assessment (mean 31 ± 13 months) (p < 0.001). Revision arthrodesis was assessed through November 20, 2025. Demographics, implant type, bilaterality, and comorbidities were compared between revised and non-revised patients.Results: Among 901 patients, 192 underwent revision arthrodesis, yielding an observed cumulative revision rate of 21.3%. Revisions occurred frequently within 24 months. Total arthroplasty had a higher revision rate than hemiarthroplasty (23.4% vs 15.5%) and remained independently associated with revision after adjustment. This study evaluated the prevalence and clinical relevance of CT-detected syndesmotic malreduction in patients with radiographically acceptable postoperative reduction.Materials and methods: Adult patients undergoing ankle fracture fixation with syndesmotic stabilization and acceptable postoperative radiographs were prospectively enrolled. All patients underwent early postoperative bilateral CT assessment with side-to-side comparison of translational and rotational parameters. Patients were stratified into anatomical reduction, minor malreduction, and major malreduction groups. Functional outcomes were assessed at 24 months using the AOFAS ankle-hindfoot score.Results: Eighty-six patients were included. CT detected syndesmotic malreduction in 52.3% of patients despite acceptable radiographs. Mean AOFAS scores progressively decreased from the anatomical reduction to the major malreduction group (91.3 ± 5.9 vs 83.5 ± 3.5 vs 74.4 ± 3.2; p < 0.001). Both minor and major malreduction were independently associated with worse functional outcomes.Conclusion: Postoperative radiographs may underestimate syndesmotic malreduction.
