Results of an online survey in a diabetes web site","authors":"Ignacio Conget , Dalia Ávila , Marga Giménez , Carmen Quiros , Vanesa Salaverria , Belen Dueñas","doi":"10.1016/j.endoen.2016.02.008","DOIUrl":"https://doi.org/10.1016/j.endoen.2016.02.008","url":null,"abstract":"<div><h3>Objective</h3><p>To assess the frequency of impaired awareness of hypoglycaemia (IAH) using a specific questionnaire (Spanish version) in a free access diabetes-related web site.</p></div><div><h3>Methods</h3><p>Data from a free access Spanish version of the Clarke test previously uploaded to the website of the <em>Fundación para la Diabetes</em> (March 2014–January 2015) were assessed. In addition to the eight questions in Clarke's questionnaire, information on type of diabetes, age, and disease duration was obtained. The Clarke test divided participants into three categories: normal awareness, uncertain and IAH.</p></div><div><h3>Results</h3><p>Of the 418 participants with type 1 diabetes, 51.2% were aged 36–55 years. In 34.7%, diabetes had been diagnosed >15 years before, while disease duration was <2 years in 11%. According to Clarke categories, 23.4% had IAH, 15.3% uncertain awareness, and 61.3% normal awareness. The longer the duration of diabetes, the higher the Clarke test score. According to the Clarke test, 14.1% of participants had experienced at least one episode of severe hypoglycaemia in the previous year, and half of these (7.4%) had suffered severe hypoglycaemia two or more times. The questionnaire was sent to members of the Spanish Society of Diabetes and the Spanish Society of Endocrinology and Nutrition.</p></div><div><h3>Results</h3><p>Eighty-seven questionnaires were received from 81 hospitals, 4 outpatient specialty centers, and 2 primary healthcare centers, which accounted for 51% of the Spanish population and for 39% of births in 2013. GD was mainly diagnosed based on GEDE recommendations (98%), and less than 50% of women were reevaluated after delivery in primary care. Fourteen (26%) of the 53 centers identified as PDUs corresponded to a minimal model. Continuous subcutaneous insulin infusion (CSII) therapy was not available in 30% of centers, and 13% of hospitals had no preconceptional clinics. No nurse support was available in 20% of centers.</p></div><div><h3>Conclusions</h3><p>Care of women with PGD has a fair coverage with PDU, but significant deficits still exist, for instance, in preconception clinic and CSII. However, organization of care for women with GD appears to be adequate. Patients under 18 years with T1DM were enrolled by consecutive, nonrandomized sampling. Data collected included sex, age, pubertal stage, time since T1DM onset, weight, height, body mass index (BMI), waist circumference, </span>glycosylated hemoglobin (HbA1c), 25-OH-D, total cholesterol (TC), </span>LDL cholesterol (LDL-C), </span>HDL cholesterol (HDL-C), and triglycerides (TG). Results were stratified by sex, age, and pubertal stage. Data were analyzed using SPSS</span><sup>®</sup>.</p></div><div><h3>Results</h3><p>Ninety patients with a mean age of 11.7<!--> <!-->±<!--> <!-->3.6 years (51.1% males) and mean HbA1c levels of 7.5<!--> <!-->±<!--> <!-->1.3% were enrolled. Of these, 26.6% had 25-OH-D levels <20<!--> <!-->ng/mL and 13.3% 25-OH-D levels ≤15<!--> <!-->ng/mL. No differences were found in 25-OH-D between patients with overweight or obesity and the rest. HDL-C levels <40<!--> <!-->ng/mL were found in 1.1%, 34.4% had LDL-C levels ≥100<!--> <!-->mg/dL, and 2.2% had TG levels ≥150<!--> <!-->mg/dL. Patients with 25-OH-D <20<!--> <!-->ng/mL had higher TG levels than the rest (76.80<!--> <!-->±<!--> <!-->45.62 vs 57.55<!--> <!-->±<!--> <!-->26.08; <em>P</em> <!-->=<!--> <!-->.04) in the multivariate analysis controlled for BMI, waist circumference, and HbA1c. A correlation was found between 25-OH-D and TG levels (−0.230; <em>P</em> <!-->=<!--> <!-->.029).</p></div><div><h3>Conclusions</h3><p>Patients in our population with vitamin D deficiency had higher TG levels. Medical history, clinical parameters, and glucose, insulin, and lipid levels were collected. EAT thickness was measured using transthoracic echocardiography. Study subjects were divided into two groups based on whether they had less than two or two or more CRFs.</p></div><div><h3>Results</h3><p>The group with two or more CRFs had higher EAT thickness, insulin, and HOMA-IR values (<em>p</em> <!--><<!--> <!-->0.05). EAT thickness showed a statistically significant positive correlation with body mass index (BMI) (<em>r</em> <!-->=<!--> <!-->0.561, <em>p</em> <!-->=<!--> <!-->0.0001), waist circumference (<em>r</em> <!-->=<!--> <!-->0.549, <em>p</em> <!-->=<!--> <!-->0.0001), systolic blood pressure (SBP) (<em>r</em> <!-->=<!--> <!-->0.256, <em>p</em> <!-->=<!--> <!-->0.028), insulin (<em>r</em> <!-->=<!--> <!-->0.408, <em>p</em> <!-->=<!--> <!-->0.0001), and HOMA-IR (<em>r</em> <!-->=<!--> <!-->0.325, <em>p</em> <!-->=<!--> <!-->0.005). However, these correlations were not significant after adjustment for BMI. The cut-off point for EAT thickness as predictor of two or more CRFs was 3.17<!--> <!-->mm. The risk (odds ratio) of having two or more CRFs if EAT thickness was >3.17<!--> <!-->mm was 3.1 (95% CI: 1.174–8.022). Many barrier and immune cells have membrane and </span>intracellular receptors<span><span> that recognize different microbial antigens. Activation of these receptors induces synthesis of 1α-hydroxylase<span>, which acts on 25 hydroxyvitamin D to generate intracellular calcitriol. Calcitriol activates its receptor and enhances the synthesis of important human antibiotics like cathelicidin<span> and β2-defensin while inhibiting hepcidin. Because of this, and due to its chronic complications, T2DM causes high human, social, and financial costs. In addition, the elderly population with T2DM has a marked clinical heterogeneity. Therefore, our main objective was to analyze the relationship of age with the clinical and biological manifestations of the disease and the prevalence of chronic complications in patients with T2DM.</p></div><div><h3>Material and methods</h3><p>A cross-sectional study of a large population with T2DM (n<!--> <!-->=<!--> <!-->405) randomly selected from a Diabetes Unit and 2 health care centers (60%). The clinical, anthropometric, and biochemical variables of the subjects were collected using standard methods to assess the effect of age on the clinical and biochemical phenotype of patients with T2DM.</p></div><div><h3>Results</h3><p>We have noted that patients with T2DM >70 years old have a clinical and biochemical phenotype different from younger subjects (<60 years) including longer times since diabetes onset, higher diastolic blood pressure levels, and lower body mass index (BMI) values. As regards to biological variables, these patients have lower triglyceride levels, impaired kidney function, and lower HbA1c values. Prevalence of metabolic syndrome is lower in patients with T2DM >70 years of age. Age was inversely related to parameters associated to metabolic syndrome (BMI, waist circumference, blood pressure, and triglyceride levels).</p></div><div><h3>Conclusions</h3><p>We have defined the clinical and biochemical profile of patients with T2DM >70 years attending health care centers.
