An online self-administered questionnaire was used, consisting of 33 questions assessing the participants’ socio-demographics, socio-academics, intentions to apply for the USMLE step 1 examination and their attitudes toward the conversion of USMLE step 1 to pass/fail scoring system. \u0000Results: A total number of 504 medical students completed the survey. Among participating students, 44.4% agreed that USMLE should be pass/fail, while only 26.8% were against it. Furthermore, 82.3% of the students agreed that by this change, Step 2 CK will carry more weight in selecting applicants for residency. Bevacizumab, a humanized anti-vascular endothelial growth factor monoclonal IgG1 antibody, is a Food and Drug Administration-approved agent for the treatment of advanced Glioblastoma multiforme. In this review, we aimed to discuss the therapeutic effects of bevacizumab for Glioblastoma multiforme treatment. \u0000Methods: We searched Google scholar, PubMed, and Scopus using keywords “Glioblastoma multiforme,” “GBM,” and “Bevacizumab.” Two authors screened the records to identify relevant studies and classify them according to our outcomes of interest. \u0000Results: Bevacizumab selectively binds circulating VEGF, interfering with the role of VEGF in endothelial cell differentiation, sprouting, and capillary formation. Consequently, it inhibits tumor neovascularization and induces the development of normal vascular structures \u0000Conclusions: Our review summarized some factors that may maximize the effectiveness of the drug at the lowest possible cost. We mention limitations of bevacizumab use and put forward solutions. This study aims to describe the extent of IBD diagnosis delay in Jordan. \u0000Methods: Our study included 110 IBD patients, utilizing interviews and medical records from Jordan University Hospital. Data covered sociodemographic characteristics, health history, time from symptom onset to diagnosis, time from the first consultation to diagnosis, number of physicians consulted before diagnosis, and IBD-related surgical history. \u0000Results: The median time from symptom onset to IBD diagnosis was 9 months. Median diagnosis times for Crohn's disease (CD) and ulcerative colitis (UC) were 11 and 9 months, respectively. Regression analysis revealed that longer diagnosis delays were associated with higher paternal education levels. Lower maternal education levels (high school or less) and a household income between 1200 and 2000 Jordanian dinars correlated with consulting more physicians before diagnosis. \u0000Conclusions: Early aggressive treatment is essential for IBD, as delayed diagnosis negatively impacts patient outcomes. Further research is needed to identify factors contributing to diagnosis delay. {"title":"Persistent hypoparathyroidism following thyroidectomy: Analysis of 276 patients.","authors":"Mutaz Albrezat, Erhan Aysan","doi":"10.58877/japaj.v2i2.140","DOIUrl":"https://doi.org/10.58877/japaj.v2i2.140","url":null,"abstract":"ABSTRACT \u0000Aim: To explore the attributes of individuals who have developed permanent hypoparathyroidism following thyroidectomy.Method: A 276 cases diagnosed with permanent hypoparathyroidism (PHP) after thyroid surgery were evaluated retrospectively.Results: 98% had total thyroidectomy, 70% had no lymph node dissection, 13% operated by endocrine surgeons, and 24% by an experienced surgeon. In 85% of the patients, PHP developed after their first operation. 63% of the patients had benign disease. No parathyroid glands were found on the final pathology report in 68% of the cases. The operations were performed at university hospitals in 20% of patients. Central lymph node dissections (CLND) were conducted on 27% of patients. Among these, 59% were performed by surgeons with under 5 years of experience.Conclusion: The onset of PHP after bilateral thyroidectomy is influenced by a myriad of factors. Our study observed a trend linking the likelihood of developing PHP to the surgeon’s specialization, experience, the extent of surgery, operations performed in non-academic hospitals, and the conduct of CLND. Notably, even when performed by experienced surgeons, CLND might elevate the risk of this complication. This trend underscores the need for greater awareness about the implications of PHP. Medical students are particularly susceptible to sleep problems due to demanding schedules that potentially affect their cognitive performance and academic achievement. This study aimed to explore the relationship between sleep quality, psychological distress, and academic performance among medical students in Jordan. Methods: A self-reported cross-sectional survey was conducted, targeting medical students from six Jordanian universities. Participants completed a questionnaire including demographics, sleep quality using the Pittsburgh Sleep Quality Index (PSQI), psychological distress using the Kessler Psychological Distress Scale (K10), the academic performance of the past year, and other like studying related factors. Statistical analyses used descriptive and Chi-square tests to explore the associations between the studied variables. Results: The study involved 707 participants, predominantly females (62.8%), with the majority (38.5%) aged between 21 and 24 years old. A high prevalence of poor sleep quality was observed (74.4%), with a mean PSQI score of 8.16 ± 3.67. Psychological distress was prevalent (77%), with 36.5% of participants experiencing severe distress. The global score of PSQI did not show a significant association with the stress overall score(P-value = 0.6). However, the K10 distress score was significantly associated with all components of the PSQI scale except for component 6. Moreover, K10 score was significantly associated with Grade Average Points (GPA) and gender. Conclusion: This study highlighted the substantial prevalence of poor sleep quality and psychological distress among medical students in Jordan. It emphasizes the interconnectedness of sleep quality, psychological well-being, and academic performance. Although global PSQI scores did not correlate with psychological distress, various sleep quality components were associated with psychological distress and academic performance indicators. {"title":"Midgut volvulus with cavernous transformation of the portal vein in a middle-aged patient: a case report","authors":"Obada Nayef Al-leimon, Abdallah Hamdan, Ahmad Al-leimon, Mohammad Rashdan","doi":"10.58877/japaj.v2i2.166","DOIUrl":"https://doi.org/10.58877/japaj.v2i2.166","url":null,"abstract":"Introduction: midgut volvulus and cavernous transformation of the portal vein (CTPV) are rare conditions in adults. congenital anomaly of the intestines that can cause midgut volvulus, a life-threatening intestinal obstruction, in adults with nonspecific symptoms and diagnostic challenges. Case Presentation: We report a case of A 46-year-old male who had severe central abdominal pain, vomiting, and obstipation for three days, indicating intestinal obstruction. The CT without contrast revealed midgut malrotation with small bowel obstruction due to volvulus. The post-operative CT with contrast findings reveals the presence of multiple collaterals of the portal vein, which is suggestive of cavernous transformation. The procedure: During the procedure, intraoperative findings revealed a Ladd band, the DJ junction in the right upper quadrant, and a gangrenous bowel segment measuring approximately 30 centimeters in length and located 50 centimeters distal to the DJ junction. The surgical approach included a Ladd's procedure, lysis of the band, resection of the gangrenous section, with a side-to-side anastomosis, and an appendectomy. Conclusion: A unique case study presents an adult with two uncommon conditions—midgut volvulus and cavernous transformation of the portal vein. Their occurrence is rarely seen in adults, requires a multidisciplinary approach for diagnosis and management, emphasizing early detection and intervention to prevent severe complications.
