The concept of microbiome is a collective term for many microorganisms, such as bacteria, archaea, fungi, protists, and viruses, inhabiting various anatomical areas of the human body. The relevance of studying their quantitative and qualitative relationships is associated with the size of microbial populations, as well as the lack of awareness of the modern scientific community about the true role of most of them, including the connection between the microbiome diversity of male testicles and sperm with the presence of azoospermia and the development of idiopathic variants of infertility. The purpose: identification of the composition of the microbiome of testicular tissue in infertile men and determination of its clinical significance. Studying the influence of microbial populations of the urogenital tract of men on the development and course of diseases associated with infertility, such as azoospermia. Materials and methods. A literature review was performed based on data published in the PubMed (https://www.ncbi.nlm.nih.gov/pubmed/) and Scientific Electronic Library eLibrary.ru (https://elibrary.ru/) databases. The search was carried out using the following keywords: microbiome; microbiota; testicular tissue; azoospermia; sequencing; genome; male infertility. A total of 167 sources were analyzed. For this publication, 48 publications were selected. Results. Lactobacillus iners, Gardnerella vaginalis, Escherichia faecalis, Escherichia coli and Staphylococcus aureus are associated with the development of non-obstructive azoospermia. In men with different forms of azoospermia, a greater number of Bacteroidetes and Firmicutes phyla were observed, while Proteobacteria and Actinobacteria were significantly less common compared to healthy controls. Also, the microbial diversity between obstructive azoospermia (OA) and non-obstructive azoospermia (NOA) had a higher degree of similarity than the control group. Interestingly, the microbiome modifications found in men with NOA are similar to those previously identified in the gut of older adults, which can be considered as further evidence of microbiologically early aging of NOA men at the testicular level. Conclusions. Microbiome analysis demonstrated for the first time that human testicular tissue is not a microbiologically sterile environment, and also provided new data on the bacteriological composition of the male urogenital tract and allowed us to consider excluding any potential contribution of microbial communities present in neighboring anatomical regions. Therefore, larger studies are needed to confirm our preliminary findings that the testes of azoospermic men contain a more dysbiotic bacterial community.","PeriodicalId":474770,"journal":{"name":"Экспериментальная и клиническая урология","volume":"34 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135924587","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Mechanical kidney injuries lead to renal dysfunction and vasospastic reaction, resulting in adaptive reactions of the organism, with the possibility of formation of secondary arterial hypertension. Purpose of the study: to determine the types of central gemodynamics, intrarenal blood flow characteristics, correlation interrelations of indices and to estimate the risks of secondary posttraumatic arterial hypertension development after Grade I-III kidney injury. Material and methods. Systemic hemodynamic parameters with identification of blood circulation type and intrarenal hemodynamic parameters were studied in 196 male victims in the posttraumatic period, the control group consisted of 30 men. During statistical processing Pearson correlation coefficients (r) and linear regression coefficient of determination (r2) were additionally calculated. Results. After kidney injury, the organ blood flow velocity was increased from 12,5% to 31,2%, resistance index (RI) increased from 4,7% to 44,8%, and pulsation index (PI) decreased from 9,2% to 41,2%. The indices of intrarenal vascular resistance were in negative from very strong to medium strength correlation with the parameters of systemic hemodynamics in the following gradation structure by decreasing strength: total peripheral resistance → diastolic blood pressure → systolic blood pressure → mean arterial pressure → stroke volume → cardiac index → minute volume of blood. Hypokinetic type steadily prevailed in central hemodynamics, the share of eukinetic type increased, and hyper-kinetic type of hemodynamics remained at a constant level. Conclusion. After kidney injury of non-critical severity the ascertained parameters of systemic and organ hemodynamics do not possess predictive power for realization of direct causal relationship in the development of secondary renoparenchymatous arterial hypertension.","PeriodicalId":474770,"journal":{"name":"Экспериментальная и клиническая урология","volume":"78 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135924730","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Alpha1-blockers previously have been considered as essential antihypertensive drugs with relatively good results in long-term administration. This effect on metabolism has been noted, in particular on the lipid spectrum and glycemic profile. However, at present, drugs of this group are not considered even in the reserve of basic metabolic therapy. The aim of our study was to study the effect of reducing insulin resistance in patients with long-term administration of a uroselective alpha1-blocker (Tamsulosin) in combination with vitamin-like drugs. Materials and methods. Under our supervision there was a group of patients of 63 people, which included 36 men and 27 women. The mean age in the observation group was 60 ± 1.56 years, ranging from 29 to 81 years. An original study design was developed, which involved combination therapy in combination with alpha-lipoic acid, L-carnitine and nootropics instead of monotherapy with one alpha1-blocker. Results. The study showed that with an average duration of therapy of 16 ± 2 months, an improvement in biochemical parameters of glucose metabolism was noted in 61% of patients. In the general observation group, there was a decrease in blood glucose (7.01 ± 0.29 → 6.32 ± 0.19, р = 0.004), plasma insulin (21.33 ± 1.75 → р=0.0007) and C-peptide (4.00 ±0.30 → 3.15 ±0.19, p=0.0003), glycated hemoglobin (5.99 ±0.17 → 5.90 ±0 .10, р=0.25). Similar changes are also observed in the subgroup of men and the subgroup of women. In patients under 60 years of age with a duration of therapy of 11 ± 3 months, deviations of the glycemic profile from the norm were less pronounced, while the correction of insulin resistance was carried out more effectively, in the subgroup of patients over 60 years of age, more therapy time was required (20 ± 3 months), but still , insulin resistance decreased less pronounced than in the subgroup younger than 60 years. Conclusions. The results of combined metabolic therapy support the hypothesis that alpha1-blockers have the properties of drugs with metabolic action in long-term administration, and in combination with vitamin-like drugs, such treatment can be considered as an alternative option to reduce insulin resistance and improve follow-up outcome.","PeriodicalId":474770,"journal":{"name":"Экспериментальная и клиническая урология","volume":"4 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135867755","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Currently, percutaneous nephrolithotripsy is the main treatment for large and coral-like kidney stones in children. The purpose of the study was to increase the effectiveness of percutaneous nephrolithotripsy (PCNL) in children using miniature-sized instruments. Materials and methods. For the period from 2008 to 2019, 320 children aged 1 to 17 years underwent nephrolithotripsy in the pediatric department of the N. Lopatkin Scientific Research Institute of Urology. Depending on the size of the instruments used, patients were divided into 3 groups. The first group included children who underwent surgery using the Mini-PCNL technique (n-211), the second group included patients operated on using the Ultra-Mini-PCNL method (n-16) and the third group included children whose percutaneous intervention was performed using a standard-sized nephroscope (n-93). The intervention was carried out according to the standard method; all patients underwent cystoscopy, catheterization of the ureter, access formation, lithotripsy, lithoextraction and installation of nephrostomy drainage. Results. In the first group of children studied, complete elimination of clinically significant stones (residual stones smaller than 4 mm) after the initial intervention was achieved in 169 (80.1%) pediatric patients. In the second group, there was 13 (81.2%) children. And in the third group, getting rid of stones was achieved in 73 (78.5%) patients. The average time required to perform surgery in all three study groups was comparable and amounted to 45 (40-125) minutes in the first group, 40 (25-85) minutes in the second, and 50 (40-180) minutes in the third group. The final efficacy of percutaneous nephrolithotripsy in children using instruments of various sizes in the first group was 89.6%, in the second – 93.7% and 88.1% in the third study group. The hospital stay after surgical treatment was: 4 (3-12) days for the first group, 3.5 (3-7) and 4.5 (3-17) days for the second and third groups, respectively. The need for additional interventions after PCNL in pediatric patients was in 69 (32.7%) children in the first group, 4 (25%) in the second and 41 (44.1%) in the third group. Conclusion. The use of mini-PCNL, ultra-mini-PCNL can reduce the duration of surgical intervention by 10% and 20%, the hospital stay after PCNL by 11.2% and 22.3%, the need for additional interventions by 11.4% and 19.1%. Can reduce the total number of complications by 10% and 12.2%, respectively, compared with the use of standard-size instruments. But how valid is it, what pathology does it describe and what are we treating? Most often it is established in patients with recurrent lower urinary tract infection (rLUTI), long-term dysuric disorders or chronic pelvic pain syndrome (CPPS). In most cases, it serves as a kind of «last resort» for the practitioner to find finally the cause of persistent complaints of the patient. In this «clinical trash» is thrown all that can potentially be associated with the bladder and is not treated by standard methods defined by clinical guidelines and generally accepted algorithms. At the same time, these are complex patients with a long history and multifactorial pathogenesis, requiring careful examination and thoughtful personalized approaches to treatment. Only such an attitude to the problem makes it possible to find the true cause of the patient's complaints. To date, the questions of the origin and treatment of patients with «leukoplakia of the bladder» remain very controversial. Unfortunately, the lack of knowledge in this matter often serves as a reason for unfair speculations and unjustified surgical activity. Metaphylaxis of urolithiasis is an important link in the effectiveness of treatment of patients suffering from urolithiasis. However, this direction in the Russian Federation has not become widespread and is implemented, as a rule, only in large medical centers. To identify the reasons for this, an assessment of the existing regulatory framework for the possibility of urolithiasis metaphylaxis in state institutions of the Russian Federation was carried out Materials and methods. The normative legal documents regulating the provision of medical care to patients with urolithiasis in the Russian Federation are analyzed. Results. Regulatory legal acts regulating the provision of medical care to patients with urolithiasis allow for the prevention of urolithiasis at the regional level. The development and introduction of clinical guidelines on urolithiasis as the main regulatory document has greatly simplified the work of the urologist. It is necessary to further coordinate the standards of medical care and quality criteria for its provision with clinical recommendations for urolithiasis. All citizens of the Russian Federation, as well as stateless persons insured in the insurance system, have the right to receive a metabolic profile assessment for urolithiasis at the expense of budgetary funds. Varicocele is detected in 15% of healthy men, in 35% – with primary and in 80% – with secondary infertility. At the moment, microsurgical varicocelectomy by sub-inguinal access is one of the most effective ways to treat varicocele in adult patients. Currently, there is no comprehensive data on the optimal treatment of varicocele recurrence. According to Russian clinical guidelines, testicular vein thrombosis is an extremely rare cause of varicocele development in the practice of a urologist. Material and methods. From November 2018 to February 2023, 118 patients were operated on for varicose veins of the scrotum organs. For all patients with the underlying disease, surgical aid was performed in the amount of micro-surgical subinguinal dissection of the testicular veins from the affected side (sides), or Marmara surgery. Results. The average duration of the operation was 46±11.3 minutes. In all the subjects, the period of hospitalization did not exceed one day. Complications in the postoperative period (according to the Cavien-Dindo scale) were not detected in any of the patients. The severity of the pain syndrome was determined by the VAS scale, and amounted to 0.78 ± 0.6 points. The average dose of non–narcotic analgesics (ketorolac, single dose – 30 mg) was 7.38 ± 3.63 mg. Recurrence of varicocele after microsurgical subinguinal dissection of testicular veins was recorded in 1.7% of patients. Conclusions. Marmara surgery is an effective method of surgical treatment of varicocele in spontaneous thrombosis. Recurrence of varicocele after microsurgical sublingual dissection of the testicular veins was recorded in 1.7% of patients. Depending on the preparation used with prostate extract in the complex therapy of CP, patients were divided into 5 groups. Preparations with prostate extract were used for 20 days. The effectiveness analysis included an assessment of the dynamics of the clinical picture, the results of laboratory and instrumental research methods, as well as questionnaire data on valid questionnaires I-PSS (International System for the Summary Assessment of Prostate Diseases, Lower Urinary Tract Symptoms (LUTS)), IIEF-15 (International Index of Erectile Function), NIH Chronic Prostatitis symptom Scale- CPSI. Results. The analysis of the results showed that the use of Vitaprost® in the complex therapy of СР is accompanied by a decrease in the number of leukocytes in the analysis of prostate secretion, positive dynamics in relation to the total score of the severity of pain syndrome and the patient's assessment of his condition on the NIH-CPSI scale of symptoms of chronic prostatitis, which may indicate a decrease in the inflammatory process in the prostate in general. The effectiveness of therapy in relation to erectile function on the IIEF-15 scale, reduction in the severity of symptoms on the IPSS scale and positive dynamics in relation to urination parameters was also noted. Conclusion. According to the results of the work carried out, it was the use of Vitaprost® that was accompanied by a significant decrease in the symptoms of CP and an improvement in the quality of life of patients, as well as erectile function of patients, an increase in the flow rate of urine, a decrease in the volume of residual urine and intensiveness of inflammation.","PeriodicalId":474770,"journal":{"name":"Экспериментальная и клиническая урология","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-09-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135924582","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
