Caregivers completed a sociodemographic questionnaire, and children underwent dental examinations to assess caries, gingival health, and oral hygiene. Anthropometric measures-weight, height, and body mass index (BMI)-were obtained and categorized into percentiles (<10, 10-90, >90) using age- and gender-specific growth charts. Dental caries was highly prevalent (93.5%), with a mean dmft/DMFT value of 7.70 (SD ± 5.22). The mean gingival index was 0.43 (SD ± 0.52). Total dmft/DMFT values were significantly higher in children ≤6 years and those with fathers of lower educational levels. Poor oral hygiene was a strong predictor of adverse outcomes for both caries and gingival health. Higher gingival indices correlated with children in the >90th weight percentile.</p><p><strong>Conclusion: </strong>The findings highlight the need for comprehensive oral health interventions that integrate meticulous oral hygiene, paternal education, and holistic growth monitoring beyond BMI. The results support implementing a 3-month recall program for professional dental care, combined with individualized oral hygiene instruction for caregivers. Participants were aged between 3 and 17 years, matched by sex and age. Antonovsky's SOC questionnaire (SOC-13) was answered by mothers from both groups, characteristics and the oral status of the children were investigated. Clinical type of CP, Gross Motor Function (GMFCS), oral hygiene quality (OHI-S), and dental caries experience (DMFT/dmft) were evaluated.</p><p><strong>Results: </strong>The CP condition of the children was significantly associated with Mother's SOC (P < 0.001). Mothers of children with CP had lower SOC scores (mean: 27.6 [±3.0]) than mothers of children without CP (mean: 30.2 [±7.7]). In the CP group, level IV, V of Gross Motor function was associated with lower mother's SOC scores (p = 0.001). In both groups, dental caries experience was associated with lower mother's SOC scores, CP group (p < 0.001), without CP group (p = 0.002). Regarding the individual characteristics, CP group presented with lower-quality of oral hygiene (p < 0.001) and high prevalence of dental caries (p = 0.001). Regarding marital status, mothers of CP group were majority single, separated, or divorced (p < 0.001), and presented more difficulties in finding a dentist for their child (p < 0.001).</p><p><strong>Conclusion: </strong>Mother's SOC was statistically significantly associated with the presence of dental caries in children/adolescents with and without CP. Intervention with an oral health education program was introduced to the orphans. The data was collected at baseline using a questionnaire to collect data about the knowledge, practice, and examination of DMFT (decayed, missing, and filled permanent teeth), dft (decayed, filled deciduous teeth), and OHI-S (oral hygiene index-simplified) was done. The knowledge part of the questionnaire was distributed immediately after the intervention. After 6 months, the knowledge, practice, and OHI-S index were determined, the data were then analyzed using Mann-Whitney A U non-parametric test, Wilcoxon matched pair signed rank test, and Fridman's 2-way ANOVA test in SPSS version 25 software.</p><p><strong>Results: </strong>89% of the children had dental caries, with mean DMFT 2.23 ± 2.1 among males and 3.3 ± 2.3 among females, dft among males was 3.1 ± 2.5 and 3.3 ± 2.1 among females. The mean score of OHI-S significantly decreased after health education intervention from 4.79 ± 1.4 to 0.62 ± 1.0. Mean scores of total knowledge and practice significantly increased after 6 months from 3.4 ± 1.3 to 7.1 ± 1.1 and 1.3 ± 0.9 to 4.9 ± 0.3, respectively.</p><p><strong>Conclusions: </strong>The orphans had moderate DMF and dft, fair knowledge about their oral cavity, poor OHI-S, and practices at baseline. To date, under 200 cases of TTS are documented in medical literature, with only sparse studies reporting on oral complications secondary to this condition.</p><p><strong>Case report: </strong>The following report presents a well-documented case of oral complications associated with TTS in an 83-year-old Caucasian female presenting with right-sided lesions on her nasal ala, scalp, buccal mucosa, and tongue, secondary to self-inflicted injury following their second microvascular decompression for surgical management of trigeminal neuralgia.</p><p><strong>Discussion/conclusion: </strong>The paucity of literature on oral complications associated with TTS supports the need for understanding guiding management modalities of such lesions for clinical practice. Forty-three percent and 81% of the parents rated their child's oral health and general health as good, respectively. Parent-rated oral health was strongly associated with general health, followed by the parents' satisfaction with their child's dentist, halitosis, difficulty in dental examination, and the number of times they helped their child with toothbrushing. Seventy-seven percent of the parents were satisfied or rather satisfied with their child's dentist. The parents expected the dentist to reassure their child and to be specialized in the treatment of persons with disabilities. Seventy-three percent of the parents brushed their child's teeth at least once a day, and 92% brushed their own teeth at least twice a day. {"title":"Traumatic dental injuries and oral health-related quality of life in children and adolescents with special health care needs in Eastern India: A case-control study.","authors":"Sri Priya Narayanan, Shilpa Mahapatra, Hitesh Kumar Sahoo, Lipsa Padhihary, Shivani Gupta","doi":"10.1111/scd.13078","DOIUrl":"10.1111/scd.13078","url":null,"abstract":"<p><strong>Aim: </strong>This matched case-control study aimed to investigate the prevalence of traumatic dental injuries (TDI) and its impact on oral health-related quality of life (OHRQoL) in hearing and/or visually impaired children attending government special schools in Eastern India.</p><p><strong>Methods: </strong>A two-step cluster analysis was performed to define cases and controls based on the Parental-Caregivers Perceptions Questionnaire (PCP-Q) scores from the 228 parent/guardian responses. The cases included children with a higher negative impact on OHRQoL (89 subjects), whereas the controls were those with a lower negative impact on OHRQoL (139 subjects). Groups were matched for age and gender and researchers were blinded to case/control status. Binary logistic regression was performed.</p><p><strong>Results: </strong>The prevalence of TDI among the study subjects was 24.12%. Compared to subjects in the control group (16.5%), a significantly higher proportion of cases (36%) reported a history of TDI. Children with TDI had considerably higher OHRQoL scores across all P-CPQ sub-scales. Falls were the most common cause of TDI, primarily at home or school and none received treatment.</p><p><strong>Conclusion: </strong>TDI significantly impacts OHRQoL in these children. Lack of treatment highlights the need for caregiver awareness and improved dental access for children with special needs. This study examined reproducibility of tongue pressure measurement (TPM) in DS patients.</p><p><strong>Methods: </strong>Thirty patients with DS (13 females) who were able to complete three measurements underwent a tongue pressure test (TPT). Reproducibility was evaluated using intraindividual standard deviation (SD) and coefficient of variation (CV). Patients were divided into two groups based on developmental age (DA): DA ≥ 4 years (over-4 DA) and DA < 4 years (under-4 DA).</p><p><strong>Results: </strong>Thirty DS patients with a mean age of 33.0 ± 11.7 years and mean DA of 3.8 ± 1.2 years were analyzed. Median tongue pressure was 17.6 kPa, SD was 2.0 kPa, and CV was 11.9. Eleven patients were in the under-4 DA group, and 19 were in the over-4 DA group. RCTs available in English were included. Oral hygiene and gingival outcomes in CSCNs, using powered or manual toothbrushes were: plaque index (PI), Quigley Hein plaque index (QHPI), gingival index (GI), and oral hygiene index simplified (OHI-S). ROB and publication bias was assessed using Cochrane tool and funnel plots, respectively. Meta-analyses were performed using forest plots. GRADE was used for the quality.</p><p><strong>Results: </strong>Twelve studies were included in SR and 11 studies in MA. In relation to GI and in the subgroup analysis of QHPI, powered toothbrushing was superior to manual toothbrushing with SMD -0.681 [95% CI (-0.911 to -0.451)] and SMD -0.85 [95% CI (-1.166 to -0.53)], respectively using REM. Comparisons with other indices exhibited no significant differences. The ROB and publication bias were moderate, quality of evidence was low to moderate GRADE.</p><p><strong>Conclusion: </strong>Powered toothbrushes had favorable outcomes in a few but not all parameters. Despite difficulties, more evidence exploring difficulties among this cohort in India is needed. This study aims to conduct a scoping review of articles published in India exploring access to dental care among individuals with IDD.</p><p><strong>Method: </strong>We followed the Arksey O'Malley framework for scoping reviews and the PRISMA guidelines for reporting studies. Findings were analyzed based on Levesque's framework for access.</p><p><strong>Results: </strong>Seventeen articles were collated from five databases. The perceived need for dental care was low, and dental visits were mainly need-based and infrequent. Knowledge regarding dental care did not translate to dental visits. Patient-related barriers were cost, transportation difficulties, fear, lack of dentist skills, patient behavior, and cooperation. From the providers' perspective, most dentists were willing to treat but noted infrastructural and knowledge-based limitations.</p><p><strong>Conclusion: </strong>Access to dental care was limited due to infrastructure constraints and a lack of dentist training. Caregivers and individuals with IDD did not prioritize dental health, faced communication barriers, and were afraid of dental procedures. It is important to improve preventive dental care and raise awareness among caregivers of individuals with IDD in India.
