In Iran, despite the high consumption of harmful products, there are limitations in healthcare taxation, which may be due to the high wall of distrust between the government and taxpayers. In this study, we aimed to identify and analyze the reasons for paying health care taxes in developed countries in comparison with Iran.</p><p><strong>Methods: </strong>In this scoping review, the data were collected among the evidence published from 2000 to 2022 in English and Persian. A search strategy was conducted to find all the evidence and resources published using the selected keywords across several databases. Finally, 129 articles were confirmed to be included in the review.</p><p><strong>Results: </strong>Taxes on harmful goods, including sin taxes value, added taxes, and green taxes were among the most important and sustainable resources to improve the health care system in each country. {"title":"Assessing the operational effectiveness of village health volunteers in Thailand: A structural equation modeling analysis.","authors":"Jatuporn Ounprasertsuk, Prakasit Wannapaschaiyong, Araya Tipwong","doi":"10.34172/hpp.025.44378","DOIUrl":"10.34172/hpp.025.44378","url":null,"abstract":"<p><strong>Background: </strong>Village health volunteers (VHVs) play an essential role in Thailand's primary healthcare system by promoting health, preventing diseases, and ensuring community well-being. However, their operational effectiveness is influenced by several key factors, including transformational leadership, employee commitment, and job satisfaction. This study investigates these relationships and their implications for sustainable community health systems.</p><p><strong>Methods: </strong>A mixed-methods approach was utilized, combining quantitative data from 280 VHVs across Thailand's lower central region with qualitative insights gained from focus group discussions (FGDs). Structural Equation Modeling was employed to evaluate the relationships among transformational leadership, employee commitment, job satisfaction, and operational effectiveness.</p><p><strong>Results: </strong>The findings reveal that transformational leadership significantly predicted employee commitment (β=0.80, <i>P</i><0.001) and job satisfaction (β=0.65, <i>P</i><0.001). Operational effectiveness was significantly predicted by employee commitment (β=0.49, <i>P</i><0.001), transformational leadership (β=0.26, <i>P</i><0.001), and job satisfaction (β=0.13, <i>P</i>=0.04). Together, these variables explained 68.0% of the variance in operational effectiveness. A new model termed T-E-J Performance (where T=Transformational Leadership, E=Employee Commitment, and J=Job Satisfaction) has been developed to guide strategic improvements.</p><p><strong>Conclusion: </strong>To sustain effective community health programs, it is vital to enhance leadership capabilities, improve job satisfaction, and foster commitment among VHVs. These findings offer actionable insights for policymakers to create leadership training programs, enhance incentives, and implement strategies for workforce retention. {"title":"Visitor responses to emergency evacuation: A human behavior approach.","authors":"Omid Akbarzadeh, Parisa Moshashaei, Hamed Golzad, Henry Liu, Seyed Shamseddin Alizadeh","doi":"10.34172/hpp.025.44366","DOIUrl":"10.34172/hpp.025.44366","url":null,"abstract":"<p><strong>Background: </strong>This study investigates human behaviours during emergency evacuations using data collected at the Tabriz Grand Bazaar (TGB), a UNESCO World Heritage site and the world's largest covered market. While past studies modelled crowd evacuations through simulations, limited empirical research exists on real human behaviour.</p><p><strong>Methods: </strong>Drawing on 749 questionnaire responses from TGB visitors, the study explores four key aspects of evacuation dynamics: (1) proactive vs. reactive responses; (2) competitive vs. cooperative interactions; (3) symmetry-breaking behaviours; and (4) route selection.</p><p><strong>Results: </strong>The analysis revealed that 44.2% of participants reported engaging in competitive behaviours (e.g., pushing), while only 8.4% showed a strong willingness to help others. Over 90% tended to follow others during evacuations, and 77.8% preferred to avoid the least crowded exit, indicating high symmetry-breaking tendencies. {"title":"Predicting consumption of the USDA protein sub-recommendations among adults: a reasoned action approach.","authors":"Paul Branscum, Karly Geller","doi":"10.34172/hpp.025.44443","DOIUrl":"10.34172/hpp.025.44443","url":null,"abstract":"<p><strong>Background: </strong>There is reportedly an under-consumption of protein foods from sub-recommendations within the Dietary Guidelines for Americans (meat, poultry, and eggs, seafood, and nuts, seeds and soy-products). The purpose of this study was to operationalise the Reasoned Action Approach (RAA) to better understand the theory-based factors associated with the consumption of all three protein sub-recommendations.</p><p><strong>Methods: </strong>A national sample of adults in the United States of America (n=372) were recruited. Protein was evaluated using a modified version of a previously validated survey. RAA variables (intentions, instrumental and experiential attitudes, injunctive and descriptive norms, autonomy, and capacity) were measured for each protein sub-recommendation. Linear regression analyses were used to evaluate determinants of intentions and consumption of protein from each sub-recommendation.</p><p><strong>Results: </strong>Adults predispositions were significantly higher (<i>P'</i>s<0.05) for consuming meats, poultry and eggs [intentions (1.43), instrumental (1.51) and experiential (1.49) attitudes, injunctive (1.23) and descriptive (1.14) norms, autonomy (2.16), and capacity (2.07)], compared to the other groups [seafood (intentions (-0.24), instrumental (0.37) and experiential (0.63) attitudes, injunctive (0.17) and descriptive (-0.46) norms, autonomy (1.73), and capacity (0.94)); nuts, seeds, and soy products (intentions (0.57), instrumental (0.76) and experiential (0.85) attitudes, injunctive (0.47) and descriptive (-0.01) norms, autonomy (1.91), and capacity (1.62))]. A significant amount of the variance was explained for each protein sub-recommendation using the RAA variables[meats, poultry and eggs (intentions adjusted R²=0.596/ behaviour adjusted R²=0.166); seafood (intentions adjusted R²=0.691/ behaviour adjusted R²=0.363); nuts, seeds and soy-products (intentions adjusted R²=0.582/ behaviour adjusted R²=0.390)]; however not all variables were significant.</p><p><strong>Conclusions: </strong>The RAA appears to be a robust model for explaining why adults under consume protein foods from the USDA sub-recommendations. A health system strengthening (HSS) approach through an innovative 3T-model can solve this ubiquitous problem. 3T includes- (i) Train rural youths (majorly women) as nationally certified & digitally literate Community Health Workers (CHWs) creating livelihood and women empowerment (ii) Technology: clinical algorithm driven software; easy-to-use affordable innovative diagnostic devices, screening tools (iii) Task-shifting: the software guides CHWs transmit structured, analysable, patient data to 'remote' doctors. CHWs addresses personnel shortage in health sector. Technology and task-shifting enable improved access to general and specialist doctors. Digital presence reduces doctors' travel and time, enabling more consultations. The 3T-model addresses critical health inequities in India. {"title":"Psychometric properties and validation of short health literacy screening (SHLS) to identify low health literacy.","authors":"Sulmaz Ghahramani, Marzieh Rahimi, Majid Khadem-Rezaiyan, Mina Zarei, Mohammad Sayari, Ali Ardekani, Kamran Bagheri Lankarani","doi":"10.34172/hpp.025.44250","DOIUrl":"10.34172/hpp.025.44250","url":null,"abstract":"<p><strong>Background: </strong>The purpose of this study was to evaluate the psychometric properties and performance of the short health literacy screening (SHLS) to identify people with low health literacy among a set of Iranian adults in Mashhad and Shiraz.</p><p><strong>Methods: </strong>In this cross-sectional study, participants were 18-65 years old Iranian adults randomly selected from two major cities of Iran, encompassing inpatients, outpatients, and healthy individuals. They completed the demographic, SHLS, and Health Literacy for Iranian Adults (HELIA) questionnaires. Psychometric properties of SHLS were evaluated. We investigated the performance of screening questions for detecting inadequate and limited health literacy based on HELIA as a comparison standard.</p><p><strong>Results: </strong>SHLS had acceptable psychometric properties (Content validity index of 0.92, 0.78, 0.85, and 0.85, Spearman-Brown correlation coefficient of 0.827, 0.928, 0.581, and 0.712 for four questions, and Cronbach's alpha of 0.713). Of 547 participants, 339 (62%) were female, and 415 (75.9%) were between 18 and 45 years old. The prevalence of inadequate and limited health literacy based on HELIA was 56 (10.2%) and 192 (35.1%), respectively. The question \"How well do you understand the medical prescriptions your doctor told you?\" (Comprehension of Prescriptions) showed a higher area under the receiver operating characteristic (AUROC) of 0.690 (0.620-0.759) for inadequate, and 0.666 (0.619-0.713) for limited health literacy. The combination of \"How confident are you filling out forms by yourself?\" and \"Comprehension of Prescriptions\" showed a better AUROC of 0.705 (0.638-0.773) for inadequate health literacy. All questions together had a higher AUROC of 0.683 (0.636-0.729) for limited health literacy. This study sought to examine married individuals' perceptions of the factors contributing to diminished sexual intimacy, as well as the barriers impeding its sustenance. By elucidating these dimensions, the research aims to provide a nuanced understanding of the psychosocial and interpersonal mechanisms underlying this phenomenon.</p><p><strong>Methods: </strong>Employing a qualitative design, the study utilized a conventional content analysis approach to investigate these phenomena. A purposive sample of 26 married men and women participated in the study, with data collected through individual semi-structured interviews. Concurrent analysis was performed during data collection, with MAXQDA 2020 software used for textual data management and organization.</p><p><strong>Results: </strong>Thematic analysis yielded five principal themes (with fourteen sub-themes) characterizing factors contributing to diminished sexual intimacy. These comprised: (1) sex drive mismatch, (2) lack of perceived emotional intimacy, (3) sexual dissatisfaction, (4) restrictive sexual stereotypes, and (5) sexual nostalgia. The findings indicate that diminished sexual intimacy arises from interacting intrapersonal, interpersonal, and sociocultural factors, which cumulatively affect sexual relationship quality and, by extension, marital intimacy.</p><p><strong>Conclusion: </strong>In light of these findings, it is recommended that sexual health delivery systems prioritize implement targeted couple consultations addressing multidimensional barriers to intimacy (psychological, relational, and societal). This demographic often experiences a convergence of health-related issues, including poor dietary practices, substance use, mental health disorders, and obesity. These issues frequently interact in complex ways, influencing long-term health trajectories. This scoping review aimed to examine the breadth of strategies and research concerning health-promoting initiatives across universities worldwide, while identifying existing gaps in the literature.</p><p><strong>Methods: </strong>Following the PRISMA-ScR guidelines, a systematic search was conducted for peer-reviewed articles published between 2014 and 2024 using the SCOPUS and Web of Science databases. After screening and eligibility assessment, 272 articles were included in the final review.</p><p><strong>Results: </strong>Thematically, 19% of studies focused on health-related policies and frameworks, 19% on mental health and wellness, 16% on technology-assisted interventions, 14% on physical activity, 7% on nutrition, 6% on behavioral habits, and others addressed curriculum restructuring.</p><p><strong>Conclusion: </strong>Findings reveal that most interventions remain in preliminary stages of implementation, with limited follow-up studies evaluating their effectiveness or cost-efficiency. There is a pressing need for robust, longitudinal research to assess outcomes, particularly in low- and middle-income countries.
