Bîrsan","doi":"10.22551/msj.2022.03.17","DOIUrl":"https://doi.org/10.22551/msj.2022.03.17","url":null,"abstract":"INFLUENCE OF CARBOPOL 971P NF ON FLOW RATE AND SWELLING PARAMETERS IN ANTIFUNGAL TABLETS (Abstract): The purpose of this research laid in develop-ing oromucosal bioadhesive tablets with miconazole nitrate based on hydrophilic bioadhesive polymers, Carbopol 971P NF, capable to form a good adhesion to prolong the duration of the active ingredient action for several hours. The aim is to observe the correlation between the disintegration time, the swelling index and the amount of polymer used in mucoadhesive tablets. Material and methods : The tablets were made using direct compression method, the mucoadhesive tablets differing in the amount of Carbopol 971P NF. The first proposed formula (FI) contains 40 mg polymer and the second one (FII) contains 10 mg polymer. After the formulation and development of mucoadhesive tablets using different grades of polymer, physico-chemical parameters such as surface area, thickness, diameter, friability, hardness, disintegration time and swelling index were determined. Results : Both proposed formulas have an optimal disintegration time of 75 and 98 minutes. The swelling index for FI (18% polymer) is 0.4435 and for FII (4.5% polymer) is 1.0316. Sîngeap","doi":"10.22551/msj.2022.03.06","DOIUrl":"https://doi.org/10.22551/msj.2022.03.06","url":null,"abstract":"CLINICAL PROFILE ASSOCIATED WITH BLEEDING BOWEL TUMORS IN PATIENTS UNDERGOING CAPSULE ENDOSCOPY (Abstract): Since the development and implementation into clinical practice of the small bowel (SB) capsule endoscopy (CE), obscure gastrointestinal bleeding (OGIB) is no longer an answerless question. In many cases of SB pathology, a definite diagnosis is made by SBCE, while other cases, as SB tumors (SBT), require additional investigations. Our aim was to identify predictors for SBT as OGIB etiology. Material and methods: We retrospectively studied all patients with OGIB who underwent SBCE in a two-year period. Only patients with positive findings were included. Patients with SBTs were compared to patients with non-tumoral cause of OGIB, and analyses for possible associated clinical factors were performed. Results: 205 SBCE exami-nations were performed for OGIB. Positive findings were noted in 134 patients (male gender: 72, female gender: 62; age range 21-79): 25 cases of SBTs, and 109 cases with non-tumoral causes responsible for bleeding (angioectasia, Crohn’s disease, NSAIDs enteritis, and other). SBT patients underwent further investigations, for confirmation and stadializa-tion. Patient’s age < 65 years, lack of anticoagulant treatment, and overt type of bleeding significantly associated with SBTs. Gender was not predictor for SBT. Surgery was the main therapeutic option for SBTs, significantly more frequently compared to the non-tumoral causes. Conclusion: Young age, overt bleeding and lack of anticoagulant treatment are factors associated with the final diagnosis of SBT. Material and methods: Prospective study, selected 105 patients with complicated gallstones and associated cholestatic jaundice, divided into three homogeneous subgroups, dependent on evolutionary com-plication. Control group - 35 patients with uncomplicated gallstones. All patients in the study group followed the staged treatment protocol in obstructive jaundice, with the primary prerogative - internal biliary decompression by endoscopic papillosphincterotomy. Pro- and antioxidant markers were assessed at hospitalization, on the 1st and 4th day after decompression. Results: Malonic dialdehyde at hospitalization marked in all subgroups with statistically significant results compared to the control group: I - 12.11±0.64 mol/l [p<0.001 (t=10.7)], II - 11.12±0.53mol/l [p<0.001 (t=10.9)]; III - 13.16±0.44 mol/l [p<0.001 (t=16.5)]. The activity of the antioxidant system evaluated by superoxide dismutase values [p<0.001(t=6.4); (t=4.1); (t=5.7)] and catalase [p<0.001(t=6.7); (t=5.6); (t=7.6)] shows a decrease in all subgroups compared to the control group. On the 1st day after drainage, malonic dialdehyde values did not show statistical significance compared to the before decompression state [I -p>0.05 (t=0.11); II - p>0.05 (t=0.91)]. An obvious reduction of its activity was found on the 4th after decompression day [I - p<0.001 (t=5.15; II - p<0.01 (t=2.6); III - p<0.001 (t =3.99)]. The return of the antioxidant system in all subgroups to values relatively similar to the control group: superoxide dismutase [I - p<0.05 (t=2.21); II - p<0.05 (t=0.0008); III -p<0.05 (t=0.33)] and catalase [I - p<0.05 (t=2.39); II - p<0.05 (t=3.52); III - p<0.01 (t=2.87)] was found more than 4 days after decompression. Conclusions: The study demonstrated the positive influence of biliary decompression in the complications of gallstones associated with cholestatic jaundice for the reduction of a massive lipid oxidation and the creation of conditions for increasing the antioxidant capacity with the restoration of the body's biological balance, which is more pronounced 4-5 days after decompression, serving as criterion for the possibility of definitive resolution of gallstones in these terms.
