Kempeneers","doi":"10.1016/j.sexol.2021.09.002","DOIUrl":"10.1016/j.sexol.2021.09.002","url":null,"abstract":"<div><h3>Objectif</h3><p>La présente revue vise à tirer les enseignements généraux de plusieurs décennies de recherches empiriques consacrées aux traitements psycho-comportementaux de l’éjaculation prématurée.</p></div><div><h3>Méthode</h3><p>Trente-trois essais faisant état de mesures avant et après traitements psycho-sexologiques ont été rassemblés et analysés.</p></div><div><h3>Résultats</h3><p>Les taux d’amélioration associés aux sexothérapies s’échelonnent de 25 % à 100 %, leur taille d’effet globale (d) se situe entre 0,83 et 3,75. L’efficacité des traitements requière la réalisation d’exercices de stimulation pénienne avec pauses (« stop-start ») et/ou d’exercices visant à la régulation des composantes discrètes de l’excitation (approche « régulatrice » ou « sexo-fonctionnelle »). Il n’est pas sûr que les variantes faisant appel à des pincements (« squeeze »), à l’utilisation d’un vibreur ou à un mode particulier de masturbation produisent un surcroît d’efficacité. Quant aux exercices centrés sur le renforcement de la musculature pelvienne, ils ne semblent guère pertinents. Les traitements les plus efficaces sont également ceux qui permettent un accompagnement visant à optimiser l’intégration par les bénéficiaires des principes thérapeutiques et à aborder avec eux d’éventuelles difficultés associées. En cela les formules d’auto-traitement apparaissent moins performantes. La combinaison d’une sexothérapie et d’une pharmacothérapie donne généralement lieu à des résultats supérieurs à ceux obtenus individuellement de chacune de ces deux options.</p></div><div><h3>Conclusions</h3><p>Les recherches empiriques cumulées sur plusieurs décennies accréditent l’utilité des sexothérapies dans le domaine de l’éjaculation prématurée et spécifient leurs conditions d’efficacité.</p></div><div><h3>Objective</h3><p>The aim of this review is to draw general lessons from several decades of empirical research on psycho-behavioural treatments for premature ejaculation.</p></div><div><h3>Method</h3><p>Thirty-three trials reporting measures pre- and post-sex therapy were collected and assessed.</p></div><div><h3>Results</h3><p>The improvement rates associated with sex therapies ranged from 25 % to 100 %, the overall effect size (d) was between 0.83 and 3.75. The effectiveness of the interventions requires penile stimulation exercises with pauses (“stop-start”) and/or exercises aimed at regulating discrete components of sexual arousal (“regulatory” or “sexual-functional” approach). It is not clear whether variations involving squeezing, use of a vibratory device or a particular mode of masturbation are more effective. Exercises that focus on strengthening the pelvic muscles do not appear to be particularly effective. It aims to determine (1) if sexters are more likely to experience in-person sexual coercion perpetration or victimization than non-sexters and (2) if sexting coercion perpetration or victimization can be predicted by prior experiences of in-person or technology-based sexual coercion perpetration and victimization.</p></div><div><h3>Methods</h3><p>Participants (<em>N</em> <!-->=<!--> <!-->1076; ages 16 to 83) completed an online questionnaire about sexual experiences including modified French-translated versions of the <em>Sexual Experiences Survey</em> – <em>Perpetration & Victimization</em> – <em>Tactics First</em>.</p></div><div><h3>Results</h3><p>Findings reveal that (1) sexters experience more in-person sexual coercion perpetration and victimization than non-sexters, (2) within a sexting coercion perpetration regression model, being a man and past experiences of in-person perpetration predicted sexting coercion perpetration, and (3) within a sexting coercion victimization regression model, being young, being a woman, and past experiences of in-person victimization predicted sexting coercion victimization.</p></div><div><h3>Conclusions</h3><p>These findings suggest that coercive tactics extend from in-person to technology-based sexual interactions and emphasize the need to investigate similarities and distinctions between in-person and technology-based sexual coercion as well as factors that make sexters more likely to be involved in sexual coercion.</p></div><div><h3>Objectifs</h3><p>Cette étude examine la relation entre le sextage, la coercition sexuelle subie et perpétrée en ligne et la coercition sexuelle subie et perpétrée en personne. The change in the size of the prostate gradually leads to lower urinary tract symptoms (LUTS). Even though LUTS and BPH rarely become life-threatening conditions, the impact can be significant on a patient's quality of life (QOL). The initial evaluation and secondary should be performed to observe other conditions and another differential diagnosis. The modalities therapy for BPH patients consists of watchful waiting, medical therapy, and surgical treatment. During the last years, the medical therapy of BPH has been improved and varied. Medical therapy of BPH is focused on relieving the bothersome symptoms and prevent the absolute indication for surgical treatment. The pharmacology effect, efficacy, safety, and cost-effectiveness have to be analyzed properly to generate good drug adherence and persistence.</p></div><div><h3>Objectif</h3><p>Cette étude vise à définir les TUBA sur un patient et à prescrire la meilleure option médicamenteuse pour chaque pathologie des patients selon la caractéristique des médicaments afin d’améliorer la qualité de vie du patient.</p><p>L’hyperplasie bénigne de la prostate (HBP) est devenue la maladie la plus fréquente étroitement liée au vieillissement chez l’homme. La modification de la taille de la prostate entraîne progressivement des troubles urinaires du bas appareil (TUBA). Même si les TUBA et l’HBP deviennent rarement des maladies potentiellement mortelles, l’impact peut être important sur la qualité de vie (QV) d’un patient. L’évaluation initiale et secondaire doit être effectuée pour observer d’autres conditions, suivi par un diagnostic différentiel. Les modalités de traitements des patients atteints d’HBP consistent en une attente sous surveillance, une thérapie médicale et un traitement chirurgical. Au cours des dernières années, le traitement médical de l’HBP a été amélioré et varié. Le traitement médical de l’HBP vise à soulager les symptômes gênants et à prévenir l’indication absolue d’un traitement chirurgical. There are appropriate indicators that PDE5 inhibitors may also modify the detrimental consequences of the immune system over-stimulation, supplying a new chance for their use in SARS-CoV2 patients. The use of sildenafil for the management of SARS-CoV2 has been suggested based on its several mechanisms of action and therapeutic effects and on the clinical features of SARS-CoV2 which similar to those of other pathologies treated with the PDE5 inhibitors. Sous cette appellation, se trouvent réunies de multiples infractions comprenant la pédopornographie, présentant elle-même de multiples facettes, ainsi que d’autres types de faits. Si les travaux de recherches anglophones sont plus anciens et quantitativement plus importants, les publications francophones sur le sujet s’avèrent relativement peu nombreuses, tendant parfois à le traiter de manière générale. Il en ressort aussi la mise en avant d’actes nouveaux, présentant une particularité.</p></div><div><h3>Méthode</h3><p>Il s’agit ici de revenir sur la littérature scientifique francophone et sur les textes de droit afin de relever les effets de ces infractions, tant sur le droit pénal et les jugements que sur les approches psychopathologiques. Autrement dit, il est question d’interroger la spécificité décrite par les auteurs et de voir ce qu’elle engage dans les prises en charge thérapeutiques.</p></div><div><h3>Résultats</h3><p>L’accent mis sur la pédopornographie et les téléchargeurs pourrait laisser penser une forme d’homogénéité des pratiques. De même, il pourrait être tentant de croire que l’intérêt sexuel pour les enfants, soit la pédophilie, constitue leur motivation première. L’article montre cependant la nécessité d’établir des programmes de recherches structurés permettant de définir plus clairement l’ensemble des pratiques sexuelles illégales permises par Internet et d’appréhender par des données objectives les profils des auteurs, en réunissant notamment des informations sociodémographiques, mais aussi psychopathologiques. L’absence de telles informations conduisant finalement à entretenir une certaine nébuleuse.</p></div><div><h3>Objective</h3><p>This article aim to question the thematic of sexual cyberviolences perpetrated on minors. Under this designation, are reunited multiple infractions such as pedopornography, presenting with many facets, as well as other types of facts. Though the english-speaking researches are older and numerous, there are only few French publications on the subject, and tend to address the topic in a general way. It puts an emphasis on newer acts which present a specific feature.</p></div><div><h3>Method</h3><p>We must come back to French scientific literature and to the law in order to reveal these infractions’ effects on criminal law and judgements as well as on psychopathologic approaches. À Haïti, en dépit de l’instauration de nombreux programmes de planification familiale par les pouvoirs publics, la fécondité précoce demeure relativement élevée.</p></div><div><h3>Objectifs</h3><p>Ainsi, au regard de la persistance de ce phénomène et de ses conséquences tant sur la jeune fille que sur son enfant, cet article a pour objectifs d’identifier et de hiérarchiser les facteurs sous-jacents de la fécondité précoce à Haïti.</p></div><div><h3>Méthodes</h3><p>Afin de parvenir aux objectifs fixés dans le cadre de cet article, nous avons mobilisé les données de la dernière Enquête Démographique et de Santé réalisée à Haïti en 2017. L’analyse des données a consisté à la réalisation de tris à plats, de tris croisés et à l’estimation d’un modèle de régression logistique.</p></div><div><h3>Résultats</h3><p>Nous observons que la religion, le niveau d’instruction, l’âge à la première relation sexuelle, l’âge à la première union et le nombre idéal d’enfants sont les facteurs qui influencent la maternité au moment de l’adolescence. Et parmi ces facteurs, on constate que l’âge à la première union et l’âge à la première relation sexuelle sont ceux qui contribuent le plus à l’explication de ce phénomène.</p></div><div><h3>Conclusion</h3><p>Étant donné que la maternité à l’adolescence à Haïti est un phénomène multifactoriel, la prévention exige l’application de stratégies multidimensionnelles par les pouvoirs publics.</p></div><div><h3>Introduction</h3><p>The World Health Organization classifies adolescent motherhood as a significant public health problem. In Haiti, despite the government's effort to establish family planning programs, fertility among adolescents remains relatively high.</p></div><div><h3>Purpose of research</h3><p>Considering the persistence of this phenomenon as well as associated consequences for young girls and their children, this paper aims to identify the underlying factors of early fertility in Haiti.</p></div><div><h3>Methods</h3><p>We analyzed data from the 2017 Haiti Demographic and Health Survey. Our analysis focused on women of reproductive age (15–49 years) who had their first birth before the age of 20 years. We conducted a descriptive analysis and estimated a logistic regression model to analyze the association of socioeconomic factors with adolescent fertility.</p></div><div><h3>Results</h3><p>We observed that religion, education level, age at first sexual intercourse, age at first union and ideal number of children significantly influenced adolescent motherhood in Haiti. Cependant, les mères qui choisissent des alternatives à l’allaitement maternel exclusif peuvent vivre des sentiments de culpabilité et d’échec. Cet enjeu soulève l’importance de comprendre le vécu de ces mères afin d’harmoniser des soins et des accompagnements qui visent l’autonomisation.</p></div><div><h3>Méthode</h3><p>Cette recherche qualitative présente l’analyse situationnelle des parcours de neuf mères qui ont opté pour des alternatives à l’allaitement maternel exclusif.</p></div><div><h3>Résultats</h3><p>Les résultats obtenus avec le support d’entrevues semi-dirigées décrivent les processus décisionnels du choix alternatif à l’allaitement, situent ce choix à travers un ensemble de soins favorable à l’allaitement et positionne ce choix au regard la norme sociale de l’allaitement. La discussion présente les différentes normes reliées à l’allaitement et les processus de négociation vécus par les mères à cet égard. Ces négociations s’exécutent par le rapport au corps, par l’intrication des normes sociales et professionnelles associées aux pratiques d’allaitement et par les injonctions relatives à la « bonne » maternité.</p></div><div><h3>Conclusion</h3><p>Les recommandations soulèvent l’importance d’une approche centrée sur l’autonomie des mères.</p></div><div><p>Context The benefits of breastfeeding on the health of infants have been established by numerous scientific studies. However, mothers who choose alternatives to exclusive breastfeeding may experience feelings of guilt and failure. This issue raises the importance of understanding the experiences of these mothers in order to harmonize care and support aimed at empowerment.</p></div><div><h3>Methods</h3><p>This qualitative research presents the situational analysis of the pathways of nine mothers who have opted for alternatives to exclusive breastfeeding.</p></div><div><h3>Results</h3><p>The results obtained with the support of semi-structured interviews describe the decision-making processes of the alternative choice to breastfeeding, situate this choice through an apparatus of care favorable to breastfeeding and position this choice with regard to the social norm of breastfeeding. The discussion presents the different norms related to breastfeeding and the negotiation processes experienced by mothers in this regard. Thus, the present study was conducted to assess the effects of sensate focus technique on the sexual function of primiparous women after their vaginal delivery.</p></div><div><h3>Methods</h3><p>This randomized clinical trial was performed on 82 primiparous women aged between 18–35 years old who had referred to health care center with their husband 2 months after their vaginal delivery. The women's baseline sexual function was assessed by the female sexual function index. Women were randomly assigned to two groups: Including intervention group where they received specific instruction on sensate focus technique and were followed up weekly for 2 months and control group where they received the information about recovery from childbirth and lactation. Sexual function of the participants was evaluated after 4 and 8 weeks.</p></div><div><h3>Results</h3><p>Mean age of the women was equal to 24.74 (4.84) years old. The two study groups were homogeneous in terms of educational level, employment, and socioeconomic status. Eight weeks after the intervention, the sexual function score was significantly higher in the intervention group than the control group (29.90 vs. 24.75, <em>p</em> <!--><<!--> <!-->0.001).</p></div><div><h3>Conclusion</h3><p>Our finding revealed that sensate focus technique has the potential to improve sexual function of the primiparous women after their delivery.</p></div><div><h3>Trial registration</h3><p>Approval number from Iranian Registry of Clinical Trials: IRCT20181218042041N1 (Registration date: 2019-02-22).</p></div><div><h3>Contexte</h3><p>La fonction sexuelle connaît des changements pendant la période du postpartum et après un accouchement par voie basse. La présente étude a été menée pour évaluer les effets de la technique du « Sensate Focus » sur la fonction sexuelle des femmes primipares après leur accouchement par voie basse.</p></div><div><h3>Méthode</h3><p>Cet essai clinique randomisé a été mené sur 82 femmes primipares âgées de 18 à 35 ans qui s’étaient rendues au centre de soins de santé avec leur mari deux mois après leur accouchement. La fonction sexuelle de base des femmes a été évaluée par le Female Sexual Function Index (IFSF). Les femmes ont été réparties au hasard en deux groupes: Le groupe d’intervention où elles ont reçu des instructions spécifiques sur la technique du « Sensate Focus » et ont eu un suivi hebdomadaire pendant 2 mois et le groupe contrôle où elles ont reçu des informations sur la récupération après l’accouchement et la lactation.
