We describe results of a retrospective chart review of 472 patients treated in the program between August 2015 and April 2019. We examined several demographic and substance use variables for individuals who consented to telemedicine treatment, retention in treatment over time, and opioid use over time to understand further the impact of prescribing buprenorphine and naltrexone via telemedicine to patients in a rural OUD treatment setting. Our findings support the effectiveness of prescribing medications via telemedicine. The inclusion of more than three times as many patients as in our prior report revealed retention rates and toxicology results that are comparable to face-to-face treatment. {"title":"The association between somatic complaints and past-30 day opioid misuse among justice-involved children.","authors":"Micah E Johnson, Sarah Clerjuste","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Background: </strong>Individuals in the criminal justice system are especially vulnerable to the adverse effects of opioid misuse. Research on justice-involved children (JIC) is needed to uncover the variables that predict opioid misuse initiation to prevent misuse or reduce harm in this population. Somatic symptoms are symptoms experienced in the body, such as physical sensations, movements or experiences, which can cause severe distress and dysfunction. These include pain, nausea, dizziness, and fainting. In this study, we hypothesize that somatic complaints will be associated with a higher likelihood of opioid misuse among Florida JIC.</p><p><strong>Methods: </strong>The study examined statewide data on 79,960 JIC in the Florida Department of Juvenile Justice database. Logistic regression was employed to investigate an ordinal measure of somatic complaints at first screen and a binary outcome measure of past-30 day illicit or nonmedical opioid use at final screen while controlling for sociodemographic and mental health factors.</p><p><strong>Results: </strong>Nearly 28% of JIC had a history of one or more somatic complaints. Compared to those with no history of somatic complaints, JIC with a history of one or two somatic complaints were 1.23 times more likely to misuse opioids in the past 30 days and those with three or four somatic complaints were 1.5 times more likely to meet criteria for past-30 day opioid misuse.</p><p><strong>Conclusions: </strong>Individuals may consume illicit or non-medical prescription opioids to manage somatic symptoms - indicating that increased access to healthcare may reduce misuse. Risk of opioid overdose sharply increases as justice-involved individuals are released from correctional settings largely due to a reduced tolerance to opioids as a result of incarceration and diminished access to legal medicines that are provided in the justice system. It becomes clear that the virus is spreading mostly deadly due to limited access to diagnostics tests and equipment. Traditional radiography and CT remain the main methods of the initial examination of the chest organs. Now, most of the diagnostics has been focused on PCR, chest X-Ray/CT manifestations of COVID-19. However, there are problems with CT due to infection control issues, the inefficiencies introduced in CT room decontamination, and lack of CT availability in LMIC (Low Middle Income Countries). Passive microwave radiometry (MWR) is a cheap, non-radioactive and portable technology. It has already been used for diagnostics of cancer, and other diseases. We have tested if MWR could be used for early diagnostics of pulmonary COVID-19 complications. This was a randomized controlled trial (195 subjects) to evaluate the effectiveness of diagnostics using MWR in patients with pneumonia caused by COVID-19 while they are in hospitals of Kyrgyzstan, and healthy individuals. We have measured skin (IR) and internal (MWR) temperature by recording passive electromagnetic radiation through the chest wall in the projection of the lungs at 30 symmetrical points on both sides. Pneumonia and lung damage were diagnosed by X-RAY/CT scan and doctor final diagnosis (pn+/pn-). COVID-19 was determined by PCR test (covid+/covid-). The best results were obtained between pn-/covid- and pn+/covid+ groups with sensitivity 92% and specificity 75%. Overall, the study suggests that the use of MWR is a convenient and safe method for screening diagnostics in COVID-19 patients with suspected pneumonia. Our aim was to identify what proportion of admissions could be avoided by upscaling ambulatory diagnostic pleural services, including use of a responsive pleural nurse specialist and hot-clinic activity. Methods: We retrospectively reviewed all pleural aspirations between 1.8.17 – 28.2.18 at our unit. Reason for admission and length of stay (LoS) were recorded. Results: 80% (149/187) of cases were emergency admissions, 6% (11/187) elective admissions, 14% (27/187) outpatient day cases. Mean age was 69 (SD 14). 57% (n=107) had malignant pleural effusion (MPE), 28% (n=52) pleural infection and 15% (n=28) other benign effusion. In emergency admissions, median LoS was 17 (IQR 10 – 28) days and median time to pleural aspiration was 3 (IQR 1 – 8) days. Reasons for admission are summarised in Table 1. Avoidable admissions (investigation of effusion only (32/149 (21%)) accounted for 425 bed days (median LoS of 11 (IQR 6 - 20) days). The majority of these cases (28/32 (87.5%)) had MPE. Conclusion: Emergency admission with pleural effusion is common and often prolonged. In this hospital-wide cohort, 21% of admissions could potentially have been avoided if the current ambulatory pleural service was upscaled. Most avoidable admissions were in patients with MPE, who often have a poor prognosis. As prior reports on APAP were from patient cohorts limited to single centers and/or single countries, baseline results from IMPALA offer unique potential insights: 138 eligible APAP patients from 30 sites in 18 countries were enrolled in IMPALA. Compared to other cohorts, IMPALA patients have relatively severe disease as demonstrated by an alveolar-arterial difference in oxygen tension (A-aDO2) of 40.6 ±18.0 mmHg and disease severity score (DSS) of 2.9 ±1.1 (data are mean ±SD). Whereas most series report a 2:1 male predominance, 43 % of subjects in IMPALA are female. The diffusion capacity for carbon monoxide (DLCO) was reduced at 48.6 ±16.3 % of predicted. The arterial O2 was reduced at 66.0 ±11.6 mmHg and reflected by a blood hemoglobin concentration of 15.3 ±1.9 g/L . Serum lactate dehydrogenase (LDH) was elevated in 72% of patients and correlated well with physiological measures. A-aDO2 and DLCO% were the strongest predictors of LDH. The 6-minute walk distance was 436 ±128 m and correlated with other physiological measures. Quality of life was reduced as measured by a Saint George’s Respiratory Questionnaire total score of 40.9 ±19.0 but, interestingly, did not significantly correlate with objective physiological measures.
