Using narratives of shopkeepers in small, rural enterprises, the purpose of this paper is to investigate the ways of thinking about health maintenance.\u0000\u0000\u0000Design/methodology/approach\u0000This study employed an ethnographic approach. In total, 20 shopkeepers and residents in a rural shopping street in Tohoku, in northeast Japan, were interviewed between June 2016 and May 2017. These interviews were analysed inductively.\u0000\u0000\u0000Findings\u0000The ways shopkeepers think about health maintenance are expressed through an interaction with their personal history. Shopkeepers and customers used narratives to arrive at novel methods to re-think health maintenance. Through data analysis, the cultural theme of maintaining vitality through long-lasting self-struggle supported by cooperation and familiar membership was identified. This cultural theme was related to three sub-themes: struggle stimulated by flexibility and responsibility; cooperation by devoting personal capital; and compensation through family support.\u0000\u0000\u0000Practical implications\u0000There is a growing focus on the important role that SSEs play in rural health. The results of this study will enable rural nurses to approach and develop health via communal interaction. In addition, the support of the family through cooperation at home should be enhanced.\u0000\u0000\u0000Originality/value\u0000There exists the potential for shopkeepers to develop a care strategy for small enterprise-related work. {"title":"Understanding the psychological drivers of loneliness: the first step towards developing more effective psychosocial interventions","authors":"G. It proposes a more holistic psychosocial approach to loneliness.\u0000\u0000\u0000Design/methodology/approach\u0000This paper draws on and reviews the published literature on the psychological aspects of loneliness.\u0000\u0000\u0000Findings\u0000Although not extensive, there is sufficient evidence to begin to develop and test more psychosocial approaches to addressing loneliness.\u0000\u0000\u0000Research limitations/implications\u0000The evidence base is not extensive and a fair degree of it has not been fully trialled with older people.\u0000\u0000\u0000Practical implications\u0000There is an a priori case for using the evidence that exists to develop and test out new psychosocial interventions for addressing loneliness.\u0000\u0000\u0000Social implications\u0000It is possible that a more psychosocial approach to loneliness will enable more to be done to address the significant distress of older people experience severe and chronic loneliness. It may also be possible to develop more effective preventative strategies which build resilience in older people.\u0000\u0000\u0000Originality/value\u0000The field of loneliness practice and research is overwhelmingly based on sociological analysis and social interventions. {"title":"All the lonely people, where do they all belong? An interpretive synthesis of loneliness and social support in older lesbian, gay and bisexual communities","authors":"J. Fish, C. Yet few studies have considered the nature of loneliness and social support for older lesbian, gay and bisexual (LGB) people; this is of particular concern as they are among the social groups said to be at greater risk. The paper aims to discuss this issue.\u0000\u0000\u0000Design/methodology/approach\u0000Peer-reviewed literature was identified through a search of Scopus, PsycINFO and PubMed. A total of 2,277 papers were retrieved including qualitative and quantitative studies which were quality assessed using the Critical Appraisal Skills Programme.\u0000\u0000\u0000Findings\u0000In total, 11 papers were included in the review and findings were synthesised using thematic analysis. The studies were conducted in five countries worldwide with a combined sample size of 53,332 participants, of whom 4,288 were drawn from among LGB communities. The characteristics and circumstances associated with loneliness including living arrangements, housing tenure, minority stress and geographical proximity.\u0000\u0000\u0000Research limitations/implications\u0000The review suggests that among older LGB people, living alone, not being partnered and being childfree may increase the risk of loneliness. This cohort of older people may experience greater difficulties in building relationships of trust and openness. They may also have relied on sources of identity-based social support that are in steep decline. Future research should include implementation studies to evaluate effective strategies in reducing loneliness among older LGB people.\u0000\u0000\u0000Practical implications\u0000Reaching older LGB people who are vulnerable due to physical mobility or rural isolation and loneliness because of bereavement or being a carer is a concern. A range of interventions including individual (befriending), group-based (for social contact) in addition to potential benefits from the Internet of Things should be evaluated. A general inductive approach of thematic content analysis was employed.\u0000\u0000\u0000Findings\u0000The four main emergent themes related to the passage of time or the perception of becoming old within the process of abuse were: abuse grown old, abuse after entering later life, vulnerability to abuse and responses to abuse. Ageing was found to be associated with an increase vulnerability to abuse and an important element in shaping how older adults experience, report and cope with victimization. {"title":"Loneliness in later life: a cross-sectional survey analysis of place-based factors in Ireland","authors":"S. Gibney, Tara Moore, S. Shannon","doi":"10.1108/QAOA-04-2018-0015","DOIUrl":"https://doi.org/10.1108/QAOA-04-2018-0015","url":null,"abstract":"\u0000Purpose\u0000The purpose of this paper is to investigate the relationship between the age-friendliness of local environments and self-reported loneliness for a representative sample of community-dwelling adults aged 55+ in Ireland.\u0000\u0000\u0000Design/methodology/approach\u0000Data were from the Healthy and Positive Ageing Initiative Age friendly Cities and Counties Survey (n=10,540) (2016). Several age friendly indicators, as proposed by World Health Organisation, were included in this study: outdoor spaces and buildings; access to social services; social participation; respect and social inclusion; and transport. Loneliness was measured using five items from the UCLA Loneliness Scale. Informed by an ecological approach to ageing, multi-level negative binomial regression models were used to investigate the association between each age friendly indicator and social loneliness. Models were adjusted for known demographic, socio-economic and health correlates of loneliness.\u0000\u0000\u0000Findings\u0000Average loneliness scores were significantly higher for those in poorer health, who lived alone, were materially deprived and those never or formerly married. Lower ratings and poorer outcomes for several interrelated age friendly place-based factors were significantly associated with higher loneliness scores: difficulty with transport, difficulty accessing social services, barriers to community activities, lower social engagement, and experiences and perceptions of ageism in the community; however, the effect sizes were small.\u0000\u0000\u0000Originality/value\u0000This study identified several modifiable age friendly features of local environments that are associated with loneliness in this older population. There is a lack of evidence regarding the therapeutic value of this approach for older people. The purpose of this paper is to explore how older people and their carers experience a cognitive-behavioural approach to team psychological formulation, within a mental health inpatient service in the UK.\u0000\u0000\u0000Design/methodology/approach\u0000A qualitative study using interpretative phenomenological analysis was undertaken. In total, 13 participants were interviewed: five older people/carer dyads, two lone older people and one lone carer.\u0000\u0000\u0000Findings\u0000Three overarching themes emerged: “Emotional impact of formulation” captured the mixed emotions that the process evoked in participants. “Making sense?” reflected the therapeutic value that participants experienced and what held them back from making gains in their recovery. “Disempowered people trapped in a biomedical world” illustrated the negative aspects of ward care, which hindered recovery.\u0000\u0000\u0000Research limitations/implications\u0000The findings provide insights into the therapeutic value of team psychological formulation and the difficulties in facilitating the process effectively. More consistency is required to ensure that team formulation is standardised. The purpose of this paper is to ascertain the acceptability of a program of equine-assisted activities (EAAP) for people with dementia by elucidating programmatic practices needed to enhance their safety and quality of life (QoL) from the perspectives of service providers.\u0000\u0000\u0000Design/methodology/approach\u0000Semi-structured interviews with five providers were analyzed using a basic qualitative approach.\u0000\u0000\u0000Findings\u0000Providers perceived the EAAP as acceptable and revealed potential mechanisms of change supporting well-being, including aspects related to the physical and social environment and person with dementia. Linkages identified among the EAAP and its physical and social context support its complexity. Providers explicated program practices that promoted safety and QoL, such as implementing staff trainings and tailoring activities to each person’s preferences and needs. {"title":"Linkage of social care and hospital admissions data to explore non-delivery of planned home care for older people in Scotland","authors":"J. Evans, K. Methven, N. Cunningham","doi":"10.1108/QAOA-05-2018-0018","DOIUrl":"https://doi.org/10.1108/QAOA-05-2018-0018","url":null,"abstract":"\u0000Purpose\u0000As part of a pilot studyassessing the feasibility of record-linking health and social care data, the purpose of this paper is to examine patterns of non-delivery of home care among older clients (>65 years) of a social home care provider in Glasgow, Scotland. The paper also assesses whether non-delivery is associated with subsequent emergency hospital admission.\u0000\u0000\u0000Design/methodology/approach\u0000After obtaining appropriate permissions, the electronic records of all home care clients were linked to a hospital inpatient database and anonymised. Data on home care plans were collated for 4,815 older non-hospitalised clients, and non-delivered visits were examined. Using case-control methodology, those who had an emergency hospital admission in the next calendar month were identified (n=586), along with age and sex-matched controls, to determine whether non-delivery was a risk factor for hospital admission.\u0000\u0000\u0000Findings\u0000There were 4,170 instances of “No Access” non-delivery among 1,411 people, and 960 instances of “Service Refusal” non-delivery among 427 people. The median number of undelivered visits was two among the one-third of clients who did not receive all their planned care. There were independent associations between being male and living alone, and non-delivery, while increasing age was associated with a decreased likelihood of non-delivery. Interview data were collected and analyzed from 22 formal service providers who helped older adults and their families navigate health and social care resources in one Western Canadian city.FindingsAlthough acknowledging structural barriers to service access, participants emphasized individual empowerment as their dominant strategy, interpreting their roles as providing information and education about services. In part, these interpretations may reflect the limited nature of their ability to help broker access or advocate; in part, they may also reflect the broader political and economic discourses surrounding care in Canada.Research limitations/implicationsWhen providers position navigation and access to care as individual problems, this can obscure structural burden as well as potential inequities among older adults. Future research should examine whether navigational role interpretations are similar or different to those of navigators in other regions.
