There is a well-recognized association between DM and cancers. In Korea, several DM cases have been reported to be associated with stomach cancer, breast cancer, acute lymphoblastic leukemia, lung cancer, and tonsil cancer. However, an association between DM and lymphoma in Korea has not been reported up to now. Bronchiolitis obliterans with organizing pneumonia (BOOP) is one type of ILD, and this is characterized by the proliferation of granulation tissue in the bronchioles, alveolar ducts and some alveoli and interstitial infiltration by chronic inflammatory cells. It develops as a manifestation of RA or as a side effect of anti-rheumatic drugs in patients with RA. We experienced a 41-year-old female patient with RA who developed BOOP during the treatment with methotrexate and bucillamine. She presented with cough and sputum for several months and pleuritic chest pain for one week. The chest radiograph showed bilateral multifocal consolidations. Vesicle formation in dermatomyositis is rare. We report a case of dermatomyositis associated with ovarian cancer in a 62-year-old woman who had vesicles and bullae on her arms. She had erythema and edema on the face, chest, abdomen, and shoulder for 2 months. Diagnosis of dermatomyositis was established by clinical manifestations, muscle enzyme elevation, and a characteristic electromyogram. While TNF inhibitors can effectively decrease the inflammation, they also decrease resistance to infection and affect immune surveillance mechanisms. Clinicians are sometimes faced with special situations such as chronic hepatitis B infection, pregnancy, complicated medical problems and older populations when prescribing TNF inhibitors. These conditions are affected by TNF inhibitors, which may induce poor clinical outcomes as well as side effects. To date, there are limited clinical data and recommendation guidelines on patients receiving TNF inhibitors in these special situations. We reviewed the effect of TNF inhibitors on each condition with review of the literatures concerning the clinical trials of TNF inhibitors in various rheumatic conditions. Methods: This study was based on data from chronic disease management surveys of Incheon. The surveys were conducted from 12 Sept 2005 to 7 Oct 2005 on 5,144 households using proportional stratified sampling methods. Three questionnaires were performed on all study subjects. All analyses were done using SPSS version 11.5. Results: Out of 16,623 subjects, the mean age was 34.5±20.2 (years, ±SD). The crude prevalence rate of musculoskeletal diseases were as follows: osteoarthritis 3.2%, rheumatoid arthritis 1.1%, spondyloarthropathy 0.3%, gouty arthritis 0.1%, Behcet’s disease 0.04%, herniated disc 1.1%, and osteoporosis 0.8%. Age-standardized prevalence of rheumatoid arthritis was 1.6%, and osteoarthritis 5.2%. The prevalence of rheumatoid arthritis and osteoarthritis tended to be higher among older age groups, low socioeconomic/education state, and homemakers. The age-standardized prevalence of arthritis was higher in rural than in urban area. There were more concurrent chronic diseases such as hypertension or diabetes mellitus in the arthritis group than in the whole population. Questionnaires were performed on subjects with musculoskeletal disease. Although cytomegalovirus infection does not occur frequently, it is a major cause of morbidity and mortality in patients suffering with connective tissue diseases, including dermatomyositis. Cytomegalovirus pneumonitis and retinitis has been rarely reported in patients with dermatomyositis. The angiogram showed multiple microaneurysmal and thrombotic lesions, notably in the renal, mesenteric and tibial arteries. A skin biopsy of the upper dermis of the left thigh with an erythematous skin rash showed the infiltration of mononuclear leukocytes in the perivascular area. During hospitalization, he was diagnosed with chronic hepatitis B, and was treated with lamivudine, and corticosteroid, azathioprine to control the PAN. The knee joint pain improved progressively, and the patient could walk normally after several months. The occurrence of CIDP in patients with systemic lupus erythematosus (SLE) has been rarely reported. We experienced a case of a 33 year-old woman with SLE and she presented with fever, abdominal pain, a tingling sensation of both of her hands and feet, and symmetrical weakness in both the proximal and distal extremities. Her symptoms had persisted for over 1 months before she visited our department. The CSF examination showed an elevated protein level and the nerve conduction studies revealed demyelination.
