However, plant-based options have attracted an increasing market, in part due to health issues associated with dairy ingestion. This review compares the nutrient content of dairy and non-dairy milks and discusses potential health implications. Nutrient data were obtained from the USDA’s FoodData Central website. Cow’s milk is higher in fat (including saturated fat) and sugar than most plant milks. Both cow’smilk and soymilk are higher in protein than almond, oat, rice, or coconut milk. Regarding micronutrients, cow’s milk is high in calcium, and, in some countries, is often fortified with vitamin D and vitamin A. Many plant milks are fortified with similar amounts of these micronutrients. Cow’s milk contains estradiol, which may be implicated in the risk of certain cancers and reduced fertility. Soymilk contains isoflavones, which are associated with reductions in prostate and breast cancer risk, menopausal symptoms, and plasma cholesterol concentrations. There is no apparent health rationale for recommending cow’s milk over plant-based milks.

Prospective studies examining the relationship between dairy intake and cancer of the prostate, breast, ovary, and colorectum were reviewed. These studies indicate that dairy consumption is associated with prostate cancer risk, possibly as a consequence of dairy-induced increases in circulating IGF-1 concentrations and the calcium and estrogen content of dairy products. Evidence also suggests a positive association between dairy intake and ovarian cancer. Findings on breast cancer have been mixed; however, recent studies of populations with a wide range of dairy intakes have shown clear associations between dairy intake and breast cancer risk. In contrast, there is a negative association between dairy products and colorectal cancer, which is likely driven by the protective effect of calcium. Current evidence suggests that dairy intake is associated with increased risk of prostate, ovarian, and possibly breast cancer, and reduced risk of colorectal cancer.

This article reports a case of a post coronary artery bypass graft, morbidly obese, uncontrolled diabetic patient who was able to reduce weight, reduce blood pressure medications, and come off insulin with a strictly plant-based diet.\u0000Case report: A 61-year-old man was seen in follow-up post coronary artery bypass grafting. He had a history of hypertension, hyperlipidemia, diabetes, morbid obesity, coronary artery disease post-stenting in 2000, and heart failure preserved ejection fraction. Post-discharge, he felt tired, had gained 7 lb with hemoglobin A1c (HbA1c) 10% on high-dose insulin, and had uncontrolled hypertension on amlodipine, bumetanide, and carvedilol. He was started on a strictly plant-based diet. He was compliant on subsequent visits. At six months follow-up, he had excellent blood pressure and glucose control. He was able to come off amlodipine, bumetanide, and insulin completely. He lost 70 lb and was more active.\u0000Discussion: A plant-based diet effectively reduces obesity, diabetes, hypertension, hyperlipidemia, and heart disease. Epidemiological studies have shown lower cardiovascular and all-cause mortality with a plant-based diet.\u0000Conclusion: A whole food, plant-based diet should be recommended to prevent and control cardiovascular risk factors and overall cardiovascular morbidity and mortality.

However, in clinical practice, the assessment of diet and exercise followed by meaningful intervention in these areas are not often emphasized by physicians, and implementation by patients is often lessthan optimal. In this case, there was a dramatic improvement in cardiovascular risk factors, including a profound degree of weight loss, along with a fall in systolic pressure, serum triglycerides, low-density lipoprotein cholesterol (LDL-C) and total cholesterol, possibly driven by the improvement in the microbiome induced by the patient’s exercise and dietary change to a whole food, plant-based diet. These remarkable changes represent an unusual response in terms of the degree, but not the direction, of improvement with exercise and a plant-based diet. The patient’s prospects of sustained weight loss and lifelong lower cardiovascular risk appear to be best with his adopted lifestyle changes, particularly the whole food, plant-based diet.
