Methods: In the setting of COVID-19 pandemic, we incorporated the use of spectral cardiac CT in patients with ischemicstroke to rule out the presence of cardioembolic sources, thrombotic complications or myocardial damage in a single session.Since July 2020, a delayed-phase image acquisition was incorporated to cardiac CT scans in the context of ischemic stroke.We describe four representative cases of the usefulness of the method and the cardiovascular findings. Results: We present four cases recorded recorded within a 40-day period. Two patients with patients with cardioembolic source(aorta and left atrial appendage) and two with ischemic stroke of undetermined source with evidence of cardiomyopathy(ischemic and non-ischemic). Conclusions: In the setting of ischemic stroke, spectral cardiac CT with delayed acquisition could be useful to rule out thepresence of cardioembolic sources and identify the underlying etiology.","PeriodicalId":447734,"journal":{"name":"Argentine Journal of Cardiology","volume":"42 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-11-12","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129757775","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Luego de la primera fase de la encuesta ARGEN-IAMST,se invito a los centros a continuar con el registro de IAM. Se incluyeron pacientes con IAM con elevacion del segmento ST en elelectrocardiograma de hasta 36 horas de evolucion.Resultados: La poblacion analizada abarco 2464 pacientes asistidos en 78 centros. La media de edad fue 60 ± 12 anos y el 80% fuede genero masculino. Los factores de riesgo prevenibles se distribuyeron del siguiente modo: tabaquismo 45%, hipertension arterial58%, diabetes 24% y dislipidemia 41%. El 11% tuvo antecedente de enfermedad coronaria. El 88% recibio reperfusion; el 21% de estospacientes recibieron tromboliticos y al 89% se le realizo angioplastia. La demora desde el inicio de los sintomas hasta la admision fuede 130 minutos (RIC 25-75: 60-305); los medicos reportaron demoras hasta el tratamiento en el 49% de los casos, con impacto en lostiempos totales de isquemia (TTI). La mortalidad intrahospitalaria fue del 8,7%. En el analisis multivariado, ser tratado en un centrocon hemodinamia no se asocio de forma independiente con la supervivencia.Conclusiones: Los datos actuales del registro continuo de IAM en la Argentina son similares a los que mostro la encuesta de 2015.Las demoras hasta el tratamiento son importantes, especialmente por el tiempo de demora en la consulta de los pacientes, lo queimpacta en gran medida en los TTI.","PeriodicalId":447734,"journal":{"name":"Argentine Journal of Cardiology","volume":"42 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-09-21","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116287271","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, it is often a poorlycontrolled disease, mainly because health care systems are oriented to the attention of acute diseases. The Argentine Ministry ofHealth proposed a new model for the care of hypertensive patients called MAPEC, based on the Chronic Care Model.Objective: The aim of this study was to evaluate the impact of MAPEC implementation to improve blood pressure (BP) control,changes in lifestyle, disease knowledge and treatment adherence in hypertensive patients treated in three primary health care centersof the City of Salta, Argentina.Methods: Blood pressure was measured with a digital blood pressure monitor and the Batalla and Morisky-Green-Levine tests wereused to evaluate disease knowledge and adherence to treatment, respectively.Results: The study included 232 patients. After model implementation, significant differences (p<0.0001) were found in bloodpressure control, disease understanding, treatment adherence and changes in hygienic-dietary measures. There was a decrease inmean BP with a reduction of 12.97 (95% CI: 9.52-16.42) mm Hg and 6.93 (95% CI: 4.70-9.16) mm Hg in systolic and diastolic BP,respectively.Conclusions: There was evident improvement in the analyzed health parameters after MAPEC implementation. This model canbe easily adapted to primary health care centers at a low cost. In addition, it agrees with the 25×25 WHO targets to reduce 25%cardiovascular premature deaths by 2025.","PeriodicalId":447734,"journal":{"name":"Argentine Journal of Cardiology","volume":"74 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-05-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121391557","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

QT interval measuredfrom a standard electrocardiogram (ECG) requires additional personnel and risk of infection. Novel technologies to obtain anECG connected to smartphones provide an alternative for the evaluation of corrected QT interval (QTc).Objective: The aim of this study was to evaluate the feasibility of using a single-lead ECG device to measure the QT interval in patientswith suspected or confirmed COVID-19 before receiving treatment with drugs that can prolong the QT interval.Methods: The ECG was obtained with a KardiaMobile (KM) device and transmitted to a smartphone. The ECG recordings weresaved as pdf files and electronically submitted to the electrophysiology section which centralized the reception and assessed themeasured QT and QTc intervals.Results: A total of 31 patients (mean age 61 years, range 20-95 years) with suspected COVID-19 enrolled for treatment with hydroxychloroquine,azithromycin, ritonavir or lopinavir were analyzed. The recordings could be read in all the cases and had to be repeatedin two cases. The mean value of the QTc interval was 423 ms (range 380-457 ms) in men and 439 ms (range 391-540 ms) in women.The response time since the ECG recording was submitted for analysis was 11 min (range 1-155).Conclusions: The QTc interval could be measured from ECG recordings obtained with KM devices connected to a smartphone andtransmitted to a centralized reading center in all patients.","PeriodicalId":447734,"journal":{"name":"Argentine Journal of Cardiology","volume":"14 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-05-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122608543","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
