Alves","doi":"10.5935/1676-0166.20210001","DOIUrl":"https://doi.org/10.5935/1676-0166.20210001","url":null,"abstract":"Objectives: To assess the exposure of neonates admitted to a neonatal intensive care unit to risk factors for hearing loss. Methods: This is an observational, cross-sectional, and descriptive study, carried out between March and June 2017. Observations and data collection were carried out in 16 days, in different shifts. The risk factors evaluated were: use of ototoxic drugs, congenital infections, mechanical ventilation for more than five days, birth weight below 1500g, ear malformations, and exposure to high noise levels. Convenience sampling, composed of premature neonates admitted to the Hospital of Santo Ângelo, located in the municipality of Santo Ângelo, Rio Grande do Sul, Brazil. Results: The study covered 30 neonates, 50% male, and 50% female. The average hospital stay was 20 days and the gestational age was 32 weeks. All neonates were exposed to at least one risk factor: use of ototoxic drugs (93.30%), weight below 1500 g (46.60%), mechanical ventilation for more than five days (23.33%), meningitis (3.3%), and congenital infections (3.3%). The other risk factors were not found in the sample. Conclusion: It was possible to assess the presence of risk factors in neonates, comparing with data from the literature. Still, it was noted the importance of neonatal hearing screening, to diagnose and intervene early in hearing loss, as well as the need for health care programs, newborns and mothers, as well as strategies that reduce the occurrence of risk factors in neonatal units. Data sources: Scientific articles were searched in Pubmed and UpToDate databases between the years 2011 and 2020, giving preference to the most updated and didactic publications. Summary of findings: The topics covered were: definition of short stature; how to use the growth chart; what should be researched in the anamnesis; what to expect on physical examination; which complementary exams should be requested in a first evaluation and some causes of short stature. Conclusions: The complaint of short stature should be confirmed using the growth charts. Cat","doi":"10.5935/1676-0166.20210002","DOIUrl":"https://doi.org/10.5935/1676-0166.20210002","url":null,"abstract":"Objective: To develop a digital tool, in a free platform, to optimize the management of first-hour sepsis care in pediatrics, aligned with the protocol of the American College of Critical Care Medicine (ACCCM). Method: A Google form was developed as a management and patient safety tool based on indicators of inadequacy regarding the protocol follow-up and meetings with the team of the Pediatric Intensive Care Unit. Initially, the local first-hour sepsis care was analyzed with a retrospective study of 34 patients with a median age of 12.2 months (0.2-185.2) was used, of which 19 (55.9%) were male, having 20 (58.8%) the diagnosis of severe sepsis and 14 (41.2%) of septic shock. Results: Of the 34 patients, two died (5.9%), and 22 (64.7%) had no follow-up of at least one of the ACCCM main recommendations. Based on this, the Google form was developed with 25 sessions that conduct the specific attendance and allow the flowcharts consultation by collecting the main data of the first-hour care in a targeted, educational, practical, fast, voluntary and anonymous way, besides providing immediate feedback to both the physician and the Unit. Conclusions: The retrospective study indicates possible inconsistency of the data record and not the lack of follow-up of the recommended protocol, since mortality was much lower than expected (5.9% versus 10-20%). Therefore, the digital tool could answer this issue allowing easily registration of essential information in the treatment of sepsis in pediatric care, as well as the management of this data, being easily replicable.
