The aim of the study is to analyze the provision of rehabilitation measures for patients who have suffered cardiovascular accidents in the Astrakhan region. Materials and methods - based on the data of the annual reporting statistical forms No. 12 \"Information on the number of diseases registered in patients living in the service area of the medical institution\" and No. 14 \"Information on the activities of the hospital\", the calculation and analysis of indicators of general and primary morbidity was carried out ( on appeal), hospitalized morbidity, hospital mortality, the need for rehabilitation measures and their provision of adult patients aged 18 years and older with diseases of the circulatory system, including cardiovascular accidents in the Astrakhan region. Results and discussion - the article shows the general situation with the incidence of diseases of the circulatory system of the adult population, including acute coronary syndrome and acute cerebrovascular accident in dynamics from 2014 to 2020, discloses the organizational and structural aspects of the existing system of organizing rehabilitation care in the region. The results of assessing the satisfaction of the needs of the adult population of the region in neuro- and cardiac rehabilitation by medical organizations licensed for medical activities with an indication of the work (services) for medical rehabilitation and participating in the federal and territorial program of state guarantees of free provision of medical care to citizens in the region are presented. It was revealed that less than half of those in need receive medical rehabilitation assistance at the expense of the mandatory medical insurance fund. Proposals have been formulated to increase the availability of rehabilitation care for patients who have undergone cardiovascular accidents. Conclusion. The problem of pregnancy management is of scientific and practical interest, it is caused by an increase in the number of multiple pregnancies. The current trend towards all cases of multiple pregnancies is explained by the activity of assisted reproductive technologies, the ever-increasing age of women in labor. This article discusses the features of the gestation period in multiple pregnancies, characteristic complications, as well as the features of the condition of the fetus and newborn in this condition. Multiple pregnancy is a model of fetoplacental insufficiency, which is explained by the fact that evolutionarily a woman's body is adapted to bear one fetus. According to research data, the course of multiple pregnancies is more often associated with the presence of various complications than the course of a singleton pregnancy. One of the most serious complications of multiple pregnancy is fetal-fetal transfusion syndrome. The occurrence of this complication requires the use of special tactics of pregnancy management, continuous monitoring of the condition of the mother and fetus, readiness to make emergency correct decisions about prolonging pregnancy or delivery, readiness to provide highly specialized assistance to the fetus and subsequent newborns. Clinical management of pregnancy, treatment and prognosis depend on the extent of development of feto-fetal transfusion syndrome, gestation, on which has developed the syndrome, presence or absence of the mother's genital or extragenital pathology, as well as the fetus. Optimization and development of outpatient care is the primary task of healthcare. For the Penza region, according to the resolution of the Government of the Penza region of 02.10.2013 N 743-PP, improving the quality of outpatient care in the field of maxillofacial surgery and increasing its availability is one of the primary tasks for our region. In this regard, the assessment of the work of the outpatient office of the Department of Maxillofacial Surgery of the Consultative Polyclinic of the State Budgetary Healthcare Institution of the Penza Regional Clinical Hospital named after V.I. N.N. Burdenko.\u0000Aim: to analyze the work and effectiveness of a dentist - surgeon in the outpatient office of the consultative polyclinic GBUZ Penza Regional Clinical Hospital named after V.I. N.N. Burdenko.\u0000Materials and methods. To achieve this goal, a retrospective analysis of the primary accounting and reporting medical documentation for 2018–2020 was carried out, a questionnaire survey of patients hospitalized through an outpatient office in the Department of Maxillofacial Surgery of the N.I. N.N. Burdenko. Statistical data analysis procedures were carried out using MS Office Excel 2010 spreadsheets and PASW Statistics 18 statistical software package.\u0000Results. The analyzed accounting and reporting medical documentation for 2018–2020, the results of the questionnaire indicate the ineffective work of the doctor surgeon - dentist in the outpatient office of the consultative polyclinic GBUZ POKB im. NN Burdenko, the workload of the department with non-core hospitalizations, the increased workload on doctors - the department of maxillofacial surgery. The ways of solving the problem of replacing a dentist-surgeon with a doctor-maxillofacial surgeon are proposed; on the organization of a rehabilitation room for patients of the adult and pediatric population with pathology of the maxillofacial region; to create a single case history.\u0000Conclusion. More than 7 million people die from oncology every year in the world. The system of organizing the provision of medical care to the population with oncological diseases in the Russian Federation is currently undergoing a stage of improvement. New programs are aimed at early diagnosis and prevention of malignant neoplasms. Each region has the right to create and implement routing taking into account the territorial characteristics and the existing material and technical base and personnel. The routing system, created for a specific region, determines the order of treatment of patients and the functions of all links in the system of providing medical care to patients with oncopathology. An optimized system for routing patients according to the \"Oncology\" profile for patients with malignant neoplasms of the maxillofacial region should be structured in accordance with the regional healthcare system. A complex system of interaction, an information system capable of containing and processing a set of information, a trained interested staff dictate the need for making management decisions in the direction of optimizing the oncological service. Management decisions and technologies should be focused on improving the quality and speed of patient admission, providing highly qualified medical care using modern equipment and algorithms. The staff, in turn, determines the basis of the health care system, in connection with which it is necessary to continue education, identify and search for specialists that a specific organization needs, on which the functioning of the entire service for providing medical care to patients with oncopathology of the maxillofacial region depends. In general, the optimization of the system of providing medical care to patients with malignant neoplasms of the maxillofacial region dictates the need to combine organizational measures and a clinical approach. Despite the presence in the arsenal of dentists of a significant number of means and methods for solving this problem, it does not go away, but, on the contrary, is overgrown with new complaints. In this work, we analyzed the frequency to analyze the frequency of choice of therapeutic methods for treating discoloration of teeth by patients of dental clinics. A questionnaire was developed for a dentist on the patient's choice of a therapeutic method for eliminating dental discoloration and a survey of 30 dentists (10 - therapists, 10 - orthopedists and 10 - general dentists) was conducted for 2019 and 6 months of 2020. It was found that during this period, 184 out of 331 patients who applied to dental clinics with this problem used therapeutic methods for eliminating tooth discoloration: tooth enamel coating with varnish (\"Colordent\" + adhesive \"Single Bond\") was carried out in 16.6% of patients, clinical bleaching of enamel in 32.6% of patients with tooth discoloration, intracoronal bleaching followed by direct restoration (replacement of existing fillings with new ones) in 53.8%. The analysis showed that therapeutic methods for eliminating discoloration of teeth predominate slightly, 55%. Of course, in choosing this direction, the doctor is guided by the clinical situation, IROPZ, taking into account the wishes of the patient and his capabilities. Materials and methods. Nutrition characteristics were studied based on information obtained from a survey of amateur athletes aged 40 years and older competing in track and field events at distances of 10 kilometers or more. In total, 1800 questionnaires were distributed, 1649 completed questionnaires were collected. The following food characteristics were taken into account: the presence or absence of adherence to any diet, the frequency of meals, the regularity of meals, the nature of the food, the significance of the quality of the food consumed, as well as the presence of the fact of eating flour, sweet. The analysis was carried out in three age groups: 40–49 years, 50–59 years and 60 years and older. Results. According to this study, the desire to eat exclusively healthy food with lots of fresh vegetables and fruits, the commitment to diet, the desire to control the quality of food in the group of veteran athletes does not increase, but decreases. People who are engaged in sports, aged 40 years and older, try to eat often and at the same time. In men, with increasing age, the number of people who want to take the largest amount of food for breakfast decreases, while in women, the opposite trend is observed. The commitment to snacks is more characteristic of women than men. Identification and procedural structuring of key processes of drug quality control systems in Russia was carried out. The matrix of key procedures was formed in coordinates of agency, target, time, and level characteristics for qualitative mapping of the processes and procedures. To quantify the control measures modality in the coordinates \"product-context\", the structural coefficient of distribution of procedures was calculated for each section of the commodity distribution network: the product emphasis in the production/ manufacturing segment and the environmental emphasis in the system of market distribution of medicines. Based on the results of risk-based mapping, it was determined that the segments of wholesale and retail pharmaceutical activities are characterized by a shift in control measures to the segment of external reactive procedures, which seems to be a negative trend. It is shown that the production of medicines is the leading segment in the implementation of risk-based quality control procedures for medicines, while in the wholesale and retail trade segment this indicator is 95% lower. Over the past decades, with the development of endovascular interventions and the development of drug-eluting stents, percutaneous coronary intervention (PCI) has become the main method of revascularization after ACS. [1,6]. Acute coronary syndrome (ACS) includes clinical manifestations such as unstable angina pectoris (NS), acute non-ST-segment elevation myocardial infarction (STEMI), and ST-segment elevation myocardial infarction (STEMI). Approximately 40% of all patients diagnosed with ACS have multivessel coronary artery disease, for which coronary artery bypass grafting (CABG) is better than PCI. [2]. The majority of studies comparing PCI and CABG have mainly included patients with stable coronary artery disease who underwent planned myocardial revascularization, rather than those requiring emergency or urgent myocardial revascularization. Thus, the results of these studies have limited applicability to patients with ACS. However, the long-term results of these studies, in particular the low need for re-revascularization, a lower rate of re-myocardial infarction, and the survival benefits of CABG, still need to be considered when determining the best course of treatment for ACS. The current recommendations for treatment in most patients with ACS give preference to early revascularization using PCI or CABG [3, 4]. Thus, our main goal here is to provide the current indications and options for surgical revascularization of the coronary arteries, including current guidelines and the latest published literature. In STEMI patients, early PCI of the main lesion remains the gold standard because it provides the fastest revascularization of the ischemic myocardium and is generally better tolerated than emergency CABG [5]. Since up to 50% of STEMI patients have multivessel coronary artery disease, early arterial revascularization without myocardial infarction has been recommended to provide optimal opportunities for myocardial rescue, reduction of ischemic watershed and improvement of left ventricular function [6,7]. A clinical case of successful beating coronary artery bypass grafting in a patient with ST-segment elevation myocardial infarction, multivessel coronary artery disease and low ejection fraction. Various causes of sexual dysfunction when taking COC have been studied: a decrease in the concentration of testosterone in the blood due to an increase in globulin, which binds sex steroids, under the influence of oral contraceptives. This mechanism is realized due to the action of two components of COC, both estrogenic and progestogenic. Female sex hormones are known to play an important role in the physiology of sexual desire and behavior. The importance of the most important of them, estrogens, in the sexual behavior of women is to prevent atrophic processes in the vagina, enhance vaginal and vulvar blood circulation, as well as to maintain sensory peripheral perception and the beneficial effect of these processes on the central nervous system. The presented clinical case demonstrates the positive effect of Claira (a drug that is a COC containing estradiol valerate in combination with dienogest with a dynamic dosage regimen) on sexual function. The content of this oral contraceptive of estrogen, identical to natural, eliminates the negative impact of COC on sexual function. Estradiol and estriol, formed during the metabolism of estradiol valerate, actively bind to receptors and have their effects in the tissues of the urogenital tract, having a positive effect on the manifestations of sexual dysfunction. These include the \"cytokine storm\", impaired hemostasis, neurotropicity and neurovirulence of SARS-CoV-2. The persistence of the virus in the body affects the course of comorbid diseases, which requires a systematic approach to patient treatment. In this paper we discuss the data from modern research papers and clinical cases both from updated international literature and from our own clinical practice: it is the most frequently met condition - Guillain-Barré syndrome – acute inflammatory demyelinating polyneuropathy. We present both rare and common neuro-muscular conditions, their clinical onset, possible pathogenesis, variants of manifestation. According to the data from current publications, the evaluation and management of patients suffering from Guillain-Barré syndrome associated with new coronaviral infection SARS-CoV-2 is does not have major differences with the classical ones, that are not associated with the pandemic. Our patient that we present as our own case, was hospitalized, and observed as an in-patient. She was treated using plasma exchange in the algorithm of management. The outcome of this case was positive. We conclude that neuromuscular manifestation of new coronaviral infection SARS-CoV-2 could be new and possibly associated with this disease, but also could be described as worsening of the premorbid disorders.
