Material and Method: This randomized control study, took place in the outpatient clinic of Mansoura General Hospital and Mansoura University Hospital, Mansoura, Egypt. Thirty stroke patients were divided into two groups and assigned randomly into the study group (n=15), which received cryo-airflow therapy, a conventional physical therapy program, and advised to wear a night ankle foot orthosis (AFO), or into the control group (n=15), which received the same program, but without the cryo-airflow therapy. Pre and posttreatment assessment for all patients was done by the modified Ashworth’s scale (MAS), the Hoffman reflex / Myogenic response (H/M) ratio of the soleus muscle on the affected side, and by two-dimensional (2D) motion analysis for ankle dorsiflexion range of motion (ROM) during the stance phase of gait. Results: Mann–Whitney and MANOVA tests revealed a significant improvement of MAS scores (p = 0.006) and ankle ROM scores (p = 0.005), while the difference in H/M ratios was non-significant (p = 0.063) in the comparison between study and control groups. Clinical and electrophysiological measures of spasticity were significantly correlated with each other (ρ = 0.765) and negatively with the ankle ROM (ρ = -0.635) when analyzed by Spearman correlation coefficient at (p = 0.000). Discussion: Cryo-airflow therapy is considered to be an effective method to reduce spasticity in calf muscle and to improve ankle dorsiflexion ROM of the affected limb in stroke patients.","PeriodicalId":44485,"journal":{"name":"Journal of Clinical and Analytical Medicine","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2019-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42157929","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

As it is encountered infrequently, there have been only a few studies on it, mostly in the form of case series or just a case presentation in the literature. In this study, we aim to investigate common clinical features and radiological measurements between patients admitted with symptoms of gastric outlet obstruction due to SMAS and those without SMAS. Material and Method: This study was conducted prospectively on individuals admitted to our clinic presenting with symptoms of gastric outlet obstruction in the previous six months and who had undergone computerized tomography. (Here, SMAS is defined by a narrow aortomesenteric angle and a short aortomesenteric distance.) Demographic features and radiological measurements were compared between patients with and without SMAS. The results of a three-month follow-up of each subject with SMAS were recorded. Results: This study includes a total of 92 subjects, 14 (15%) of whom had a diagnosis of SMAS (Group 1) and 78 (85%) of whom did not (Group 2). Group 1 had a predominance of female and significantly younger patients (mean age: 31.1 ± 10.2), compared to Group 2 (mean age: 44 ± 11.7) (p < 0.001). Abdominal subcutaneous fat tissue thickness was thinner in group 1 than in group 2 (18.1 ± 8.1 and 23.7 ± 12.2, respectively; p = 0.039). Eleven of the 14 SMAS patients (78.5%) recovered with medical treatment, but in the other three cases, surgical intervention was considered. Discussion: SMAS is an important and preventable cause of small bowel obstruction. Although it is a rare condition in the general population, accurate diagnosis of the disease leads to improvement of symptoms; simple medical management can preclude the need for surgery.","PeriodicalId":44485,"journal":{"name":"Journal of Clinical and Analytical Medicine","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2019-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42751269","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Subacromial impingement syndrome (SAIS) is one of the most frequent causes of shoulder discomfort. Osteochondroma-related subacromial impingement syndrome is rarely reported in the literature. In this case report, we describe the treatment of a 19-year-old female patient with a solitary osteochondroma who presented with pain and restricted movement in the shoulder region. Direct radiography indicated that a bone density mass had almost completely filled the subacromial region. The unique aspect of this report was the origin of the exostosis. Although osteochondromas originating from the posterior superior glenoid are uncommon, they can cause SAIS. Removal of the mechanical irritation with arthroscopic or open surgical techniques can reduce pain and facilitate recovery of shoulder function.","PeriodicalId":44485,"journal":{"name":"Journal of Clinical and Analytical Medicine","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2019-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48671199","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

HO mostly occurs on and around the hip joint. Shoulder and elbow are the other common-affected joints. There are three well-known etiologies of HO as of traumatic, neurogenic, and genetic. But, HO is a rare clinic entity in hemiplegic patients. HO is a condition that affects the patient’s mobility, self-care, and independence. In hemiplegic patients, especially in case of joint-related pain, local tenderness, increased temperature, and swelling HO should be considered in the differential diagnosis. The treatment options of HO are preserving the affected joint’s range of motion, pharmacological treatment (etidronate, nonsteroid anti-inflammatory drugs), radiotherapy, and surgical excision of the mature bone tissue in the advanced cases. This report presents a case with HO developed on the left elbow 2 months after the stroke due to a hemorrhagic cerebrovascular event.","PeriodicalId":44485,"journal":{"name":"Journal of Clinical and Analytical Medicine","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2019-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46928490","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

GSM: +905383425644 E-Mail: drysezgin@gmail.com ORCID ID: 0000-0002-3626-0264 Abstract Aim: We think that vitamin B12 may affect serum lipid parameters because it is a cofactor of the enzyme that plays a role in fatty acid catabolism, and also because of its association with obesity and increased risk of myocardial infarction in its deficiency. In this study, we aimed to investigate whether vitamin B12 insufficiency, which is common and associated with many diseases, is related to serum lipid parameters. Material and Method: This is a cross-sectional retrospective study. In this retrospective study, data such as serum vitamin B12, glucose, lipid, thyroid function tests, ferritin, and demographic data such as age and gender, were obtained from records of the examinations of the patients who applied to the family medicine clinics, at the northern region in Turkey. The study included 228 patients who underwent concurrent lipid and vitamin B12 analysis and who met the inclusion criteria. Patients who met the exclusion criteria were not included in the study. Results: There was a statistically significant, low positive correlation (r = 0.278, p = 0.001, n = 228) between serum HDL and vitamin B12 levels. There was a statistically significant low negative correlation (r = 0.322, p = 0.001, n = 228) between serum triglycerides and vitamin B12 levels. Discussion: As a result, we can say that serum vitamin B12 levels affect lipid parameters. We may suggest that the risk of diabetes, obesity, and coronary artery disease may increase due to high triglyceride or low HDL levels in the absence of vitamin B12.","PeriodicalId":44485,"journal":{"name":"Journal of Clinical and Analytical Medicine","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2019-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"45941312","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
