Relational continuity is associated with multiple, overlapping benefits for patients, doctors and society. Continuity increases trust, patient satisfaction and adherence to advice, while reducing hospital use and deaths. Repeated consultations are needed with a patient for a GP to acquire enough ‘accumulated knowledge’ to develop a sense of continuing responsibility. This fosters GP sensitivity and mutual understanding, which enable GPs to provide ‘higher-level’ quality of care. However, the level of continuity is reducing in UK general practice. It is a condition requiring active consideration to achieve diagnosis. The main pathologies discussed here are primary adrenal insufficiency (PAI) and tertiary adrenal insufficiency. PAI is also commonly termed Addison’s disease. Secondary adrenal insufficiency is not considered in detail in this article. The article will address the underlying pathophysiology, presentation and investigation of adrenal insufficiency. An incorrect diagnosis can have a significant impact on the patient by delaying treatment, increasing the risk to their health and in some cases may even affect a person’s employment. Faltering from these predicted centiles will often result in a review in primary care accompanied by heightened parental anxiety. Although a drop in growth velocity may well be normal for an individual, it may also be the first warning sign of underlying pathology. The challenge for clinicians is to differentiate what is normal from faltering growth that occurs as the culmination of underlying medical, environmental, or psychosocial factors.
