Aim of the study. Based on an analysis of the medical and demographic situation in the Amur Region, the Far Eastern Federal District and the Russian Federation in the context of the COVID-19 pandemic, recommendations are given for making managerial decisions at the municipal, regional and federal levels. Materials and methods of research: statistical, analytical. The materials of official state statistics of the Ministry of Health of the Russian Federation and Rosstat were used. Results and discussion. The mortality rates of the population of the Amur Region, the Far Eastern Federal District and the Russian Federation for the years of analysis (2016-2020) have been studied. It was revealed that in the Amur region. the indicators are higher for all the years of analysis, in 2020 they amounted to 16.2 parts per thousand, in the Far Eastern Federal District - 13.9 parts per thousand, in the Russian Federation - 14.6 parts per thousand. The difference in mortality rates in the administrative entities of the Amur Region (2020) is 2.3 times. The mortality rate of the population of the Amur Region from COVID-19 (in 2020) was 0.84 parts per thousand 00, for the urban population it is higher - 0.93 parts per thousand 00, than for the rural population - 0.67 parts per thousand 00. The first detected incidence of the population in the Amur Region, the Far Eastern Federal District and in the Russian Federation (2020) was studied. It has been established that in the Amur Region the figures are higher - 80294.8 per 100 thousand of the population than in the Far Eastern Federal District (74596.5), in the Russian Federation (75840.1). In the Amur Region, there are higher rates of primary morbidity with diseases of the digestive system. Very high rates were noted in Tynda and Zeya, which determines the need for an in-depth study of the causes of this incidence in the region. The frequency of detected COVID-19 in the Amur Region was 3141.3 per 100 thousand of the population (3.9% of the total number of primary incidence), in the Far Eastern Federal District - 3394.9 parts per thousand 00 (4.55%), in the Russian Federation - 3384.5 parts per thousand 00 (4.46%). The primary incidence of the entire population of the Amur Region has decreased over the years of analysis (2016-2020). Higher rates of newly diagnosed morbidity were noted in children (0-14 years old) and adolescents (15-17 years old) than in the general population, which requires strengthening preventive work with this age group of the population. Conclusion. The identified features of the medical and demographic situation in the Amur Region should be taken into account by the heads of governments at the municipal regional and federal levels in order to develop management decisions. We examined the health status of 155 people who suffered a coronavirus infection. Informed consent to the study was obtained from all participants. They were interviewed, asked to fill out a series of questionnaires to assess symptoms and quality of life, and underwent a medical examination. Our results showed that after 8 months, people who have had COVID-19 complain about the symptoms of post-covid syndrome. At the same time, the most common clinical manifestations were sleep disorders (50.9%), hypertension (47.7%), weakness (33.5%), dizziness (32.9%), shortness of breath (27.7%), headaches (24.5%), gastrointestinal disorders (diarrhea, abdominal pain) (18.7%). Ivanova","doi":"10.25789/ymj.2022.79.12","DOIUrl":"https://doi.org/10.25789/ymj.2022.79.12","url":null,"abstract":"Aim: To analyze the work in the specialized anesthesiology and intensive care unit in the Republic of Sakha (Yakutia) for patients with COVID-19 in the context of the pandemic. Materials and methods: A retrospective observational study was conducted based on work perfomed by anesthesiology and intensive care unit (AICU) in the State Budgetary Institution of the Republic of Sakha (Yakutia) \"Yakut Republican Clinical Hospital\" (YRCH) for the period since March, 2020, to December, 2021. Results and discussion: YRCH have repurposed the hospital beds and enhanced the material and technical equipment supply of the AICU. Given the epidemiological situation, the bed capacity and the staff schedule of the unit were in the scope of regulation. The medical staff level was 76.5 % in 2020 and 80 % in 2021. The level of nursing and medical attendant staff for the entire period was 100 %. During the study period 1,796 patients were admitted (488 patients in 2020, 1,308 patients in 2021). The bed turnover was 24.4 and 28.2 patients per bed, the average rate of bed occupancy was 136 and 244.6 days, the average length of stay of patients was 5.5 and 6.0 days in 2020 and 2021, respectively. In total, 1,015 patients have died (mortality rate - 56.5 %), with 281 patients to have died in 2020 (mortality rate - 58.0 %) and 734 in 2021 (mortality rate - 56.2 %). The mortality rate of patients significantly increases with age and amounted to 71.2 % in patients older than 81 years. The main cause for admission in AICU was acute respiratory failure (ARF), the treatment has used is stepwise respiratory therapy. Standard oxygen therapy (15-20 l/min) was effective in 64 (3.6 %) patients, high-flow oxygen therapy (HFOT) was used in 1,732 (96.4 %) patients, non-invasive mechanical ventilation (NIV) was used in 717 (39.9 %), invasive mechanical ventilation was used in 1,015 (56.5 %) patients. Conclusion: The complex of measures for the deployment of specialized anesthesiology and intensive care for adults with COVID-19 in the YRCH has allowed to succeed with an overflow of patients suffering a severe course of the disease.
