Our study aimed to investigate the effect of a simultaneous teamwork approach on metabolic and clinical outcomes of patients with Type 1 diabetes using an insulin pump. Material and Methods: This retrospective study included 49 previous insulin pump users with Type 1 diabetes who participated in a simultaneous teamwork protocol. Metabolic and clinical parameters were collected before and after the teamwork approach. A separate analysis of 16 patients in whom insulin pump was initiated with simultaneous teamwork and pregnancy data from 9 patients were also reported. Results: Simultaneous teamwork resulted in a significant reduction in the frequency of hypoglycemic episodes in previous insulin pump users with Type 1 diabetes. Hemoglobin A1c decreased, but the difference was statistically insignificant. Simultaneous teamwork was also efficient on patients initiated an insulin pump and for pregnancy follow-up in 2 separate subsets of patients with Type 1 diabetes. Material and Methods: This study is a bibliometric analysis of research publications acquired from the Web of Science database. We evaluated the first 100 articles receiving the most citations in the field of thyroid cancer since 1975. Then, all research publications were compiled using journal information into different categories based on the study type, country, research centers, authors, and publication dates. All data were reported as percentages, numbers, and graphs. Results: In this study, we listed 8,608 full-length English-language articles. Our findings showed that the most cited article received 4,325 citations to date, while the Journal of Clinical Endocrinology Metabolism, a Thyroid journal, received the most number of citations (n:6531), publishing seventeen of the articles from our study. Also, thirty-two articles were from the field of oncology. The country with the most number of publications was the United States of America (n=70) in the years 2005, 2008, and 2009 (n=8). Conclusion: It is essential to acknowledge the top-cited articles in thyroid cancer since they contain essential information, research, and advances in its treatment. Our analysis of the first 100 articles included the most important papers with the most significant impact on thyroid cancer research. {"title":"Anti-Insulin Antibody Syndrome Following Alpha-Lipoic Acid Use in Turkish Patients: Report of Three New Cases","authors":"Emine Kartal Baykan","doi":"10.25179/tjem.2021-83643","DOIUrl":"https://doi.org/10.25179/tjem.2021-83643","url":null,"abstract":"Anti-insulin antibody syndrome is a rare autoimmune disorder that is characterized by severe hypoglycemia, elevated blood insulin level, insulin autoantibodies, and the absence of pancreas islet cell pathology or exogenous insulin exposure. Alpha-lipoic acid is a sulfhydrylcontaining compound that is used to treat diabetic peripheral neuropathy and was first reported as a cause of anti-insulin antibody syndrome in Japan in 2003. In the literature, 17 cases with anti-insulin antibody syndrome following alpha-lipoic acid use have been reported from Japan, along with one case from Turkey, two from Korea, and seven cases from Italy. In this article, we present three cases with anti-insulin antibody syndrome developed after alpha-lipoic acid use. Case 1: A 72-year-old male patient was observed to have lost consciousness at home. This patient had no history of diabetes mellitus, diabetic medication, or insulin use. The patient complained of tiredness, palpitations, and sweating that began 3-4 h after eating but regressed after eating during the past month. The patient had a history of diagnosis with lumbar disk hernia two months ago and was using alpha-lipoic acid (600 mg). Case 2: A 67-year-old diabetic female patient using alpha-lipoic acid (600 mg) following the diagnosis of diabetic peripheral neuropathy. She complained of shivering and sweating two to three hours after a meal that started after starting alpha-lipoic acid therapy. Case 3: A 46-year-old male patient complained of fatigue, palpitations, and sweating for the last three weeks. Seven weeks ago, he started using alpha-lipoic acid (600 mg) after being diagnosed with carpal tunnel syndrome. The patients were diagnosed with anti-insulin antibody syndrome secondary to alpha-lipoic acid use, following which their alpha-lipoic therapy was discontinued. Alpha-lipoic acid is widely used in the treatment of diabetic peripheral neuropathy, nutritional support, or as an anti-aging agent. Material and Methods: A total of 592 patients with Type 2 DM aged ≥60 years and 269 diabetic patients aged <60 years as middle-aged with initial admissions or patients with two months of antihyperlipidemic medicine pause were recruited from the internal medicine outpatient clinic during December 2012-March 2018. Demographic and biochemical data were collected retrospectively. Abnormal lipid levels were accepted as total-cholesterol >200 mg/dL, low density lipoprotein-cholesterol (LDL-C) ≥100 mg/dL, triglyceride (TG) ≥150 mg/dL, and high density lipoproteincholesterol (HDL-C) <50 in women <40 mg/dL in men. Results: In total, 506 (85.4%) elderly and 248 (92.1%) middle-aged patients had one or more types of dyslipidemia. There were statistically significant differences in the frequencies of isolated high TG (x2=7.54, p=0.01) and combined two parameter dyslipidemia such as LDL-C ≥100 mg/dL & HDL-C <40 mg/dL & TG <150 mg/dL (x2=4.30, p=0.03) for men between elderly and middle-aged patients. Combined two-parameter dyslipidemia such as LDL-C ≥100 mg/dL, TG ≥150 mg/dL & HDL-C >50 mg/dL (x2=6.64, p=0.01), and isolated low HDL-C (x2=2.91, p=0.05) for women were found to be different between the elderly and middle-aged patients. This study evaluated the determinants affecting the total treatment cost in hospitalized patients with diabetic foot ulcers (DFU), an important complication of diabetes. Material and Methods: Factors affecting cost in 300 cases of hospitalization due to DFU were analyzed retrospectively. Hospital invoices were examined, and incurred cost of each patient was calculated in Turkish lira and then converted into US dollars at the exchange rate on the date of hospitalization, considering the number of days of hospitalization. Results: Among the 300 included patients, the ratio of male/female participants was 1.94. The mean age of the patients was 62.6±11 years. Hemoglobin A1c mean was determined as 9.5±2.4%. The average hospitalization time was 17.8±12.9 days. When the average cost was analyzed according to Wagner phases, the average cost increased as the stage of the disease. The factor that most affected the cost was hospitalization cost (p=0.001, r=3.24). Diagnostic imaging tests affected the total cost the most (p=0.001, r=2.52) after hospitalization. Conclusion: The cost of hospitalization was the most important factor affecting the total cost in the analysis of DFU. Therefore, taking precautions before the advancement of ulcer stage and treatment at an early stage is expected to reduce the treatment cost.
