The technological breakthrough was done as a part of and became familiar through cryptocurrencies, where it is used to openly store currency transactions among its users. Blockchain technology has been since proposed and used in various domains ranging from open contracts to electronic voting—as well as in various purposes in eHealth, medical and well-being applications. However, its usage in these sectors possesses several ethical questions as these environments are full of personal and private patient information. To study the state-of-the-art of the blockchain ethics in healthcare, this study presents a systematic literature study (SLR) on this phenomenon. By collecting the relevant primary studies from Scopus, the results show that the utilisation of blockchain is swiftly maturing with new research and applications published constantly in this domain. However, the ethical discussion related to the use of blockchain technologies is still taking its baby steps in healthcare. Despite a few openings, ethical research is practically non-existing when compared against the full extant literature on the topic. {"title":"Value for all? Dynamics of partnership formation in occupational healthcare collaboration","authors":"A. Kork, Inka Koskela, M. However, there is little processual understanding of how cooperative partnerships ‘work’ in healthcare collaboration. From the value creation perspective, their mobilization, management and maintenance are challenging. To understand the value of cooperative partnerships, we explore the dynamics of partnership formation in occupational healthcare collaboration.\u0000The empirical data is based on a two-year qualitative case study examining e-value co-creation in healthcare. The research data was obtained through a participatory action research method. We facilitated and followed up a developmental process of the partnership between an occupational health service company and its customer organization. This partnership aimed to add strategic value through the co-creation method to improve the well-being of employees and to promote eHealth solutions.\u0000In analyzing the data, we adopted a process orientation that allowed us to explore dynamics in partnership formation and its e-value co-creation. We used Ring and Van de Ven’s [1] framework to examine how cooperative interorganizational relationship develops through the stages of negotiation, commitment and execution. Our longitudinal case study analysis reveals how interaction, mutual sensemaking and institutional logics affect partnership and its value creation.\u0000The results show that the formation of a cooperative partnership is a challenging inter-organizational learning process. Our study demonstrates three tensions characterizing the dynamics of partnership: asymmetrical roles and positions between partners (customer and service provider) in co-creation, exploitation of institutionalized practices versus the exploration of new methods for collaboration, and tradeoffs between the operational logic and the co-creation logic.\u0000To create value for all in cooperative partnership, we emphasize the necessity of dialogue, mutual trust, interorganizational learning and processual feedback of accomplishments. {"title":"Rural youth attitudes towards immigrants","authors":"Saaga Somerkoski","doi":"10.23996/fjhw.111179","DOIUrl":"https://doi.org/10.23996/fjhw.111179","url":null,"abstract":"The aim of this study is to research the attitudes towards immigrants among rural Finnish second grade students. The study was carried out in Southern Ostrobothnia region which is strongly countryside and where the number of immigrants is the lowest in Finland. Also, the level of education in the region is the lowest in Finland. During the next decades, immigrants will be an important source of labour, since the population in Finland is getting older - faster than in any other European country. The data (n=275) was collected from colleges and upper secondary schools. The analysis showed that gender, parents level of education and the multicultural connections had an effect on persons attitudes towards immigrants. Youths´ attitudes also varied towards different immigration groups and nationalities. Attitudes towards the immigration of children and labour for example, were more positive than attitudes towards unemployed migrants and refugees. In terms of nationalities, Western immigrants were more positively welcomed than immigrants from war zones and those from the crisis areas. Answers to the open-ended question in the end of the questionnaire showed that the attitudes towards immigration were particularly concerned with refugees and the policy of immigration. Open answers included both racist and negative statements as well as statements to helping refugees. The results of this study may be of use when the new refugee policies are implemented in near future. The increase in immigrant labour requires respect, cooperation and cultural tolerance. An educational intervention, Basics of Digital Pedagogy was developed by the TerOpe project’s experts. The participating educators and educator candidates of the educational intervention were recruited from the universities and university of applied sciences, which were involved in TerOpe project. All the participants of the educational interventions were invited to take part in this study. The educational intervention was conducted during spring 2019. Pre- and post-tests were implemented digitally by using an Educators’ and Educator Candidates’ Competence in Digital Pedagogy self-assessment instrument (OODI), which was developed for this study. The OODI includes 32 items divided in six digital competence areas professional engagement, digital resources, teaching and learning, assessment, empowering learners and facilitating learners’ digital competence. The data was analysed statistically. The self-assessed level of overall competence in digital pedagogy and competence in all competence areas of digital pedagogy increased statistically significantly during the intervention. The educational intervention used in this study seems to increase educators’ self-assessed competence in digital pedagogy. Asikainen","doi":"10.23996/fjhw.107884","DOIUrl":"https://doi.org/10.23996/fjhw.107884","url":null,"abstract":"Medical secretaries may have several separate electronic nursing information systems in use, but regardless of the systems, their task is to make sure that the patient information is correct and usable.\u0000The purpose of this study is to describe the support provided by the hospital information systems for the work of medical secretaries in patient administration tasks in different phases of the care process. The data were collected in a central hospital where medical secretaries had long been using partly electronic information systems. The data were collected using an abridged version of the Hospital Information System Monitor (HIS-monitor).\u0000The majority of the secretaries (N=60) gave a positive assessment for the support provided by the information system for their work at patient admission, when ordering diagnostic or therapeutic examinations or procedures, and at patient discharge. In the planning and organization of care, most thought that the systems provided poor support for informing all those involved in patient care. At patient admission, nearly half considered that the support for ensuring data protection (46%) and the systems’ compliance with legal obligations (44%) was poor. In connection with ordering diagnostic and therapeutic examinations and procedures, nearly half (43%) thought that information on the availability in ancillary units was not readily and easily available. At patient discharge, 40% considered that the systems did not support the identification of missing or incorrect information.\u0000The hospital information system provides partial support for medical secretaries’ work. Paivystyksellisella hoidon tarpeen arvioinnilla tarkoitetaan ammattilaisen suorittamaa arviota potilaan tarvitsemasta paivystyshoidosta. E-palveluiden yleistyessa ihmiset arvioivat yha useammin itse omaa hoidon tarvettaan sahkoisilla tyokaluilla ennen paivystykseen saapumista ja ilman kontaktia terveydenhuollon ammattilaiseen. E-palveluiden kayttoonotto on haasteellista ilman ymmarrysta teknologian, ihmisten ja toimintaympariston valisista suhteista. Taman tutkimuksen tarkoituksena on kuvata johtajien kasityksia 1) paivystyksen hoidon tarpeen arviointitoiminnasta, 2) e-palveluiden yleistymisen vaikutuksista hoidon tarpeen arviointitoimintaan, 3) e-palveluiden yleistymisesta aiheutuvaan toiminnan muutokseen varautumisesta ja 4) toiminnan muutoksen onnistumiseen vaikuttavista tekijoista. \u0000Tutkimusaineisto kerattiin syksylla 2019 teemahaastattelulla viiden yliopistosairaanhoitopiirin yhteispaivystyksen operatiivisesta johtamisesta vastaavilta hoitotyon johtajilta ja laakarijohtajilta (n=10). Aineisto analysoitiin sisallonanalyysilla. \u0000Tulosten mukaan e-palveluiden yleistymisen vaikutukset kohdistuvat yhteispaivystyksessa ihmisiin ja toimintaan. Ammattilaisten tyo, asiakkaiden toimintamallit seka hoidon tarpeen arviointitoiminta muuttuvat ja paivystystoiminta kehittyy. Johtajat ymmartavat e-palveluiden yleistymisen vaikutukset sosioteknisen lahestymistavan mukaisena, koko yhteispaivystysta koskevana toiminnan muutoksena. E-palveluiden yleistymisesta aiheutuvaan toiminnan muutokseen on varauduttu, mutta siina on myos haasteita. Varautumista pyritaan edistamaan ja siihen tarvitaan tukea. Yhteispaivystyksessa toiminnan muutoksen onnistumiseen vaikuttavat esimerkiksi asenne, hyotyjen osoittaminen, toimintaymparisto ja resurssit. \u0000Edellytykset e-palveluiden kayttoonoton onnistumiselle yhteispaivystyksen hoidon tarpeen arviointitoiminnassa ovat optimistiset. Toiminnan muutokseen on tarkeaa varautua ja huomioida muutoksen onnistumista edesauttavat ja estavat tekijat. Ilman riittavaa taloudellista varautumista toiminnan muutoksen onnistuminen voi olla vaikeaa. Occupational health care has also encountered the need for digital health care services due to the expansion of mobile work, as the companies and work are more digitalized and internationalized. This research, conducted in three different occupational health care organizations in 2016-2018, aims to explore the features affecting the development of preventive digital services in occupational health care. The research comprised of exploring the current situation of preventive services in occupational health care and the co-creation, piloting, and evaluation of preventive digital services. The research utilized the realistic evaluation method. The results describe the functional environments of occupational health services and the facilitating and hindering factors found in the environments during the piloting process.
