The diagnostic work-up should be completed if the condition does not resolve after five years of age or in older children who experience a recurrence of nocturnal enuresis. This case report presents a 12-year-old boy with nocturnal enuresis secondary to hyperthyroidism. In many cases, nocturnal enuresis is linked to emotional disorders (like school or family problems), but increasingly the condition is rather a consequence than the reason. Currently, nocturnal polyuria, inadequate capacity of the urinary bladder and functional urinary tract disorders are considered to be the most common causes of nocturnal enuresis. Obesity and sedentary lifestyle affect the morphology of osteochondral structures and the growth plate, leading to serious orthopaedic consequences. The stigma of obese children also increases the rates of anxiety disorders, depression and learning disabilities in the paediatric population. The COVID-19 pandemic has enforced the introduction of isolation rules, changing the living conditions of society. Working and studying remotely and keeping social contacts to a minimum resulted in a lower intensity of physical activity and altered eating habits, which significantly contributed to further weight gain in overweight and obese children. In the case of severe acute respiratory syndrome caused by coronavirus infection, organic changes associated with obesity may increase the risk of thromboembolism, reduce the glomerular filtration rate of the kidneys and increase the inflammatory response of the body, which may consequently lead to invasive mechanical ventilation. Previous studies on obesity in children have shown that a high inflammatory index increases the risk of long-term complications in severe acute respiratory syndrome coronavirus 2 infection and reduces the chances of a quick recovery. It is necessary to make the society aware of the extent of health complications of childhood obesity, introduce social education on the principles of nutrition, and facilitate access to holistic treatment for obesity. A synchronous head and neck lesion brings the attending physician into a quandary, as there is no gold standard for managing such patients. Failure to respond to treatment should provoke physicians into considering other possible diagnoses, as often a secondary lesion may be the hidden culprit. Case report: Herein, we present a case of a synchronous cervical mass: Kimura disease and nasopharyngeal carcinoma in a 48-year-old woman. She was initially treated for Kimura disease based on fine-needle aspiration cytology evidence, and later found out to have a synchronous malignancy in the fossa of Rosenmüller. Failure of Kimura disease to respond to steroid treatment led to a repeated examination, which resulted in the diagnosis of nasopharyngeal carcinoma. The patient was treated with concurrent chemoradiotherapy. Conclusion: Kimura disease, a rare benign head and neck tumour, is treated successfully with steroids. This condition is defined as a disorder of tissue oxygenation during labour and is responsible for approximately 23% of neonatal mortality. Perinatal hypoxia can cause injury and failure of many organs, including the brain, heart and kidneys, mainly in the mechanism of the ischaemic-reperfusion injury. Currently, biochemical markers are more frequently incorporated in neonatal diagnostics. Routinely used in adult patients, these organ-specific chemicals enable diagnosis with high sensitivity and specificity. The manuscript presents a review of research on the use of cardiac markers (N-terminal fragment of pro-B-type natriuretic peptide, cardiac troponins I and T, and cardiac creatine kinase isoenzyme) in selected neonatal diseases. Still, no cutoff values have been established for any of the markers described in this paper. Aim of the paper: Assessment of the course of myocarditis in children. Materials and methods: Retrospective study (2009–2019) included 81 patients with myocarditis with analysis of the clinical course, biomarker levels, electrocardiogram, echocardiography and magnetic resonance imaging. Results: The most frequently reported symptom of myocarditis was chest pain in 73 (90%) patients. In 66 (81.5%) patients, the serum troponin I concentration was above the norm (the sensitivity 81.5%), the mean 12,539 ± 11,394 ng/L. In 24 out of 25, the concentration of N-terminal pro-brain natriuretic peptide was above the limit (mean of 866 ± 976 pg/mL). 71 patients had an abnormal electrocardiogram (sensitivity 88%). In echocardiography, 23 (28%) patients had left ventricular ejection fraction below normal limit, the mean 64.5 ± 7.1%. In the subgroup of patients treated with an angiotensin converting enzyme inhibitor, an improvement in left ventricular ejection fraction and end-diastolic dimension was observed. In the subgroup of overweight and obese subjects, 93.3% of patients showed abnormalities in control cardiac magnetic resonance after 6 months, including 33.3% with signs of active inflammation. 60% patients in the subgroup with a body mass index <20 kg/m2 had normal cardiac magnetic resonance imaging. Conclusion: Myocarditis course in adolescents is usually mild. In the overweight and obese patients the occurrence of the disease is more frequent and myocardial abnormalities last longer. {"title":"Somatisation as a manifestation of emotional disturbance in children","authors":"Zdzisław Michał Zawilski","doi":"10.15557/pimr.2022.0006","DOIUrl":"https://doi.org/10.15557/pimr.2022.0006","url":null,"abstract":"Somatisation is a phenomenon consisting in signalling complaints originating “from the body” (pain, discomfort, disturbances of physiological functions), whose cause is not explained on the basis of physical examination and additional examinations, and whose basis is usually an unconscious emotional factor. The most common somatic manifestations of negative emotions include abdominal pain, headache, palpitations or stabbing sensation in the chest, dizziness, shortness of breath, cough, squeezing sensation in the throat, numbness of limbs, as well as diarrhoea, vomiting or fainting. Although the described type of disorder occurs particularly often in people burdened with other mental problems, such as anxiety, depression or personality disorders, nowadays it is observed more and more often in children not showing any psychogenic abnormalities so far. A significant influence on the development of such symptoms has the environment of upbringing, from which the developing person draws behavioural patterns. In addition, the entire general social context with all its conditions, both positive and negative, is important. In the case of children, due to the lack of possibility of regular and constructive self-observation, developed abstract thinking and conscious, consistent verbalisation of inner experiences by the patient, the process of diagnosis and therapy of such disorders often requires diagnostic hospitalisation, multi-specialist consultations, long-term treatment in the outpatient mode, as well as at least partial modification of environmental relations, including the educational process. It is the most common cause of photosensitivity in children between 6 and 14 years old. It is characterised by pruritic or tingling erythematous papular or plaque lesions of sunlight-exposed skin which occur in spring or early summer. The first signs of polymorphous light eruption typically appear several hours to days after the first exposure to intense sunlight and take the form of an irritable rash persisting for a few days, and subsiding within a week or two with minimal or no scarring. Its recurrences may have a detrimental impact on the quality of life. Due to the significant heterogeneity of clinical manifestations of polymorphous light eruption, the disorder is relatively challenging to diagnose. The differential diagnoses include photosensitivity associated with lupus erythematosus, solar urticaria, actinic prurigo, erythema multiforme, photocontact allergic dermatitis, and genetic dermatoses. UVA and UVB phototesting followed by histological examination is an important diagnostic tool to objectify light-related symptoms. Once the diagnosis is established, the patient should be monitored and educated on prevention. The management of polymorphous light eruption is based on prevention such as sun protection and sunscreens. At the time of the pandemic, testing for SARS-CoV-2 is important in all paediatric patients who have fever and acute abdominal pain with diarrhoea or vomiting, to differentiate acute COVID-19 or possible multisystem inflammatory syndrome in children (MIS-C) from acute appendicitis, and thus avoiding unnecessary surgery. On the other hand, early accurate diagnosis and consequent appropriate surgical and antibiotic treatment of appendicitis are important because they can prevent complications such as abscess formation and other postoperative complications. Delayed diagnosis can lead to significant morbidity that can outweigh the damage caused by COVID-19 alone. The fact that at the time of the COVID-19 pandemic radiological examination also has its limitations and can very often show a false-positive finding in children with MIS-C must also be taken into account. It arises between the muscle fibres and passes through the intermuscular septa. Due to the infiltrative nature of the tumour, recurrence rate is higher than in other subcutaneous lipomas. Complete surgical excision of lipoma is often regarded as a definitive treatment modality and longterm follow-up is mandatory. Here we report a case of a 49-year-old male presenting with huge left neck mass measuring 12 × 10 cm, which was successfully removed. Computed tomography scan of the neck showed a well-defined septated lesion with a fat component in the right sternocleidomastoid muscle, and the histopathology examination confirmed the diagnosis of well-circumscribed intramuscular sternocleidomastoid lipoma. The infection can affect all floors of the respiratory system. Respiratory syncytial virus is highly infectious and is the most common aetiological agent of bronchiolitis. It is estimated that 90% of children develop respiratory syncytial virus infection by the age of 2 years. In Poland and other countries of the northern hemisphere, these infections usually occur between October and May, with a peak in January–February. During the global COVID-19 pandemic, a significant change was observed in the epidemiology of respiratory syncytial virus infections in Europe and worldwide. The massive use of nonpharmacological interventions, such as closing nurseries, kindergartens, limited interpersonal contact, social distancing, strict hygiene rules and the use of protective masks, resulted in the absence or episodic occurrence of many seasonal infections, including those of respiratory syncytial virus aetiology, in 2020. In 2021, a so-called compensatory epidemic of syncytial virus aetiology was observed, i.e. a significant increase in the incidence associated with the extinction of collective immunity and an increasing proportion of susceptible individuals. The aim of the present study was to retrospectively analyse the incidence of respiratory syncytial virus infections in children hospitalised in the Department of Paediatrics, Paediatric Nephrology and Allergology of the Military Institute of Medicine in Warsaw in the years 2020–2021.
