Poor adherence to secondary prophylaxis increases the risk of recurrent acute rheumatic fever. Studies on adherence to secondary prophylaxis in Ethiopia are scarce. Thus, this study aimed to determine adherence to secondary prophylaxis and risk factors among patients with rheumatic heart disease. Methods: Facility-based cross-sectional study design was employed to conduct this study from December 25/2021 to January 22/2022. The study was performed among rheumatic heart disease patients in randomly selected four public hospitals in Addis Ababa. Data were collected through face-to-face interviews using a structured questionnaire from follow-up patients after using a pre-tested questionnaire. Data were entered into EPI info version 7.2 and exported to SPSS version 26 for analysis. Multivariable logistic regression analysis was performed using variables with a p-value of less than 0.25 in bi-variable logistic regression analysis. Adjusted Odds Ratio (AOR) with a 95% confidence interval was estimated to determine the strength of association with poor adherence. Statistical significance multivariable logistic regression analysis was declared when the p-value is < 0.05. Results: Three hundred and eighty-one study participants completed the study with a response rate of 95%. The mean age of the respondents was 26.45 ± 10.5 years. The overall poor level of adherence was 29.1% (95% CI: 24.7– 33.8%). Rural residency (AOR = 2.637, 95% CI: 1.068– 6.513), living with > family members (AOR = 2.879, 1.282– 6.465), living more than 30 km from health clinic (AOR = 3.247, 95% CI: 1.051– 10.033), lack of penicillin V (AOR = 6.772, 95% CI: 3.234– 14.177) and fear of catching Covid-19 (AOR = 0.04, 95% CI: 0.014– 0.114) were independently associated with risk factors for poor adherence. Conclusion: Considerable proportion of patients with rheumatic heart disease in Addis Ababa had a poor level of adherence to secondary prophylaxis. The aim of this study is to investigate baseline characteristics, risk factors, and etiology of HF among patients admitted to a tertiary hospital in Somalia. Methods: Between May and October 2021, data on epidemiological characteristics, risk factors, and etiology of HF among patients admitted to an emergency unit of a teaching hospital in Mogadishu, Somalia were retrospectively collected and analyzed. Results: A total of 155 patients were disgnosed with HF, their mean age was 65.9 ± 14 years, 46.5% (n = 72) were aged 65 years or older, and 58.7% (n=91) were female. The prevalence of HF was 3.4% (n = 155); 47.7% (n = 74) had HF with preserved ejection fraction (HFpEF), 34.9% HF with reduced ejection fraction (HFrEF), and 17.4% HF with mildly reduced ejection fraction (HFmrEF). More females than males had HFrEF and HFmrEF (77.8% vs 59.3%, p < 0.01). In contrast, HFpEF was more frequent in males (55.4% vs 44.6%, p < 0.01). Regarding cardiovascular disease risk factors, patients with HFpEF had advanced age (≥65), were male, and had co-morbid hypertensive heart disease (HHD) and thyroid disease. On the other hand, patients with HFrEF were more in older age (50–64 years), were women, and frequently had ischemic heart disease and diabetes mellitus compared to patients with HFpEF and HFmrEF. Among participants with HFpEF, HHD (58.1%) was the most common etiologies of HF, whereas in HFrEF, ischemic heart disease (38.9%) was prominent. Conclusion: HF in Somalia, different risk factors and etiologies were found in three groups of HF patients. Hypertension plays a predominant role both in its risk factors and the underlying cause of HF. So liver diseases affect the heart, and heart disease affects the liver. Hyperdynamic circulation, the inability of ventricular contractility, hypertrophy of the heart, and electrophysiological alteration of the heart (prolongation QT interval) are some complications of cirrhosis. The most known complication of cirrhosis in the heart is cirrhotic cardiomyopathy. The original definition of cirrhotic cardiomyopathy was stated at the World Congress of Gastroenterology (2015). It is characterized by cardiac alteration (both in structure and function) in patients with cirrhosis without previously known cardiac problems. The pathological mechanism of cirrhotic cardiomyopathy includes physiological and chemical changes in cardiac muscle cells and increased inhibition of cardiomyocytes. Beta-adrenergic receptors defect and desensitization, endogenous cannabinoids, and cardiomyocyte depressants (NO and inflammatory cytokines) decrease cardiac contractility. Forty to fifty percent of the patients with cirrhosis had QT interval prolongation. QT interval prolongation is the most known electrophysiological abnormality in cirrhotic cardiomyopathy. Salem","doi":"10.2147/rrcc.s336664","DOIUrl":"https://doi.org/10.2147/rrcc.s336664","url":null,"abstract":"Background: The association between Guillain-Barré syndrome (GBS) and Takotsubo cardiomyopathy (TC) has been appreciated for over two decades, while the physiological mechanisms between the two conditions are less so. In the time since, the progress in understanding molecular mechanisms and the accumulation of reported cases in the literature together have provided the beginnings for a deeper understanding of the disease pathways connecting these two conditions. Methods: Case report. Results: We report a 75-year-old woman with a history of prior TC who presented with symmetric bilateral paresthesias and weakness all preceded by symptoms concerning for an upper respiratory infection. The patient required intubation shortly after arrival due to respiratory failure, and routine electrocardiogram found evidence of ST-elevation myocardial infarction in multiple leads. Subsequent echocardiogram revealed findings consistent with TC, and electromyographic evaluation confirmed GBS. The patient clinically improved with plasmapheresis and had returned to her cardiac and neurologic baselines on outpatient follow-up. Conclusion: GBS contributes directly to the pathogenesis of TC, both through direct action on cardiac nerves and an increase in resting sympathetic tone. While the stress of intubation likely contributes to a sympathogenic state within GBS, it is unlikely the principal factor predicting the development of TC within this unique subset of patients. TC should be considered in any patient with an acute neuropathy whenever signs of progressive dysautonomia are present. paralysis from autoimmune nerve damage, and Takotsubo Cardiomyopathy (TC), a temporary expansion and paralysis of part of the heart, both are regarded as rare conditions. Their co-occurrence, while rarer still, is common enough for a connection to be considered between the two. Included is an example case of a 75-year-old woman in which they occurred together following a mild upper respiratory illness. The most common reasoning for this connection is emotional stress, which tends to happen when a patient needs mechanical support to breathe. While this explanation may satisfy on an individual basis, it fails to explain larger trends: a TC would be expected in patients who need support breathing, such as the critical care population. After understanding the overlap between these conditions, the autonomic nervous system and the effects of its’ dysfunction in this context must be considered. GBS is capable of directly disrupting the autonomic nervous system, with the resulting dysautonomia potentially severe enough to lead to the development of a TC. Elevated catecholamines, a common result of autonomic dysregulation, should be recognized not only as evidence of a potential underlying dysautonomia, but most importantly as a risk factor for the development of TC.
