{"title":"[Recognition of Medication and Adherence to Medical Prescriptions in Patients Using Generic Drugs].","authors":"Raúl Muñoz Martín, Mercedes Corcobado Márquez, Rosa María Gómez-Calcerrada Jiménez","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Bases: </strong>The prescription based on the active ingredients has fostered the rise of generic drugs, increasing the variety of presentations, which can cause confusion, mistakes and encourage the medication nonadherence. The elderly may be at higher risk due to associated comorbidities and polymedication.</p><p><strong>Objectives: </strong>To assess recognition of the medication and adherence to medical prescription in elderly patients with changes in their medication appearance.</p><p><strong>Methodology: </strong>Descriptive cross-sectional study of a sample made by recruitment in Primary Health Care. We performed structured interviews to 96 patients over 65 years: we evaluated the adherence to medical prescription through the Morisky-Green test and we assessed the ability to recognize the medication by a discrimination test using drug boxes.</p><p><strong>Results: </strong>99% of the patients (CI95% 94.3-99.8%) reported never to fail in taking their medication. 84.4% (CI95% 75.8-90%) recognized their medication without errors, 67.7% (CI95% 57.8-76.2% recognized the correct active ingredient and 56.2% (CI95% 46.3-65.7%) the right amount of active ingredient. A higher level of recognition was found in women, patients with a superior education level and those without polymedication (p < 0.05). Half of the patients (51%; CI95% 41.2-60.8%) showed adherence to medical prescription, but we found no significant relationship with the socio-demographic variables or the degree of recognition of the medication.</p><p><strong>Conclusions: </strong>High rate of lack of adherence to drug therapy, but similar to other publications. The education level influences the recognition of the medication, while polymedication has a negative effect. We intend to explore the qualitative evidence on the experiences of people with this treatment and their formal and informal caregivers.</p><p><strong>Method: </strong>Systematic literature review of original articles indexed from 2005 to 2015, which study the NIV in adult population with qualitative methodology in six databases. Peer review of scientific quality with tool of the CASPe Program. Qualitative metasynthesis of the findings.</p><p><strong>Results: </strong>15 articles met selection criteria, based on Grounded Theory and Phenomenology, which use interviews, observation and focus group, directed both patients, and families and health professionals. Its studies emphasize the feeling of anxiety and loss of control, although the NIV relieves them dyspnea (ambivalence). Search daily well-being through routines and active participation. Interaction dependence/autonomy, especially in decision-making, finding differences that make complex the process information and advance directives. Practical wisdom of nursing care 24 hours with patients id described basic, and difficulties being narrated: lack of time, overload of work, technocentrism, inexperience.</p><p><strong>Conclulsions: </strong>The qualitative evidence of treatment with VNI is varied although scarce; different plots (acute situations, chronic, terminals) are explored, from different points of view. In that fact the impact of illness and hospitalization drives the nursing staff to include the family in health care. Nurses think their relationship with families is not easy, which generates different attitudes towards them and present the possibility to allow its participation in care. The aim of this research was to analyze the attitudes of these professionals in the family involvement in patients care.</p><p><strong>Material and methods: </strong>A descriptive cross-sectional study was applied to the nursing staff in Pediatric Department of the University General Hospital Gregorio Maranon (HGUGM), in Madrid, Spain. The data was collected using the \"Families' Importance in nursing care: Nurse's attitude scale\".</p><p><strong>Results: </strong>Most of the professionals, mostly units of patients with chronic disease, take into consideration the family like a resource in nursing care, finding that a good relationship with families give them job satisfaction, without differences between categories. This is based on the fact that answers to the questions posed are as subjective as the individuals themselves. In that way, it is necessary quantitative and qualitative data for comprehensiveness (a more realistic and comprehensive approach to the object of the study).</p><p><strong>Objective: </strong>Reflect on this methodology through a practical example.</p><p><strong>Synthesis: </strong>We explain, through Participatory Education Program Focused on Colorectal Cancer Surgery Patient (mixed methods research project: multistrand, sequential and mixed in each of its three phases), what is this methodology and its advantages. The second phase of the study emerges from the results of the first one, and so on. Appropriate use of ICT can improve and ensure coordination between different health professionals and levels of care. The aim of ICT is to facilitate the continuum of care and foster care efficiency and quality. However, professionals should make appropriate use of ICT based primarily on non-maleficence and confidentiality. The ICT:\u00001. Improve care continuity.\u00002. Facilitate integration and coordination, in order to provide personalized and excellent care between different levels of care.\u00003. Reduce health costs (in patients of high complexity, for example).\u0000The ICT facilitate the non-contact care, spacing the time between visits. The disadvantage of ICT are related to confidentiality and depersonalization of patient care. Nurses, by using ICT, play a leading role, given its role in health promotion and disease prevention. In these contexts, ICT can help innovate and improve the quality of care and health outcomes. In this regard, nurses have a great opportunity to innovate in care and take leadership in remote monitoring of patients. ICT have created new professionals, patients and providers challenges that should be studied in order to minimize problems and seek solutions. It interferes with normal development of children. Its impact transcends childhood to appear in adolescence and adulthood. Diagnosis would be developed if the child shows the main symptoms of inattention and/or hyperactivity-impulsivity, with consequences in different aspects of daily life, such as academic achievements, family life or social interactions with other children. These symptoms will come up along life in different ways. When the child is young, hyperactivity and impulsivity prevail, but as he/she grows, the dispersion and difficulty to remember things and facts will become more evident. The aim of the study was to assess with the Vanderbilt scale, based on DSM-IV diagnostic criteria, the differences of the impact of ADHD among parents, in comparison to the medical diagnosis. A retrospective study was carried out by using this questionnaire, which completed by both parents individually. A total of 78 children took part in this study, all of them diagnosed with ages ranging from 6 to 16 years old, who attended to Neuropedriatics at VAlle del Nalón Hospital(Asturias), between October 2011 and March 2013. Regarding attention deficit, the average responses given by mothers (7.38 ± 1.81) was higher than that offered by fathers (647 ± 2/46). The same is true for hyperactivity/impulsivity, with average scores of mothers (5.80 ± 2.90) which were one positive response higher than those given by male patients (4.74 ± 3.04).
