Objective scientific knowledge for many authors more valuable than true subjective belief is determined by research on primary data but a renewed analysis of already recorded or published data is common too. Ever since, an appropriate experimental or study design is an important and often a seriously underappreciated aspect of the informativeness and the scientific value of any (medical) study. The significance of study design for the reliability of the conclusions drawn and the ability to generalize the results from the sample investigated for the whole population cannot be underestimated. In contrast to an inappropriate statistical evaluation of a medical study, it is difficult to correct errors in study design after the study has been completed. Various mathematical aspects of study design are discussed in this article. \u0000Methods. In assessing the significance of a fair study design of a medical study, important measures of publication bias are introduced. Methods of data or publication bias analysis in different types of studies are illustrated through examples with fictive data. Formal mathematical requirements of a fair study design which can and should be fulfilled carefully with regard to the planning or evaluation of medical research are developed. \u0000Results. Various especially mathematical aspects of a fair study design are discussed in this article in detail. Depending on the particular question being asked, mathematical methods are developed which allow us to recognize data which are self-contradictory and to exclude these data from systematic literature reviews and meta-analyses. As a result, different individual studies can be summed up and evaluated with a higher degree of certainty. \u0000Conclusions. This article is intended to give the reader guidance in evaluating the design of studies in medical research even ex post which should enable the reader to categorize medical studies better and to assess their scientific quality more accurately.

The incidence of atrial fibrillation is continuously rising. Meanwhile several risk factors for AF development have been identified but the etiology is not cleared. \u0000Methods: A systematic review and re-analysis of studies which investigated the relationship between AF and some risk factors was conducted. The method of the conditio sine qua non relationship, the method of the conditio per quam relationship, the method of the exclusion relationship and the mathematical formula of the causal relationship k were used to proof the hypothesis. Significance was indicated by a p-value of less than 0.05. \u0000Results: The studies analyzed were able to provide direct and indirect evidence that AF is caused by a process of inflammation while a direct identification of the factor causing AF was not possible. A total of twenty five (25) adult male wistar rats were used for the study. The rats were divided into five groups of five rats each. Group I (normal control) was administered with 2ml/kg distilled water p.o. Group II was administered with 200mg/kg aqueous leaf extract of T. daniellii p.o. Group III was administered with 200mg/kg methanolic leaf extract of T. daniellii p.o. Group IV and V were administered with 200mg/kg aqueous and methanolic leaf extracts of A. cordifolia respectively. Administration of extracts lasted for 14 days after which animals were sacrificed and serum developed from blood samples was collected and used for analysis to evaluate the lipid profiles of experimental models using standard methods. Results obtained from the study show that the highest level of Low Density Lippoprotein (LDL) (31.21±0.04 mg/dL) was recorded on Group II and was not significantly different from the control group (33.20±0.10 mg/dL). However, for High Density Lipoprotein (HDL), the highest level was recorded on Group IV (48.14±0.08mg/dL). Similarly, this was not significantly different from the Control group (51.80±0.10mg/dL). Group II presented the highest level of Triacylglyceride (TG) (48.80±0.02mg/dL), which however, was considered not significantly different from the control group (51.17±0.08mg/dL). For Total Cholesterol (TC), highest level (64.64±0.49mg/dL) was recorded on Group II and value was not significantly different from that recorded on the normal control (63.83±0.10mg/dL). In conclusion, it can be deduced from this study that extracts of T. daniellii and A. cordifolia lacks the potential to alter the lipid profile of patients depending on them for one medicinal use or the other and may thus be considered potential candidates for drug development.

Investigations were conducted using standard methods. Oral administration of 200mg/kg aqueous leaf extracts of T.daniellii caused a non-significant increase in the activity of ALT (5.43±0.60IU/L), AST (16.93±0.26 IU/L) and ALP (160.70±1.04 IU/L) compared to the values recorded on the normal control (group I) ALT (3.84±0.16 IU/L), AST (14.19±0.52 IU/L) and ALP (157.26±0.64 IU/L). Group III administered with 200mg/kg methanolic leaf extract of T. daniellii manifested a significant elevation in the activity of ALT (13.15±0.89 IU/L), AST (22.84±0.38 IU/L) and ALP (170.40±0.44 IU/L) compared to the normal control. Similarly, groups IV and V which were orally administered with 200mg/kg aqueous and methanolic leaf extracts of A. cordifolia showed significant increase in the activity of ALT (6.32±0.33U/L), AST (17.70±0.030U/L) and ALP (161.13±0.09U/L) and ALT (7.55±0.59U/L), AST (19.35±0.26U/L) and ALP (165.38±0.35U/L) respectively compared to the values recorded on the control (group I). In conclusion, drug development protocols involving T. daniellii leaf should preferably use water as an ideal solvent. On the other hand, the hepatotocity associated with both aqueous and methanolic extracts of A. cordifolia could imply the presence of hepatotoxins in the leaf of the said plant.

Reports even demonstrated the presence of Epstein-Barr virus in gastric carcinoma (GC). However, the pathogenic role of EBV in GC is uncertain. The present investigation was carried out to investigate a possible causal relationship between GC and EBV. \u0000Statistical Analysis: The method of the conditio sine qua non relationship was used to proof the hypothesis whether gastric cancer is a necessary condition (a conditio sine qua non) of the presence of EBV in human gastric tissues. In other words without GC no EBV in human stomach. The mathematical formula of the causal relationship k was used to proof the hypothesis, whether there is a cause effect relationship between gastric cancer and EBV. Significance was indicated by a p-value (two sided) of less than 0.05. \u0000Results: In toto 26 ISH based studies with a sample size of N = 11860 were re-analyzed. All the studies analyzed support the null-hypothesis without GC no EBV positivity in human stomach. Numerous factors, including smooth muscle cell proliferation, vasospasm, vascular fibrosis and occlusion, direct vascular injury and inflammation, impaired repair of vasculature, are involved in the pathogenesis of PAH. It has been suggested that galectin-3 as a biomarker of excessive fibrosis and inflammation can be useful predictor of both severity and prognosis in patient with PAH. The short communication is reported that elevated Gal-3 levels were found in majority patients with PAH depending on clinical status and of the disease. Although elevated Gal-3 levels were associated with a higher risk of all-cause mortality, cardiovascular mortality, and right ventricle heart failure, the value of this biomarker in PAH patients at high risk stratification is uncertain and requires to be investigated in large clinical trials.
