ABSTRACT Cross-Sectional Study: Academic Performance Associated with Depression and Anxiety in Students of the Faculty of Medical Sciences, 2015 BACKGROUND: Depression and anxiety are emotional disorders that could be related to academic performance, mainly in students of health sciences areas, who are vulnerable to suffer these kind of disorders. The aim of the study was to determine the relationship between the presence of anxiety and depression, with low academic performance in students of the Faculty of Medical Sciences of University of Cuenca. METHODS: An observational cross-sectional study was performed; the universe included students belonging to the Faculty of Medical Sciences of the University of Cuenca. A virtual survey was used to collect the information, and the \"Goldberg Anxiety and Depression Scale\" was applied in its Spanish version, the presence of low academic performance was also analyzed. Descriptive statistics was used; the data processing was done in the SPSS software version 22.0. RESULTS: 1 127 students were included, 73.8% (n=832) female and 26.2% (n=295) male; the mean age was 21.68 years (± 2.8). The percentage of students in the Schools of medicine, nursing and medical technology were 33.9% (n=382), 17.7% (n=200) and 48.4% (n=545) respectively. The frequency of depression in the schools of medicine, nursing and medical technology was 34.7%, 47.5% and 17.8% respectively; the frequency of anxious disorders was 35.2%, 50.8% and 14.1% respectively. No statistically significant association was found between these disorders and low academic performance. CONCLUSION: It has been reported a significant frequency of depressive, anxious or mixed symptoms in students of the Faculty of Medical Sciences of the University of Cuenca. Standard laparoscopic cholecystectomy requires the dissection of Calot’s triangle and exposure of the cystic duct; this procedure is associated with bile duct injury and bleeding from the liver vascular bed. Laparoscopic subtotal cholecystectomy could be an alternative in these situations, since it is considered as a procedure associated with few complications. METHODS: A prospective cohort study was conducted. The universe included 180 patients in whom a difficult cholecystectomy was performed; the same was defined as: empyema, gangrene, perforation, severe inflammation with fibrosis, Mirizzi syndrome, vesicular plastron, liver cirrhosis and anatomical variants. It was classified into two groups; Group 1 (traditional cholecystectomy, 90 patients), and Group 2 (rescue cholecystectomy, 90 patients), group 2 used rescue techniques such as: Pribram, subtotal reconstructive, fenestrative and retrograde dissection. The presence of complications (bile duct injury, bleeding, collections and residual choledocholithiasis, infection of the surgical site) was considered as an event. The softwares SPSS 22.0 and Epidat 3.1 were used for the statistical analysis. RESULTS: It was observed that the general incidence of complications in patients with difficult cholecystectomy was 9.44%, the incidence of complications in exposed patients (traditional cholecystectomy) was 14.44%, versus an incidence in unexposed patients (salvage cholecystectomy). 4.44%; RR 3.25 (95% CI: 1.02 - 9.58), p= 0.04. The following techniques were used: retrograde chole-cystectomy 63.33% (n=57), modified Pribram cholecystectomy 21.1% (n=19), reconstructive subtotal 14.44% (n=13), and fenestrative subtotal 0.9 % (n=1). The ischemia-reperfusion injury commonly presents in several critical care entities, including: ischemic stroke, acute coronary syndrome, sepsis and shock, major trauma, surgery and transplantation. Complications are multiple and dependent of the site of presentation; the common denominator is microvascular dysfunction that could potentially trigger multiple organ dysfunction syndrome.
