However, amlodipine poisoning, a widely used dihydropyridine calcium chanel blocker, is not fully documented in Ecuador. Treatment consists of classic measures for shock management and specific measures for this type of intoxication. CASE REPORTS: We present two case reports, both of teenage patients admitted into the pediatric intensive care unit for suicide attempt by taking amlodipine and some other drugs. EVOLUTION: During hospital stay, they presented a different evolutionary course. In both cases vasoactive drugs were needed, dosage was modified according to clinical course. Also in both patients, calcium gluconate was administered along with other support measures described in this paper. Finally, both patients presented a good outcome and were discharged after psychological and psychiatric assessment and follow up. CONCLUSION: The low frequency of amlodipine poisoning and the lack of evidence-based knowledge, constitute it as a diagnostic and therapeutic challenge. Based on our experience, we highlight the importance of early suspicion and prioritizing the use of vasopressors over fluid resuscitation. Additionally, we recommend documenting the exact dose of intake and inquiring about consumption of other drugs to properly classify the severity of the poisoning and stablish the treatment plan. Finally, constant clinical monitoring and support of laboratory tests will guide the conduct.","PeriodicalId":417745,"journal":{"name":"Revista Médica del Hospital José Carrasco Arteaga","volume":"37 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-07-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121584352","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

When an atypical case is presented, is necessary to use diagnostic scores, like Alvarado Score, Modified Alvarado Score, RIPASA, among others. The aim of this study was to determine the test validity of Inflammatory Response Score versus Alvarado Score, for acute appendicitis diagnosis in 16 year old and older patients. METHODS: This is a descriptive, cross-sectional, validity test study, to compare AIR Score and Alvarado Score, with the Gold-Standard histopathology results. A sample of 292 patients that went under appendectomy at Hospital José Carrasco Arteaga during 2018 was studied. We considered sensitivity, specificity, positive predictive value (PPV), negative predictive value (VPN), ROC curve and Likelihood Ratio. RESULTS: 49.7% patients were young adults, 62.7% of the patients were male. Alvarado Score showed a 88.5% sensitivity, 29.6% specificity, PPV of 81.7%, 42.2% NPV, 1.23 LR+ and 0.38 –LR; compared to 94.7% sensitivity, 76.5% specificity, 93.5% PPV, 80.3% NPV, 4.02 +LR, 0.611 -LR for Appendicitis Immflamatory Response Score. ROC curve for AIRS was 0.897, higher than Alvarado Score (0.611); being AIRS a more precise diagnostic test than Alvarado Score. CONCLUSIÓN: AIRS showed higher sensitivity (94.7%) and specificity (76.5%) than Alvarado Score (88.5%, 29.6% respectively). Alvarado Score is sensitive enough for acute appendicitis diagnosis, but not specific enough to exclude the diagnosis. AIRS shows a higher Likelihood Ratio and ROC curve than Alvarado Score, but the posttest probability is low for identifying acute appendicitis cases.","PeriodicalId":417745,"journal":{"name":"Revista Médica del Hospital José Carrasco Arteaga","volume":"118 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-02-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124698498","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Clínica Humanitaria, Fundación Pablo Jaramillo Crespo. BACKGROUND: Low birth weight (newborn with less than 2 500gr), is one of the most important causes of perinatal and infancy morbidity and mortality. Every year more than 20 million children are born with less than 2 500gr, more than 96% of these cases occur in developing countries, which demonstrates more likelihood of this situation occurring in poor socioeconomic conditions. Mortality in the first year of life is 14 times higher in newborns with low birth weight. The aim of this research is to study the association of low birth weight in full-term newborns with extra-fetal risk factors in the Humanitarian Clinic of the Pablo Jaramillo Crespo Foundation in Cuenca, between January and June 2015. METHODS: A case-control study, aimed to relate extra-fetal factors to low birth weight in full-term new-borns. We included a total of 453 patients, treated between January and June 2015. Socio-demographic variables of the mother, preventive factors and morbidity were evaluated. According to inclusion and exclusion criteria, 53 cases and 400 controls were obtained. RESULTS: There was a higher frequency of low neonatal weight in young women and adolescents, in those who come from rural areas, in those who had a partner, 14% of mothers with morbidity during pregnancy had low neonatal weights. There was no statistical significance between low neonatal weight and prenatal controls, substance use and maternal morbidity. CONCLUSION: There was a higher frequency of low neonatal weight in young women and adolescents, in those who come from rural areas, in those who had a partner. We didn’t find statistical association between any of the variables and low birth weight.","PeriodicalId":417745,"journal":{"name":"Revista Médica del Hospital José Carrasco Arteaga","volume":"31 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122855217","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
