Surgical removal is typically performed to improve cosmetic appearance and reduce the risk of malignant transformation. In this report, we present the case of a 26-year-old woman with a medium-sized CMN on her left breast. The nevus measured 14×8 cm, and the patient desired a single-stage excision. However, this approach would result in a large skin defect that would be challenging to reconstruct using a local flap or skin graft. Moreover, it could potentially compromise the maintenance of natural sagging and the contour of the breast. Consequently, we opted to place a thin thoracodorsal artery perforator free flap following the removal of the CMN. The patient was satisfied with the overall surgical results. Consequently, aesthetic reconstruction for large cutaneous defects of the cheek remains a challenge for plastic surgeons. We experienced a case of an 83-year-old woman with a large (7×10 cm) facial defect following the removal of squamous cell carcinoma. We elevated a cervicofacial skin flap containing the superficial layer of the superficial musculoaponeurotic system and a blood supply via the dermal plexus in a random pattern. Through this method, we achieved aesthetically favorable reconstruction of a large facial defect using a cervicofacial transposition skin flap. By avoiding extensive dissection, we achieved adequate coverage for even a quite sizable facial defect, with a short recovery time. Numerous treatment modalities, ranging from simple revision, Z-plasty, and W-plasty to laser treatments, have been used to treat scar deformities. However, for wide depressed scars, additional methods are needed to completely restore the contour of the depression caused by tissue adhesion. We report on the case of a 34-year-old woman with a wide depressed scar deformity on the left upper buttock and the encouraging results of an autologous fat graft injection technique that utilized a cutting wire to form a pocket for the fat graft site, while simultaneously resolving the adhesion caused by the tissue. However, with a long period of redness, patients complain about cosmetic discomfort, and the possibility of pigmentation changes is induced. We investigated the use of a long-pulsed 1064 nm Nd:YAG laser as a noninvasive treatment for improving the redness of these scars.Methods A retrospective chart review was conducted on 36 patients who underwent excision of a nevus on the face. Fourteen patients received laser treatment and another 22 patients used only scar management ointment. Patients were followed up 1 week after the sutures were removed. The photographic images taken at the time of suture removal and 2 months later were reviewed. The evaluation was performed on a 7-point scale by adding the Japan Scar Workshop (JSW) scar scale’s redness and erythema scores.Results The average initial JSW scar scale score of the treatment group was 4.6, and that of the nontreatment group was 4.2. When the re-evaluation was performed 2 months later, the score of the treatment group decreased to 2.2 and that of the nontreatment group decreased to 3.1. The patient had undergone augmentation rhinoplasty 5 years ago, after which hypertrophic scarring occurred in the left nostril. Several corticosteroid injections were administered as the first line of treatment, but with no symptom improvement. Therefore, we proceeded with surgical scar removal, with the use of a nasal conformer. However, scarring in the left nostril recurred. Accordingly, we proceeded with further surgical treatment using the scar folding technique. After scar folding, neither scarring nor nostril stenosis recurred during 1 year of postoperative follow-up. I show deviation of the nasal septum on the axial view of computed tomography and show that it cannot be straightened at this time. In the preoperative design of upper blepharoplasty in elderly patients, the extent of excision is not the same in both eyelids. Usually, laxity is more severer on one side than on the other side. Patients planning to undergo aesthetic procedures often want to make their faces symmetrical. A symmetrical face has often been regarded as a feature of true beauty, and many movie stars are hailed for their mirror-image looks. However, a perfectly symmetrical face is quite rare. Facial symmetry is observed in only 2% of the world’s population. For example, Amber Laura Heard (1986) (Fig. 1), an American actress who plays leading roles in horror films, is regarded as having a perfectly symmetrical face. Some degree of congenital or acquired asymmetry is normal and universal. Significant facial asymmetry, however, causes both aesthetic and functional problems [1]. Factors such as aging, trauma, and lifestyle choices, such as smoking or sun exposure, may contribute to asymmetry. Sometimes an asymmetrical face is the result of an individual’s genetics. There is a link between increasing age and facial asymmetry. Smoking exposes the face to toxins and can cause vascular problems. Excessive exposure to ultraviolet rays can also cause an asymmetrical face. Trauma, such as a broken nose, a deep cut, or being involved in a vehicle collision, can damage the face. Furthermore, using dentures or getting dental veneers may change the contours of the face. If a person has always had asymmetrical features, there is no cause for concern. However, new and noticeable facial asymmetry may be a sign of a serious underlying condition, such as stroke or Bell’s palsy. Facial drooping may be a sign of a stroke. Bell’s palsy is a condition involving paralysis of the facial nerves, usually causing one side of the face to droop [2]. When patients complain of facial asymmetry, the underlying cause should be investigated. Its causes are congenital disorders, acquired diseases, and traumatic and developmental deformities. Many cases of developmental facial asymmetry have indistinct causes. An appropriate assessment includes the patient’s history, a physical examination, and medical imaging [3]. When my patients want perfect symmetry, I usually quote a sentence from Murder on the Orient Express, written by Agatha Christie (1890–1976). Hercule Poirot says, “I was looking forward to seeing La Sainte Sophie. I love cathedrals. Mankind imposing order on the chaos, impossible symmetry. This reduces graft viability, resulting in problems such as reductions in size, changes in shape, and resorption of the cartilage graft in the late post-graft period. This study aimed to investigate the effects of adipose-derived mesenchymal cells and conditioned medium (CM) on cartilage graft viability.Methods Dissections were performed 4 months after the injection of 0.5 mL of CM or 2×106 mesenchymal stem cells (MSCs) in 0.5 mL after grafting into a control group and two experimental groups (n=21 rabbits in total). Chondrocyte viability and type II collagen expression in the grafted areas were analyzed by hematoxylin-eosin staining and immunohistochemical methods, respectively.Results In the MSC and CM groups, chondrocyte proliferation at the graft tissue incision margin (MSC: P<0.01, CM: P<0.0001), chondrocyte proliferation at the auricular cartilage incision margin (MSC: P<0.05, CM: P<0.0001), integration of the graft with the surrounding cartilage (MSC: P<0.001, CM: P<0.0001) and type II collagen expression levels (MSC: P=0.001, CM: P=0.0002) significantly increased.Conclusions Xenogenic injection of MSCs and CM contributed to new cartilage production without any tumoral effects or immune reactions. In particular, the cell-free nature of CM strengthened its potential for safe use. However, in patients who do not wish to undergo balancing procedures, it is difficult to select the appropriate implant size, making it challenging to achieve a symmetrical breast shape. Therefore, this study investigated the differences in breast asymmetry and other complications in patients who underwent a two-stage procedure or direct-to-implant (DTI) breast reconstruction to determine whether the two-stage procedure can produce more favorable outcomes.Methods The participants of this study were patients who underwent immediate two-stage breast reconstruction or DTI breast reconstruction from May 2018 to April 2022, did not receive postoperative radiotherapy, and did not wish to undergo any balancing procedures. An acellular dermal matrix was used for breast reconstruction in all patients, and a single reconstructive surgeon performed all the operations. Statistical significance was set at P<0.05.Results No significant differences in complications were found between the patients who underwent DTI breast reconstruction and those who underwent two-stage breast reconstruction. In the two-stage breast reconstruction group, breast volume asymmetry was observed in 18.4% (seven patients), which was significantly lower than the percentage of 44.7% (17 patients) observed in the DTI group.Conclusions Breast asymmetry was observed in a significant proportion of the patients in both groups. To improve outcomes and avoid incisional surgery, numerous noninvasive face-lifting techniques have been studied. This includes thread-lifting, a technique that promises to correct facial aging with limited scarring, rapid recovery, and minimal complications. As the population ages, an increasing number of ordinary people in South Korea are undergoing thread lifting procedures for the purpose of rejuvenation. The procedure involves insertion of a thread under the skin into the subcutaneous tissue, using a long needle as a guide. Dents or barbs prevent the thread from slipping and provide uniform aggregation of soft tissue to create a new volume contour when the thread is lifted. This procedure has gained worldwide popularity and is frequently performed. However, some minor complications have been reported. The cause of capsular formation remains unknown, but chronic inflammation is commonly considered to cause excessive fibrosis. Curcumin has anti-inflammatory effects and thus can relieve the symptoms of inflammatory diseases, as demonstrated in animal studies. This study aimed to evaluate the effects of curcumin on the fibrous envelope covering silicone implants in a rat model.Methods Two solid 1.8-cm oval-shaped silicone implants were placed beneath both sides of the back in 20 Sprague-Dawley rats. The control group included 10 rats that were fed a normal diet (group A), while the experimental group (group B) included the remaining 10 rats that were fed ground curcumin. En bloc excision was conducted at 8 postoperative weeks. Capsular thickness and inflammatory cell distribution were examined using a fixed tissue sample.Results Gross findings and histologic differences between the groups were observed. The experimental group had a significantly lower mean total capsular thickness than the control group (177.4±31.4 μm vs. 145.9±32.5 μm, P=0.007). A significant decreasing tendency was found in several inflammatory cells in the experimental group (7,070±744.3/mm2 vs. 2,640±301.7/mm2, P=0.001).Conclusions Curcumin significantly reduced the inflammatory reaction, and will help to lower the risk of capsular contracture.
