It became mandatory for GP practices in England to offer online consultations (also known as e-consultations) from April 2020 and video consultations from April 2021. This has led to an exponential increase in the workload for practices, with patients sending in forms for minor issues due to ease of use rather then self managing or seeing a pharmacist. This move to digital access coincided with the COVID-19 pandemic in early 2020. The Standard Operating Procedure at the time was for practices to switch a total triage model. This meant no patient could come to the practice without first being triaged. So direct booking of appointments were disabled and a patient would have to phone the practice or submit an online form, with a member of the team calling the patient (normally a clinician) to see if the issue could be dealt with remotely. This led to an increased use of video consultations and use of text messaging services, where a patient could send in pictures directly to the GP i.e. skin rashes. This was a significant change in practice that occurred very quickly and led to a dramatic reduction in face-to-face appointments but made access overall quicker. As of mid 2021, the standard operating procedure has ceased. But most practices have carried on using this process. Currently half of GP practice appointments are face-to-face and half are virtual in England. There has been increasing frustration in the public domain that patients cannot choose to have a face-to-face consultation for any reason at the time they want. There have also been concerns of missed diagnosis due to lack of face-to-face reviews during the pandemic. The counter-argument from the profession is that NHS treatment should be based on need and not choice and that the use of triage and digital solutions allows practices to prioritize care for those that need it the most.
