The diagnosis is based on clinical criteria, after ruling out other causes of hypersomnia. There are no lesions in brain imaging studies while EEG demonstrates slowing of bioelectrical activity in some cases. Kleine–Levin syndrome has a self-limiting course with symptoms becoming milder over time until they disappear completely. The pathogenesis includes hypothalamic pathology or autoimmune background (association with certain human leukocyte antigens). This article presents a young patient with a typical clinical picture of Kleine–Levin syndrome. Due to its rare occurrence, the knowledge about this disease entity is not common – the correct diagnosis was made a year after the onset of first symptoms.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67583597","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to investigate the effect of selected sociodemographic, clinical and lifestyle factors on the CDT performance in the Polish elderly population. Methods: CDT performance was assessed in 399 elderly subjects randomly selected out of all participants of a nationwide study on aging, PolSenior2, who met the inclusion criteria. CDT was scored using the Manos–Wu method. The short version of the Geriatric Depression Scale (15-item GDS) was used as a mood measuring tool. Results: Our study revealed a significant relationship between CDT performance and age. Multivariate regression analysis demonstrated also that interactions between reading newspapers and gender and between playing games and GDS score were independent predictors of an incorrect CDT. Of note, even though years of education were related to the CDT score (0–10) in the correlation analysis, education did not predict the global CDT score in the regression analysis. Conclusions: Advanced age was related to incorrect CDT performance. Engagement in some leisure activities may predict the CDT score, while global CDT performance, as assessed by the Manos–Wu method, is relatively unaffected by education.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"1 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67583693","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We also refer to the latest literature reports on the aetiology, pathogenesis, diagnosis and treatment of this clinical entity, including its potential relationship with lymphoproliferative and lymphohistiocytic processes. Furthermore, we present a spectrum of diseases that can clinically and radiologically mimic CLIPPERS (CLIPPERS-mimics), along with the proposed differential diagnosis. Finally, the paper presents the recommended treatment strategy for both acute disease and a maintenance therapy, with particular emphasis on the role of steroid therapy.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"59 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2020-12-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67583393","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and methods: Based on the BMI, we divided the patients into the following groups: underweight, normal weight, overweight, and obese. The severity of stroke was evaluated by the National Institute of Health Stroke Scale (NIHSS). STATA 15 software (StataCorp, 2017) was used for statistical analysis. Results: The results demonstrated that there was no association between the BMI and changes in patient condition during hospitalisation in the stroke unit. The BAI had no clear correlation with the short-term outcome. However, a comparison of accuracy revealed that the BAI was a more precise indicator, and could better predict NIHSS improvement over treatment than the BMI. Among the analysed indicators, only the WC correlated with the difference between the NIHSS scores on admission and at hospital discharge. Conclusions: The BMI, used in clinical practice for decades, is far from a precise predictor of functional outcome after ischaemic stroke. This is the first study that takes into account the obesity indicator BAI in patients after acute ischaemic stroke. According to our results, in the future we should focus more attention on abdominal adiposity indicators such as the BAI or WC.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"20 1","pages":"103-108"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67582936","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The present study reviews the available literature on the use and effectiveness of this therapy, both its classic form and more recent approaches (the so called “third wave”), among patients with brain injuries and those treated with surgical methods. We present a number of studies describing the use of cognitive-behavioural therapy in the treatment of patients with craniocerebral injuries, brain tumours, intracranial bleeding and vascular malformations. The obtained results are promising for the treatment of depression and anxiety disorders, post-traumatic stress disorder as well as sleep disorders, impulsivity and aggressive behaviour. The findings suggest that cognitive-behavioural therapy techniques may be effective in both these groups of patients and their families or caregivers. Currently, due to the small number of randomised controlled trials, high heterogeneity of study groups and a variety of research protocols, these findings do not allow for drawing final conclusions and proposing recommendations. Nevertheless, they demonstrate that the strategy offers promise when treating patients with brain damage. It is suggested that psychotherapy should be initiated after the end of the acute phase of the disease as it requires a certain level of self-awareness and an ability to test hypotheses, so it may be inadequate for patients with profound cognitive impairment. It requires the cooperation of a multidisciplinary team: doctors, physiotherapist, neuropsychologist and psychotherapist who, in addition to therapeutic skills, should have knowledge of the brain–behaviour relationship.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"20 1","pages":"109-118"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67582782","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Reading epilepsy, where all or almost all epileptic seizures are induced by reading, is a very rare form of reflex epilepsy. Two types of this disease have been distinguished: the most common type is caused by sudden muscle jaw contractions (myoclonic variant) and it lasts for a very short time (for example few seconds), while the second type is characterised by focal alexia seizures (focal variant with alexia). The article highlights the clinical picture and treatment of reading epilepsy in a 18-yearold patient. In this case, seizures were always caused by at least 2 hours of continuous reading and were preceded by jaw myoclonus. The treatment included sodium valproate and levetiracetam.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"20 1","pages":"136-140"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67583642","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It was then that research on the effects of marijuana on the course of different diseases began. Aim: The aim of this paper was to analyse the use of cannabinoids in the treatment of children with autism spectrum disorder. Results: There are different methods for cannabinoid administration. Researchers use various preparations with doses ranging between 0.04 mg to 900 mg. Preliminary studies are conducted on animal models, usually in genetically modified mice. Cannabinoids have been found to have a positive effect on social and emotional behaviours of children, as well as to show anxiolytic and antipsychotic effects. Cannabidiol plays an important role in the secretion of vasopressin and oxytocin. Some patients experience adverse effects. Antidepressant and anticonvulsant effects have been observed in patients with neurological and mental disorders. Conclusions: Cannabinoid therapy is not the standard of care in children with autism spectrum disorders, and the doses used vary significantly. There is evidence for the positive effects of cannabinoids in patients with comorbidities. It is possible that cannabinoids have an anxiolytic effect and allow for a reduced use of antipsychotics in the analysed group of patients. Antidepressant and anticonvulsant effects have also been observed. Furthermore, it was shown that CBDs administered in mice with Dravet syndrome improve social behaviour, dysfunction of which is one of the central symptoms of autism spectrum disorder.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"20 1","pages":"119-124"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67582949","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Materials and methods: The results of 3–4 language and cognitive assessments were available for four individuals (three women, one man) with lvPPA. The length of the observation period was 2–4 years. Language evaluation was comprehensive and addressed narrative speech, naming, word and sentence comprehension, repetition, reading and writing. Neuropsychological examination, whenever feasible, assessed visuospatial function, praxis, memory and executive functions. Results: Anomia was not an isolated symptom at the time of lvPPA diagnosis. Rapid deterioration of both spoken and written communication was observed. Reading aloud single words and high-frequency word comprehension were preserved longer than other linguistic competences. The narrative speech was progressively impoverished in terms of idea density, manifested by a reduced use of nouns and verbs in particular and an increased use of pronouns. At the advanced stage of the disease, the idea density was very low. There was marked deterioration of visuospatial functions, praxis and episodic memory in all patients. Progression to full-blown dementia was observed in all patients. Conclusions: The progression of linguistic and cognitive symptoms in lvPPA, albeit slightly heterogeneous, is relatively fast. The patients usually reach dementia stage within 1–2 years of the diagnosis and sometimes even earlier than 2 years of the declared symptom onset. When planning long-term management of lvPPA cases, rapid deterioration and progression to full-blown dementia within a relatively short time should be considered.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"20 1","pages":"125-135"},"PeriodicalIF":0.0,"publicationDate":"2020-11-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67583451","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Withdrawal symptoms usually occur 6–8 hours after the last alcohol consumption and peak between 24–72 hours. Typical manifestations include tremor, nausea, vomiting, insomnia, agitation, and excess sweating. Complicated withdrawal syndrome may manifest with seizures (including cluster seizures), status epilepticus, acute psychosis and delirium, when hallucinations are accompanied by disorientation in time, place and situation. About 10–15% of patients develop complicated withdrawal syndrome. The seizures in alcohol withdrawal syndrome are triggered in the brain stem and have a mechanism other than that of classic epileptic seizures. Benzodiazepines, which are more effective than other classes of anticonvulsants (they primarily reduce the frequency of convulsions in the first two days after alcohol discontinuation and significantly reduce the risk of death), are the gold standard for the treatment of alcohol withdrawal syndrome. The Clinical Institute of Withdrawal Assessment for Alcohol Scale (CIWA-Ar) is a helpful tool for making decisions on treatment initiation or discontinuation. It should be noted that chronic use of benzodiazepines in the secondary prevention of alcohol withdrawal seizures is not recommended due to the addictive potential of these agents and a more severe course of alcohol withdrawal seizures with benzodiazepine co-dependence.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"20 1","pages":"59-65"},"PeriodicalIF":0.0,"publicationDate":"2020-10-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"67582905","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This type of stoke accounts for about 20% (14–24%) of all ischaemic strokes. The high incidence of wake-up stroke may be associated with diurnal variability of heart rate and blood pressure, coagulation processes, as well as episodes of atrial fibrillation, which are more common in the morning. The risk of wake-up stroke increases during REM sleep. Individuals with obstructive sleep apnoea account for the majority of patients with wake-up stroke. Until recently, wake-up stroke was considered a contraindication for reperfusion treatment due to the unknown time of onset and a potential risk of intracranial bleeding. The latest research has shown that wake-up stroke occurs shortly before awakening. Therefore, these patients could be qualified for reperfusion treatment, which would improve their clinical status.","PeriodicalId":41161,"journal":{"name":"Aktualnosci Neurologiczne","volume":"20 1","pages":"66-70"},"PeriodicalIF":0.0,"publicationDate":"2020-10-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"48509949","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
