Moros Marco","doi":"10.24129/j.reacae.28272.fs2003021","DOIUrl":"https://doi.org/10.24129/j.reacae.28272.fs2003021","url":null,"abstract":"78 ABSTRACT Objective: to determine the implementation and use of musculoskeletal ultrasound (Eco MSK) in locomotor system clinics on the part of specialists in orthopedic surgery and arthroscopy in the setting of the Spanish Association of Arthroscopy (AEA). The performance of ultrasound-guided therapy (UGT) or ultrasound-assisted surgery (UAS) was also evaluated. Method: the technical secretariat distributed a survey to all members of the AEA, consisting of 23 questions with 2 or 3 possible answers each. A total of 82 completed surveys were received: 70 answered by traumatologists (85%) and 12 by sports physicians and other specialists (15%) (all of them members of the AEA). Results: sixty percent of the responders were already using Eco MSK in their daily practice, with training through accredited courses and master programs though 66% had been using the technique for less than three years. For confirming the diagnosis, 46% relied on other imaging tests. Forty-one percent performed \"all\" infiltration techniques under ultrasound guidance, and 85% of those surveyed had no experience with UAS though 80% considered it to have a promising future. Conclusions: high satisfaction with the use of Eco MSK was observed (86% of the participants). The main limitation of the study is the small sample size: 82 responders out of 1200 professionals that received the survey. Material and methods: A retrospective study was made of 18 patients subjected to reconstruction of the medial patellofemoral ligament (MPFL) with a tendon allograft affixed by two anchorings in the patella and transosseous femoral tunnel. Anteromedialisation osteotomy of the extensor apparatus was added in 8 patients. The study parameters were the presence of TD (according to the Dejour classification), the distance between the anterior tibial tuberosity and the trochlear groove (TT-TG), the postoperative functional outcome according to the Kajula scale, and the cases of patellofemoral dissociation relapse. The postoperative functional outcomes and relapses were analysed according to the grade of associated TD. Results: The mean patient age was 24.5 ± 7.6 years at the time of surgery, and 66% of the patients were women. The mean postoperative follow-up was 6.5 years (range: 1-12 years). Some grade of TD was recorded in 77.8% of the patients: four cases of Dejour type A trochlea, three cases of type B, two cases of type C and 5 cases of type D. An anteromedialisation tibial osteotomy was added in the cases with increased AT-TG (33.3%). One instability relapse was recorded in the postoperative period. There were no differences in the postoperative functional outcomes between the groups with or without TD (p > 0.05). There likewise were no differences in the case of the relapses. In many such conditions it is still in the experimental phase. One of the fields in which the greatest advances have been made is cartilage treatment, ranging from the use of autologous chondrocyte cultures in a liquid medium to the utilization of biomaterials such as type I/III collagen membranes, and lastly cell dose augmentation with the Instant CEMTRO Cell (ICC) implantation of chondrocytes in membrane technique. In this regard, in Outerbridge grade III/IV cartilage lesions of the knee, ankle and now also the hip, the ICC technique can be used to regenerate the cartilage tissue. This is a regenerative medical technique, since hyaline cartilage tissue is formed with the same structure and function as the healthy joint cartilage. Method: A literature review is made of the basic science referred to cartilage injuries, the surgical technique, and clinical outcomes versus other chondral repair and regeneration techniques. Results: According to the published studies, AMIC improves pain and the clinical and functional scores, affording better outcomes than microfracture surgery and with results similar to those of other repair techniques. Conclusions: Matrix-induced chondrogenesis is a safe, technically reproducible and effective technique for the treatment of cartilage injuries, with good clinical outcomes for the patients. However, long-term studies are needed to assess the evolution of the results over time. Level of evidence: 5. Clinical relevance: It is important to know one of the options available for the treatment of chondral lesions.
