Materials and methods: analytical retrospective cases and controls study, comparing three criteria for suspicious diagnosis of choledocholithiasis: clinical, laboratory and ultrasound criteria. Results: of 146 patients with cholecystolithiasis, 17 (11%) had choledocholithiasis. The clinical criteria for suspicion were nonspecific. There was a significant relationship when comparing cases and control groups between the levels of total and direct bilirubin and alkaline phosphatase, as well as the diameter of the common bile duct measured by ultrasound. However, suspicion through the ultrasound criteria shows greater sensitivity, specificity and positive predictive value","PeriodicalId":409700,"journal":{"name":"Cirugía paraguaya","volume":"26 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-08-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127887150","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

La Cirugía Mayor Ambulatoria se estaba desarrollando como una modalidad de trabajo que resultaba eficiente al sistema de salud, pero no lograba ponerse en el centro de la escena. En este momento, parece ser el modelo ideal para evitar las infecciones por Covid-19 y ante esta oportunidad, potenciar su desarrollo de manera definitiva. El Servicio de Cirugía General del hospital de Clínicas Nicolás Avellaneda de Tucumán, presenta una unidad de CMA integrada con amplia experiencia la cual sirvió como soporte para dar respuesta a la necesidad de resolver las listas de espera quirúrgica, evitar que patologías simples progresaran a complicadas, trabajar con bajos costos y minimizar riesgo de contagios durante el periodo de pandemia en nuestra provincia. Presentamos los pasos y las precauciones tomados para rediseñar y adaptar la modalidad diaria de trabajo.","PeriodicalId":409700,"journal":{"name":"Cirugía paraguaya","volume":"19 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125445178","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

La cirugía en pacientes con enteritis por radiación crónica debe reservarse ante la presencia de complicaciones, ya que está relacionada con una alta morbilidad y estancia hospitalaria prolongadas, así como posibilidad de re operación. Exponemos el caso de una paciente con oclusión intestinal secundaria a enteritis por radiación que desarrolló años después de la conclusión de su tratamiento por cáncer de cuello uterino, con el objetivo de que esta enfermedad sea sospechada en pacientes con antecedentes de radiación.","PeriodicalId":409700,"journal":{"name":"Cirugía paraguaya","volume":"16 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-08-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116630904","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and Methods: Retrospective cross-sectional descriptive study of 26 operative files of patients with diagnosis of achalasia who required surgical intervention in our service in the period January 2016- December 2018. Re- sults: It was observed that the most used surgical method was predomi-nantly laparoscopic, with Heller-dor’s technique being the procedure of choice. There was only one postoperative complication: the esophageal fistula. The mortality of the series in this period associated with surgical intervention was nil. Discussion: The surgical treatment meets the ob-jectives of achieving relief of dysphagia, with a significant improvement in swallowing, normalize the nutritional condition and improve the pa- tient’s quality of life. Laparoscopic surgery achieves results comparable to conventional surgery and adds the advantages of minimally invasive procedures. Conclusion: Heller myotomy modified by partial fundo-plication laparoscopy is the preferred technique for the management of achalasia, with which it is possible to obtain adequate control of symp-toms and long-term satisfaction in acceptable ranges.","PeriodicalId":409700,"journal":{"name":"Cirugía paraguaya","volume":"9 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-04-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115362519","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Objetive: To describe the efficiency of the preperitoneal transabdomino technique (TAPP) and open Lichtenstein-type in the treatment of inguinal hernia in the general surgery service of the Hospital Nacional de Itaugua in the period from August 2017 to August 2018. Methodology: Observational, descriptive, retrospective cross-sectional study; a direct systematic review of medical records with diagnosis of inguinal hernia was performed; the follow-up of the patients was carried out in the post-operative controls by office. Results: 92 patients underwent surgery, of which 51 (55%) had Lichtenstein-type hernioplasties and 41 (44.5%) laparoscopic video hernioplasty, TAPP technique. 95% of the total patients had a hospital stay of 2 days. 96% of the Lichtenstein repairs and 80% of the TAPP lasted from 31 to 90 minutes. There was no intraoperative complication. The main postoperative complication of Lichtenstein-type repair was the hematoma of the operative wound (15%); in the repair by TAPP was the chronic inguinal pain (5%) and the hematoma of the operative wound (5%). The return to productive activities was 53% in the third week for the Lichtenstein technique and 78% in the first week for the TAPP technique. Conclusion: The hospital stay was 2 days in 95% of the cases; the operative time was 31 to 90 minutes. Only post-operative complications, with very low percentage in both and the return to daily activities were faster with the TAPP technique.","PeriodicalId":409700,"journal":{"name":"Cirugía paraguaya","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-08-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126618238","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
