Bu nedenle BPH’nin etiyopatogenezi ve ilişkili risk faktörlerinin ortaya çıkarılması tanı ve tedavi için önemlidir. Son yıllarda yapılan çalışmalarda etiyolojik faktörler olarak heredite ve genetik faktörlerin önemi vurgulanmaktadır. Herediter BPH’nin tespiti hastalık patogenezinde sorumlu bir genin varlığını düşündürmüştür. Bu amaçla birçok çalışmada BPH ile androjen reseptör gen, vitamin D reseptör gen, α1a-adrenoreseptör gen polimorfizmi ve androjen metabolizmasındaki genlerin polimorfizmleri arasındaki ilişki araştırılmıştır ve çelişkili sonuçlar elde edilmiştir. Bilgilerin netleşmesi için farklı popülasyonlarda daha fazla sayıda çalışmalara ihtiyaç vardır. Anahtar Kelimeler: Benign prostat hiperplazisi, heredite, polimorfizm, patogenez Benign prostate hyperplasia (BPH) is one of the most common diseases in men of middle and advanced ages. For this reason, the discovery of etiopathogenesis and associated risk factors is important for diagnosis and treatment of BPH. Recent studies have emphasized the importance of hereditary and genetic factors as etiological factors. The detection of hereditary BPH suggested an existence of a gene in the pathogenesis of the disease. For this purpose, the relationship between BPH and the polymorphisms of androgen receptor gene, vitamin D receptor gene, α1a-adrenoreceptor gene and the polymorphisms of the genes which are responsible in androgen metabolism has been investigated by many studies and contradictory results have been obtained. Böylece ameliyat komplikasyonları ve hastanede kalış süresi kısalabilmektedir. Geleneksel yaklaşımlardan farklı olan bu çok yönlü yaklaşımlar hızlandırılmış cerrahi süreç (fast track surgery) olarak adlandırılmaktadır. Erken mobilizasyon; erken ekstübasyon ve yoğun bakımda kalış süresinin azaltılması ile birlikte, hastanede kalış süresinin kısalmasını hedefleyen “fast track” cerrahi zincirinin bir halkasıdır. Bu çok yönlü yaklaşıma hemşireler de cerrahlar ve anestezistler ile birlikte bakım ekibinin aktif katılımcıları olmakta ve hatta hastalar da bu yaklaşıma dahil edilmektedir. Bu derlemenin amacı ürolojik radikal cerrahi sonrası erken mobilizasyonun etkinliğinin değerlendirilmesidir. Anahtar Kelimeler: Erken mobilizasyon, radikal cerrahi, ürolojik cerrahi Contemporary evidence based trials suggest that same changes in the traditional approach for the care of patients provide a rapid recovery and return to daily life after surgery. Consequently, a decrease in the surgical complications and shorter hospital stay would be provided. These new multi-perspective approaches which are different from the classical approach are defined as “fast track surgery”. Early mobilization besides early extubation and rapid discharge from extensive care unit constitute the major components of fast track surgery in order to bring a shorter hospital stay. Nurses together with surgeons and anesthesiologists are all active members of the “care team” which may also include the patient by him or herself. Gerek medikal gerek cerrahi tedavi seçenekleri son 20 yılda büyük bir gelişim göstermiştir. Cerrahi tedavide 30-80 ml hacmindeki prostatlar için transüretral prostat rezeksiyonu (TUR-P) halen altın standarttır. Günümüzde halen yaygın uygulanmakta olan klasik monopolar TUR-P yanında bipolar TUR-P sistemler de giderek artan oranlarda uygulanmaktadır. Yakın zamanlı meta-analiz sonuçları klinik etkinlik açısından bipolar ve monopolar sistemler arasında fark saptamamakta ve kanama, pıhtı retansiyonu açısından bipolar avantajını öne çıkartmaktadır. Çok merkezli, yüksek hasta sayılı ve farklı bipolar sistemlerin cerrah faktörünün de kontrol edilebildiği prospektif çalışma sonuçlarını elde edinceye kadar mevcut pratikte monopolar veya bipolar sistemler ile tedavi gerekli hastalarda tedaviye devam edilmesi uygun olacaktır. Anahtar Kelimeler: Prostat, transüretral, rezeksiyon, monopolar, bipolar Benign prostate hyperplasia (BPH) related lower urinary tract symptoms (LUTS) is among the most common diseases in the aging male. Both medical and surgical treatment modalities have been improved strikingly in the last 20 years. Transurethral resection of the prostate (TUR-P) is still the gold standard for the surgical treatment for the prostates with the volume of 30-80 ml. Today, besides monopolar classical TUR-P systems which are still widely used, bipolar TUR-P systems are also being used in the increasing numbers. Results of recent meta-analyses did not determine difference between monopolar and bipolar systems in terms of clinical efficacy, in addition, showed bipolar advantages in terms of bleeding and clot retention.
