Its aggressive nature and poor prognosis require prompt diagnosis and treatment, which is often challenging. We herein report a case of a woman who complained of lumps on her left and right neck which move with swallowing motion for 15 years, and have gotten bigger in the previous 6 months. Follicular thyroid carcinoma was first suspected based on previous fine needle aspiration biopsy. Total thyroidectomy and selective neck dissection procedure were performed and histopathology examination post-surgery showed Hurthle cell carcinoma. The patient also received thyroid replacement hormone therapy. Patients with this condition have very severe permanent aural symptoms. As yet, no effective medical or surgical interventions have been reported. We present the case of a 36-year-old female patient who developed patulous Eustachian tube after irradiation for nasopharyngeal carcinoma. A silicone plug was inserted into the patient’s Eustachian tube. Immediately thereafter, her aural symptoms resolved and have not recurred over the subsequent 3 years. We report two cases of stapes malformation in which the stapes footplate was absent, while the oval window was formed but was covered with mucosal membrane. Both the patients visited our department for long-standing hearing loss, which was due to congenital ossicular anomalies of rare variation, as suggested by intraoperative findings. Through two operated cases, we have also discussed the origin and the mechanisms in the development of the stapes and the oval window. The findings suggest that the formation of the oval window is independent from the development of the stapes footplate. They are distinct to dermoid cysts located elsewhere in the body as there is a potential for extension to intracranial structures. Presentation in adulthood is rare and often because of a missed diagnosis in childhood. We present the case of an adult man with a superficial nasal dermoid cyst presenting over the nasion. The operation was performed by direct excision at the nasion with a novel modified horizontal zig-zag skin incision, instead of the traditional vertical excision, with a good cosmetic result. The cyst was radically excised and there was no evidence of extension intracranially. {"title":"Transmastoid Trans-facial canal approach to facial nerve tumors.","authors":"Colin E McCorkle, Bryan K Ward","doi":"10.1080/23772484.2019.1709472","DOIUrl":"10.1080/23772484.2019.1709472","url":null,"abstract":"<p><p>Facial nerve tumors within the temporal bone present several surgical challenges due to the tortuous course of the facial nerve and the nerve's close relationship to other important structures. Surgical approaches often have either sacrificed hearing/vestibular function or involved brain retraction. We present a case of a patient who was diagnosed with a facial nerve schwannoma (House-Brackmann IV/VI). Magnetic resonance imaging (MRI) showed the tumor was limited to the middle ear. The patient had already undergone facial reanimation procedures and elected to have the tumor removed. A transmastoid trans-facial canal surgical approach was used to remove the tumor without disturbing the ossicular chain. The patient's hearing and vestibular function remained intact. Operating from within the facial canal provides the surgeon additional room to dissect facial nerve tumors from the middle ear to the geniculate ganglion when using a transmastoid approach. This surgical approach is similar to the previously described transmastoid/supralabyrinthine approach to excise facial nerve tumors within the temporal bone but modified to keep the ossicles intact. Every bite wound is unique, and thus requires a customized treatment plan. Here, we report the case of a 27-year-old woman who was transferred to the main hospital of Vienna due to a facial dog-bite injury. Primary closure of the wound was possible. The patient received amoxicillin–clavulanate as antimicrobial therapy, and was vaccinated against tetanus, polio, pertussis and diphtheria due to her unknown immunization status. At 183 days after the attack, the aesthetic outcome and nasal ventilation were satisfactory. Based on the reconstructive ladder for wound closure, more severe injuries may require skin or composite grafts, distant or local flaps and microsurgical procedures. Anti-infective therapy against tetanus, rabies and other bacteria also represents a central pillar of dog-bite injury treatment. A superior helical ear composite graft was harvested for the reconstruction of the soft triangle of the left nostril. Fibrin glue is used frequently with split-thickness skin grafts but not with composite grafts before. It was used intraoperatively as a sealant, haemostatic and fixating agent, promote wound healing and prevent skin graft contracture. The complete composite graft-take occurred along with good aesthetic outcome and integration. The symmetry of both nostrils was restored. No graft shrinkage or contracture was observed for more than 10 months follow up. In this case, fibrin glue helped the composite graft complete take and integration with the restored good aesthetic appearance and minimal graft contraction. Here, we present the clinical characteristics and surgical treatment of a case with bilateral second branchial cleft fistulae coexisting with bilateral pre-auricular fistulae. A 32-year-old man with complaints of a mucus-like discharge that was occasionally secreted on both sides of his neck at 2 years of age when swallowing or pressing the upper area of the two openings. He underwent surgical treatment. The fistulae tracts were traced ascending to the area adjacent to the internal and external carotid arteries, passing the glossopharyngeal and hypoglossal nerves, and opening into the tonsillar fossa finally. A 38-year-old man presented to our hospital with vertigo and right-ear hearing loss. The symptoms occurred immediately after a traumatic ear injury after falling on a branch of a camellia tree. Computed tomography showed the presence of air in the vestibule and stapediovestibular dislocation. He underwent surgery treatment. The stapes was found to be depressed into the vestibule, which was cracked, with a piece of the tree branch lodged in the crack. Hence, we did not reposition the stapes. We sealed the oval window using perichondrium, a cartilagenous columella was placed. In this case, immediate surgical intervention was necessary to prevent infection.
