Our patient is an 80-year-old female who had undergone left ear surgery 72 years prior. She presented with a 3-month history of left facial asymmetry associated with otalgia. Patient had been having intermittent discharge from the left ear since her first ear surgery but worsened in the recent 3 months. The patient underwent mastoid exploration. Histopathological study showed no evidence of dysplasia or malignancy. At one-year post surgery, the patient was noted to have soft tissue swelling in the mastoid region which was reported as squamous cell carcinoma. Scans showed extensive disease with possible extradural involvement. Therefore, she was given radical radiotherapy. Chronic ear infection, especially cholesteatoma, may progress to temporal bone malignancy. Obtaining relevant clinical history, in these situations, may be a challenge, due to the fact that young children cannot give a detailed account of what happened and in some cases an adult caretaker may not have been present. In order to complicate even further, many aspirated or swallowed FBs in children tend to be small and radiolucent, making the radiological diagnosis even more challenging. A 16-year-old healthy male presented to the emergency room with the chief complaint of a swollen mass in the left midjugular area of his neck, off-midline. He recalled, that 3 months prior to the incident, he accidentally swallowed a grain of wheat while picking his teeth. One month later, he started complaining of intermittent dysphagia as well as localized edema and erythema in his neck, without pyrexia. Two months later, a neck ultrasound demonstrated a 7 mm hyper echogenic linear mass in a small fluid collection. A contrast enhanced computed tomographic (CT) scan of the soft tissues of the neck revealed an abscess measuring 0.8*2.36*2.0 cm, superficial to the sternocleidomastoid (SCM) muscle. Due to the fact that a superficial collection was diagnosed, the patient underwent incision and drainage during which a splinter of a grain of wheat, measuring 1 cm in length, was surgically extracted. The rapid and accurate diagnosis of MG is mandatory but challenging because patients with MC require intubation and mechanical ventilation or noninvasive ventilation (NIV). We describe a case of an 81-year-old woman diagnosed with MG related dysphagia. Simultaneous fiberoptic endoscopic examination of swallowing (FEES) and ice-pack test (FEES-ice pack test) revealed a significant decrease in pharyngeal residue, suggesting MG. The day following the test, she experienced respiratory failure. FEES-ice pack test results and respiratory distress suggested MC. NIV followed by mechanical ventilation was started quickly, leading to improved respiratory condition. We show that the FEES-ice pack test led to prompt treatment for MC. Recently, transoral surgery (TOS) has become a treatment option for locoregional recurrence after radiotherapy for laryngopharyngeal carcinoma, aimed at preserving laryngeal function. A 67-year-old woman with crT2N0M0 adenoid cystic carcinoma of the tongue base, previously cured with CIRT, underwent salvage endoscopic laryngopharyngeal surgery at another hospital. One month after the surgery, she suffered massive pharyngeal bleeding and was transferred to our hospital for emergency hemostasis. We observed massive necrosis of the tongue base, extending to the epiglottic vallecula, and bleeding from the wound. Consequently, surgical debridement and cauterization were performed. Although wound healing took several months, laryngeal function was finally preserved. Careful postoperative observation is required when salvage TOS is performed in patients who previously underwent CIRT.
