Various indicators of newborn sepsis are expensive and occasionally unavailable in peripheral hospitals. The neutrophil-lymphocyte ratio (NLR), which is affordable, is a component of complete blood counts and does not call for further testing. This study intended to investigate the neutrophil-to-lymphocyte ratio (NLR) as a predictor of neonatal mortality rate in the local hospital in rural locations.Methods: This study sampled newborns with sepsis in the perinatal room of dr. Soebandi Hospital Jember for this cross-sectional study, from January 2021 to June 2022. NLR is calculated by manually dividing the number of neutrophils by the actual number of lymphocytes. Our neutrophil and lymphocyte data were obtained from the patient's lab results. This study analyzed neonatal sepsis NLR data using Spearman analysis to find the relation between NLR value and mortality rate of neonatal sepsis.Results: This study obtained 43 data on neonatal sepsis in the perinatology ward during this period. The data found to show that 37 neonatal sepsis survived and 6 died. The collected data analysis found that the lowest NLR neonatal sepsis is 0.4, the highest NLR neonatal sepsis is 25.6 and the mean value for NLR neonatal sepsis in perinatology was 3.88. This study found no correlation between the NLR value of the sepsis neonate and the neonatal mortality rate Sig. The detrimental effect of this condition has led to the development of various treatments options, such as Moringa leaves extract (Moringa oleifera), which was suspected to have ameliorative effects. Therefore, this study aimed to determine the effect of Moringa leaves extract on the histopathological appearance of the testes of Wistar rats induced with metabolic syndrome.Methods: This was an experimental design study, with a post-test-only control group, involving 30 Wistar rats (Rattus norvegicus). Metabolic syndrome was induced in the samples through the administration of high-fat and fructose feeds along with streptozotocin and nicotinamide. Furthermore, the moringa leaves extract was administered to the treatment groups in different doses of 150, 250, and 350 mg/kg BW. Testicular histopathological damage scores were then assessed on each group. The Kruskal-Wallis test which followed by Mann-Whitney test was used to determine the significant differences between the groups, while the correlation was analyzed using Spearman test.Results: There were significant differences in the histopathological scores between the treatment and control groups. Furthermore, the Wistar rats treated with Moringa extract showed better histopathological scores (2.1±0.64) compared to those without the treatment (2.7±0.54), p<0.05. The results also showed that higher doses of the extract significantly correlated with better outcome in testicular histopathological structure of metabolic syndrome rat model (p=0.000, r=-0.396).Conclusion: Moringa leaves ethanolic extract treatment improved testicular histopathological appearance in Wistar rats induced with metabolic syndrome.","PeriodicalId":404004,"journal":{"name":"Smart Medical Journal","volume":"56 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131562244","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Although cases of thyroid crisis are rare, the mortality rate is significant which reaches 20–50%. The presence of comorbidities such as pneumonia infection increases the risk of death dramatically. Infection is a factor that can trigger thyroid crisis and exacerbate thyrotoxicosis. This condition of thyroid crisis most often occurs in people with hyperthyroidism caused by Graves' disease. We present the case of a patient with Graves' disease, aged 22 years, who developed a thyroid crisis due to community-acquired pneumonia infection. This study aims to provide a case report regarding the thyroid crisis due to pneumonia.Results: We carried out a physical and some appropriate supporting examinations to confirm the diagnosis. Wayne's index was 26, indicating hyperthyroidism. Assessment with Burch-Wartofsky obtained a score of 45, indicating suspicion of a thyroid storm. Then supportive care was provided to the patient. The community-acquired pneumonia as a triggering factor was treated with antibiotics. Thyrotoxicosis conditions were treated with antithyroid, beta-blocker, and glucocorticoid drugs. The patient's condition improved after treatment.Conclusion: It is crucial to detect and treat thyroid storm as quickly as possible since it has a high mortality rate. Wayne's score is useful for identifying hyperthyroidism based on clinical symptoms. The Burch-Wartofsky Point Scale scores are used to diagnose thyroid storms. Thyroid storm should be managed according to PERKENI (Perkumpulan Endokrinologi Indonesia) and ATA (American Thyroid Association) recommendations. This case serves as an example of how to make a proper diagnosis and treatment to prevent morbidity and death due to thyroid crisis.","PeriodicalId":404004,"journal":{"name":"Smart Medical Journal","volume":"43 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127549914","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We report the case of a man with a right double J (DJ) stent that was placed 5 months previously. The patient underwent anamnesis, physical examination, supporting examinations and was then treated with ureteroscopic lithotripsy and PCNL. Complete clearance of the stone can be performed and the DJ stent can be removed uninterruptedly from the percutaneous access. Encrusted stents are still a challenge in urology where endourology remains the best choice of treatment. The management of DJ stents should also be based on follow-up and prevention by taking into account predisposing factors to encrustation such as urinary tract infections, diabetes mellitus, chronic renal failure, history of urolithiasis, diet and malabsorption.","PeriodicalId":404004,"journal":{"name":"Smart Medical Journal","volume":"57 11","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-05-16","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114024166","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

GS symptoms do not normally manifest until beyond the age of six years. We present a rare case of a 21-year-old woman with a background in generalized weakness muscle cramps with tetany, fever, and dyspnea. The goal of this study is to give a case report of Gitelman syndrome suspicion.Result: Gitelman syndrome is identified through clinical indicators and lab testing, such as low potassium and magnesium levels in the blood, metabolic alkalosis, and low calcium levels in the urine. We do a variety of physical and laboratory tests to determine the disease and rule out other possibilities. Molecular proof is difficult to get due to a lack of resources and the high expense of genetic testing, which has become one of our case report's flaws. This patient was diagnosed as suspecting Gitelman syndrome and treated for electrolyte imbalances. The patients were given replacement therapy, which comprised intravenous potassium, sodium, and chloride therapy, as well as calcium therapy. In addition, we also provided some symptomatic therapy. She was discharged five days later in fine condition.Conclusion : This is one of the few reports of a 21-year-old lady being suspected of having Gitelman syndrome (GS).
