We report the case of a patient with chronic kidney disease and severe stenosis of the internal carotid artery (ICA) on the right side of the neck who underwent CAS through a balloon distal protection device with little use of contrast medium. Case : A 79-year-old male was admitted to our hospital for left hemiplegia. Diffusion MRI revealed an acute symptomatic brain infarcts in the right hemicerebrum; a carotid duplex ultrasound revealed right ICA stenosis (peak systolic velocity, 287cm/s). Blood test showed chronic kidney disease (eGFR, 33.8ml/min/1.73m 2 ). Ultra-sound-guided CAS was performed to use a minimized contrast medium. Ultrasound-guided CAS plays a vital role in the assessment of distal balloon protection, including inflation or deflation of a balloon. The total volume of iodinated contrast medium was 7ml. The day after CAS, the patient did not develop contrast medium-induced nephropathy. Conclusions : Ultrasound-guided CAS is safe and feasible for patients with preexisting chronic kidney disease, and intraprocedural status change can be confirmed in real time.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"40 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2019-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"72812408","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Calf deep venous thrombosis (DVT) has been detected frequently in the residents due to earthquake. It was examined whether it relates to DVT, cerebro-cardiovascular diseases (CCVDs), etc. Subjects and Methods : We conducted screening for 522 residents in Kashiwazaki City in 2016–2017. HITS were detected by using 2.0MHz paste type probe at carotid artery for 5min. Results : Carotid HITS was detected in 39 (7.5%). Statistically significant risk factors in the carotid HITS positive group were arterial velocity pulse index (AVI), CCVDs and atrial fibrillation. Univariate analysis of cerebro-cardiovascular disease indicated an odds ratio of 3.8 (95% CI: 1.8–7.9, p = 0.001) in the carotid HITS positive, and furthermore odds ratio 3.6 (95% CI: 1.6–7.9, p = 0.001) was found in the same group in multivariate analysis. Conclusion : Carotid HITS was a significant risk factor for CCVDs and atrial fibrillation in the general popula-tion. Therefore, it was suggested that detection of HITS at carotid artery by paste type probe may be useful for prevention of embolic disease.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"138 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2019-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82262918","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, few studies have followed up patients with carotid characteristics using contrast-enhanced ultrasonography (CEUS). Objective and Methods : Between July 2015 and September 2017, we enrolled 50 patients with carotid stenosis who underwent carotid ultrasonography (CUS), CEUS, and black-blood magnetic resonance imaging (BB-MRI) to evaluate the carotid characteristics. We assessed the correlation between enhancement-positive, ulcer-positive, T1-high, and recurrence or ipsilateral ischemic stroke or death. Results : Of the 50 patients hospitalized for carotid stenosis, 8 (16%) developed recurrence of ipsilateral ischemic stroke or died in the first year of follow-up. In the ulcer-positive group, the recurrence or death rate was 35% (6/17) during the first year, which was statistically different from that in the ulcer-negative group ( p < 0.05). Conclusions : Follow-up by CEUS in patients with ulcer-positive carotid plaque can provide valuable informa-tion regarding ipsilateral ischemic recurrences or death.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"30 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2019-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"85838655","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Previous studies showed a positive correlation of CSVD with physiological parameters such as pulse wave velocity (PWV) or with radiographical findings such as leukoaraiosis; however, the role of these parameters remains controversial. Therefore, we assessed whether there is a relationship between stroke subtypes and these potential CSVD-related parameters. Methods : In a multicenter, retrospective study from 8 hospitals, we enrolled 874 patients with a history of cerebral infarction or ICH who underwent both carotid ultrasound and MRI examination between June 2008 and May 2010. We evaluated the values of PWV, ultrasonographic parameters and MRI findings, and compared them between the stroke subtypes. Results : Of patients, 331 (38%) were classified into the large artery atherosclerosis (LAA) group, 387 (44%) into the small vessel occlusion (SVO) group, and 156 (18%) into the ICH group. Lower max intima media thickness (IMT) and lower plaque score, and higher PWV were shown in SVO and ICH groups than those in LAA groups. Higher pulsatility index (PI) of common carotid artery (CCA) in SVO group, higher PI of ICA and higher prevalence of cerebral micro-bleeds (CMB) in ICH group were also shown than those in LAA group. After adjustment for vascular risk factors, the following parameters in both SVO and ICH groups were signifi-cantly different from those in the LAA group: lower max IMT, lower plaque score, higher periventricular hyperintensity (PVH) grade, and higher prevalence of CMB. PIs of CCA and internal carotid artery (ICA) were higher in ICH group than those in LAA group. Conclusions : The similar trends of some physiological and radiographical parameters were shown in ICH and SVO group compared with LAA group, which reflected the underlying pathophysiology of CSVD.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"28 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2019-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87003225","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Brain MRI showed multiple subcortical lesions. High-intensity transient signals (HITS) was detected in the carotid artery and transesophageal echocardiography (TEE) was performed to search for embolic sources. Mobile string-like structures were found in the aortic arch. We started anticoagulation therapy. One month later, TEE reexamination revealed that the aortic arch cord had shrunk, but numerous cords were found in the pulmonary artery. She was suspected with Angiostrongylus cantonensis infection and treated with antiparasitic agent and steroids. One month later, the TEE was performed again, and the cord-like structure in the aortic arch almost disappeared, and the abnormal structure in the pulmonary artery also disappeared. It is known that A. cantonensis can cause eosinophilic meningoencephalitis, but its prognosis is good, and it is thought that it may be mild. In this case, although the definitive diagnosis has not been made, the possibility was considered to be high because antiparasitic agent was effective. HITS detection for investigating the cause of multiple subcortical lesions was considered to be useful in determining the embolic mechanism.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"22 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2019-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"85004445","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
