Three days later, she was taken to the emergency room by ambulance since she was vomiting and scarcely moving. On admission, neurological examination revealed only vertigo, and NIHSS score was 0. Laboratory examination showed slightly increasing D-dimer and elevated CA15-3 and BCA225 levels. Since MRI showed a hyperintense area in the cerebellum, she was hospitalized under the suspicion of cancer-related stroke. Enhanced CT showed no deep vein thrombosis, and transesophageal echocardiography, transthoracic echocardiography, and carotid ultrasonography did not indicate an embolic source. However, transcranial color flow imaging (TC-CFI) showed 40 microembolic signals (MES) in the basilar artery (BA) for three minutes, with no MES in the middle cerebral artery (MCA). Despite the risk of hemorrhagic transformation, heparin was started on the six day of admission. Thereafter, MRI showed no recurrence, and MES was not seen in both the MCA and BA by TC-CFI. In this case, MES decreased signifi-cantly after heparin treatment, as indicated by TC-CFI, suggesting that MES detection by TC-CFI was useful for understanding the morbid state of cancer-related stroke.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"16 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83061040","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We aimed to determine whether intraplaque blood flow (IPBF) could be visualized without contrast medium using B-flow ultrasound imaging, and to evaluate the relationship between IPBF and ultrasound findings or clinical significance. Methods : We investigated IPBF in 66 carotid arteries with ≥ 30% area stenosis using B-mode ultrasound imaging with a Logiq s8 ultrasound system (GE) and a linear probe (9L). We assessed characteristics such as signal intensity and plaque uniformity, as well as their relationships with IPBF. Results : We visualized IPBF in 20 funicular and linear lesions with either a straight or a tortuous course. Among them, IPBF was connected with the outside of the vessel in 4 lesions, with the vascular lumen in 4 lesions, and with both sides in 9 lesions. Twenty lesions with IPBF had hypoechoic and heterogeneous plaques more frequently than 46 lesions without IPBF. Lesions with IPBF were symptomatic relatively compared with those without IPBF. Conclusion : B-flow ultrasound imaging could detect IPBF without contrast medium. IPBF was observed more frequently in hypoechoic and heterogeneous plaques, and lesions with IPBF were symptomatic relatively, there-fore seemed to indicate plaque instability.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"76 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83852371","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

TOPU is applicated from transoral carotid ultrasonography, and useful for delineation of pharyngeal lesions. Peritonsillar abscesses is well known as common disease in otorhinolaryngology, and often needed a puncture drainage of abscesses. This procedure usually is performed blindly into the inside of pharynx, thus, it has a risk of fatal complications and low success rate. TOPU-guided needle aspiration appears to be useful for safe drainage of peritonsillar abscesses. We report a case of peritonsillar abscess whom we performed puncture drainage with TOPU guiding.","PeriodicalId":40206,"journal":{"name":"Neurosonology and Cerebral Hemodynamics","volume":"1 1","pages":""},"PeriodicalIF":0.1,"publicationDate":"2020-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75959222","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
