The existence of LP could change the paradigm of dementia as a degenerative, chronic, progressive and irreversible disease (where neuronal death plays the leading role), towards a model where functional deficits of neuronal networks acquire importance, which raises new potentially reversible therapeutic and rehabilitative possibilities. We analyze the ethical consequences that these episodes may have with respect to the implicated persons (patients, caregivers and professionals in charge of their care) and try to answer the following question: Do persons with advanced dementia continue to maintain their personal identity despite suffering cognitive impairment so severe?. The LP indicates that this is possible. Increasing longevity entails an increase in the demand for professionals who prescribe physical activity, specifically physiotherapists and physical-activity educators.</p></div><div><h3>Objectives</h3><p>The main objective of this study was to explore the perceptions of a group of third- and fourth-year Physiotherapy students (n<!--> <!-->=<!--> <!-->9) and Sport Sciences students (n<!--> <!-->=<!--> <!-->5), and experts who work with older adults (n<!--> <!-->=<!--> <!-->3) about their knowledge of the older adult population and healthy aging. The secondary objectives were to: (a) explore what knowledge future professionals need about physical activity programming and about physical activity programs aimed at maintaining and improving health among older adults; (b) explore what would be the best methodology to acquire such knowledge; and (c) explore whether a relationship is perceived between knowledge about the older adult population and motivation to work with this population group.</p></div><div><h3>Methods</h3><p>Two discussion groups with students and three interviews with experts were conducted. The aim of the study was to assess the characteristics of patients over 65 years of age who consulted the ED for traumatic brain injury (TBI) in 2022, their relationship with cognitive impairment, functional dependence, use of oral antiplatelet/anticoagulant drugs and complications.</p></div><div><h3>Materials and Methods</h3><p>Retrospective study conducted from 1 January to 31 December 2022. Demographic data were collected: age, sex, origin; cardiovascular risk factors; cognitive impairment using the Pfeiffer questionnaire; physical disability according to the Barthel Index; number of drugs; use of antiplatelet and oral anticoagulant (OAC); mechanism of fall; performance of cranial X-ray/CT, and presence of complications: intracranial haemorrhage (ICH), death.</p></div><div><h3>Results</h3><p>599 patients were included. The mean age was 82.3<!--> <!-->±<!--> <!-->8.2 years. 63.8% were female and 36.2% male. 75.3% were from home, 24.7% from residence. No dementia in 61.4%, moderate-severe dementia in 38.6%. 58.1% were functionally independent, 25.1% had moderate-severe dependence. 85.7% had CVRF: HT 476 (79.5%), dyslipidaemia 354 (59.1%), DM 217 (36.2%), obesity 173 (28.9%), smoking 15 (2.5%). The number of drugs per patient was 9.2<!--> <!-->±<!--> <!-->4.3. Polypharmacy was present in 94.7% of patients. 35.9% were taking antiplatelet drugs and 30.2% anticoagulants. Intracranial haemorrhage occurred in 11 (2.3%) patients. Four (0.7%) patients died.</p></div><div><h3>Conclusions</h3><p>The TBI in our study was caused by low-energy trauma in a female patient, without dementia, functionally independent and with polypharmacy. Telemonitorización de pacientes con deterioro cognitivo y sus cuidadores","authors":"Mercedes Fernández López , Montserrat Martín-Baranera , Meritxell Davins Riu , Benito Fontecha Gómez","doi":"10.1016/j.regg.2024.101492","DOIUrl":"https://doi.org/10.1016/j.regg.2024.101492","url":null,"abstract":"<div><h3>Introduction</h3><p>Given the growing increase in dementia, the need to control these patients, together with the rise of new technologies, makes a change in the current control system imperative.</p></div><div><h3>Material and method</h3><p>We have carried out a single-center, clinical study with two groups, a control group of 72 patients/caregivers, who followed the usual controls in consultations, and another telematic group of 76 patients/caregivers, who followed the controls through of the Tecuide platform. The aim of the study was to analyze the perception of the support they receive and the psychosocial well-being of people over 60<!--> <!-->years of age who take care of their grandchildren.</p></div><div><h3>Methodology</h3><p>Cross-sectional descriptive observational study. A sample of people over 60<!--> <!-->years of age, who attended the pediatrics service in three health centers accompanying their grandchildren, during the first four-month period of 2018. They were administered the Duke-UNC questionnaire, with measures of social support and involvement in the care of their grandchildren.</p></div><div><h3>Results</h3><p>The majority cared for their grandchildren between 5 and 14<!--> <!-->hours per week, performing recreational and maintenance activities. The participants reported a significant relationship of the caregiver's role in their quality of life and well-being; 88% of the subjects perceived a high level of social support. This study aims to analyze models of advance care planning in elderly individuals living in nursing homes. We reviewed papers published in Cochrane, PubMed and Embase. A total of 26 studies were selected, including a total of 44,131 people over 65 years of age. We analyzed the types of intervention (interviews, videos, workshops, documentation, etc.) and their results derived from the application. We conclude that no study implements a standardized intervention model. These interventions include decision-making (transfers to hospital, resucitation orders) and the adequacy of therapeutic effort (antibiotherapy, nutrition, serotherapy, etc.).
