On clinical examination, the patient was tachycardic with a fever of 39 and abdominal tenderness. The biological assessment showed anaemia with thrombocytosis and elevation of C-reactive protein with hypoalbuminemia. A careful colonoscopy was performed showing fragile erythematous mucosa with spontaneous bleeding and pseudopolypoid mucosal lesions (A) and Endoscopic signs of severity consisting of deep ulcerations and of mucosal detachment (B). Pathological examination showed severe inflammation with deep ulcerations and cryptic abscess suggestive of severe acute ulcerohemorrhagic colitis, stool examination was negative for pathogens and Clostridium difficile toxin, serology and cytomegalovirus PCR (polymerase chain reaction) were negative. The diagnosis of severe acute colitis complicating the ulcerohemorrhagic colitis disease was retained, the patient was put on injectable corticosteroids, the evolution was favourable after 5 days with normalization of the C-reactive protein level and disappearance of the rectorrhages thus a relay by oral corticosteroids associated with azathioprine was indicated. After 2 years of follow-up, the patient remained asymptomatic with salicylate and azathioprine.","PeriodicalId":397112,"journal":{"name":"PAMJ Clinical Medicine","volume":"32 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-12-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132268507","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

On arrival, during the medical history taking, he reported neither old trauma of the left elbow nor weight loss. Physical examination of the elbow showed neither limitation of movements nor evidence of a palpable mass nor soft tissue swelling. X-rays of the elbow showed a cystic lesion in the distal humeral metaphysis. We performed a computed tomographic scan of the arm, which demonstrated a well-defined fat density intramedullary lesion within the distal metaphysis of the right humerus without signs of cortical destruction or periosteal reaction. Completed T1-weighted Magnetic Resonance images revealed an intramedullary lesion with an equal signal intensity to subcutaneous fat. T2-weighted images also showed a high signal intramedullary lesion similar to subcutaneous fat. According to the typical imaging findings, a symptomatic intraosseous lipoma was suspected prompting the patient to undergo surgery for curettage and bone grafting. The patient´s initial postoperative course was uncomplicated and so he was discharged with an oral analgesic to be reviewed in two weeks with x-ray control of the left elbow. The histopathological findings confirmed the intraosseous lipoma of the humerus. At the last follow-up of three years, the patient showed no pain and no recurrence of the bone lesion.","PeriodicalId":397112,"journal":{"name":"PAMJ Clinical Medicine","volume":"23 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127796251","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Cette lesion, initialement asymptomatique, a occasionne, apres 2 mois d´evolution, des douleurs inguinales puis du membre inferieur gauche, non systematisees. La douleur, au depart mecanique et calmee par le repos et les antalgiques, est progressivement devenue inflammatoire et insomniante, amenant la malade a consulter. A l´examen, elle etait apyretique et en bon etat general. La palpation retrouvait une masse mal limitee, adherente au plan profond, legerement sensible et fremissante (A). La vitesse de sedimentation etait a 45mm a la 1e heure et les globules blancs a 6500 elements/mm3 avec eosinophilie a 2,1%. Les radiographies standards de la hanche gauche etaient normales. L´echographie de la cuisse gauche a mis en evidence une enorme collection enkystee, homogene sans calcification ni vegetation en dedans bien limite par une fine membrane (capsule) qui se rehausse au Doppler. L´imagerie par resonance magnetique (IRM), pratiquee pour mieux etudier les rapports de la masse avec les structures vasculaires et nerveuses avoisinantes, a montre un kyste hydatique type 1 dans la loge des adducteurs de la face interne de la cuisse gauche mesurant 108*82mm, presentant des contacts etroit avec l´artere femoral superficielle (B, C, D). Une biopsie exerese du kyste a ete realisee dont les resultats anatomo-pathologiques sont revenus en faveur d´une hydatidose de la cuisse. Le traitement chirurgical a ete enchaine par un traitement medical a base d´Albendazol pendant 28 jours avec une bonne evolution clinique et radiologique.","PeriodicalId":397112,"journal":{"name":"PAMJ Clinical Medicine","volume":"57 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-11-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131138401","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Similar to most of the second line drugs, CS has many adverse drug reactions (ADR). The ADR due to CS related to the central nervous system (CNS) are very rare and are reported in few isolated case reports. We herein, present a first case series from the national capital of India of three MDR-TB patients with CS-induced ADR’s of the CNS. In all the three cases the drug was withdrawn and the symptoms were relieved. The present case series will definitely serve as a tool to spread awareness about the rare ADR’s associated with CS.","PeriodicalId":397112,"journal":{"name":"PAMJ Clinical Medicine","volume":"53 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-11-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133067464","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It may rarely be practiced by healthy adults for masturbation. A diagnosis is usually made radiologically. Iatrogenic foreign bodies were found to be the most frequent type of insertion encountered. Endoscopic retrieval is usually successful, with minimal morbidity. The aim of this paper was to expose our experience about intravesical foreign bodies. Cependant, d’autres etiologies sont meconnues telle l’hypoplasie congenitale, des progres moleculaires ont augmente le nombre de diagnostics possibles. L’hypoplasie congenitale des surrenales est une maladie genetique rare, heterogene sur le plan clinique, de revelation parfois precoce et letale en absence de traitement adapte, une survie prolongee en l’absence de traitement conduisant a un diagnostic tardif de la maladie est aussi possible, certaines de ces hypoplasies congenitales sont liees a des mutations observees dans le gene DAX1 sur l’X qui code pour un membre de la superfamille des recepteurs nucleaires aux hormones. Nous rapportons ici l’observation d’un enfant de 12 ans, qui a presente une insuffisance surrenalienne peripherique, avec absence d’individualisation des surrenales sur tomodensitometrie; chez qui l’hypoplasie congenitale des surrenales a ete suspecte.","PeriodicalId":397112,"journal":{"name":"PAMJ Clinical Medicine","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-11-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131281569","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
