Takase","doi":"10.11480/620302","DOIUrl":"https://doi.org/10.11480/620302","url":null,"abstract":"The \"Guidelines for Obstetrical Practice in Japan\" are the first consensus-based guidelines in the field of obstetrics and were published in 2008 owing to an obstetric medical care crisis in Japan. The Guidelines describe appropriate methods for obstetric care and were based on consensus among obstetricians in Japan. Originally, the Guidelines were not intended to be a rulebook to limit physician discretion; however, from a medical care litigation standpoint, medical practices that conform to the Guidelines are following the \"medical standard\". The present study aimed to investigate obstetricians' perceptions of the Guidelines, to assess the degree of compliance, and to consider the implication of physician discretion in medical litigation. A questionnaire exploring the perception of the Guidelines was completed by 341 obstetricians was conducted from February 2009 until May 2009; questionnaires were evaluated using simple aggregation, correlation analysis, and principal component analysis. Although more than 80% of physicians responded that content of the Guidelines was appropriate, only 34.7% complied with recommendation level A. CRF and related variables were measured using self-report scales in a cross-sectional manner.\u0000\u0000\u0000RESULTS\u0000The average age was 44.9 years among 64 women. The higher CRF group comprised 22 women (34%), 10% of whom had severe fatigue. The participants had higher physical activity levels compared with the findings in previous studies, and reported an average of 40 min/day of moderate to vigorous activity. Moderate to vigorous levels of physical activity were derived from essential social activities rather than leisure time exercise. There were no significant differences in physical activity levels between the lower and higher CRF groups.\u0000\u0000\u0000CONCLUSION\u0000Our study results suggested that the higher level of physical activity in daily living itself had no relationship with decreasing CRF among premenopausal cervical and endometrial cancer survivors. Clinical oral malodor was measured in 163 Malaysian dental students using organoleptic method and Oral ChromaTM and they were asked about their perception of self-oral malodor. Oral examination was performed to assess oral health status. Statistical analyses were performed using SPSS version 19.0. There were 52.7% students who had clinical oral malodor, while 19.0% students perceived they had oral malodor. The sensitivity (0.244) of self-perceived oral malodor was lower than its specificity (0.870). Tongue coating was closely associated with clinical oral malodor whereas high plaque index was closely associated with perceived oral malodor. These results showed that clinical oral malodor was prevalent among dental students, but students' perception of oral malodor did not always reflect actual clinical oral malodor. Furthermore, associating factors of clinical oral malodor differ from perceived oral malodor. Because few studies to date have evaluated disease staging in Japan, a small pilot study was designed to determine whether disease staging is available and useful in actual medical practice in Japan. The relationships between disease staging and length of stay, medical costs and age were retrospectively evaluated in patients admitted to Japan Association for Development of Community Medicine - Tokyo Bay Urayasu Ichikawa Medical Center for appendicitis, type 2 diabetes mellitus, and cerebrovascular diseases from April 2012 to March 2013. Patients were easily staged based on information at the time of hospital discharge. Disease stages were found to be affected significantly by length of hospital stay and medical costs. Age also affected disease stages in patients with appendicitis. Both proteins have a predicted nuclear localization signal and five CCCH motifs (zinc-binding motifs), and share a degree of sequence similarity with Nab2, a yeast protein that contains nucleic acidbinding motifs and tandem CCCH zinc fingers. Nab2 binds homopolymeric RNA and single-stranded DNA and regulates poly(A) tail length and the export of mRNA to the cytosol. The CCCH motifs of UKp83/68 bound poly(A) and ssDNA strongly and other RNA homopolymers and dsDNA less efficiently. The UKp83/68 protein localized within the nucleus with a fibrous or punctate structure that reflected the distribution of SC35, a known marker of nuclear speckles which are nuclear domains enriched in pre-mRNA splicing factors and located in the interchromatin regions of the nucleoplasm of mammalian cells. The distribution of UKp83/68 changed during the different stages of mitosis. During prometaphase, when the nuclear envelope disintegrates, the protein becomes partially localized on the chromosomes; at other times, transiently dispersed over the cytoplasm with the formation of fibrous structure. The transient expression of UKp83 in HEK293T cells had no apparent effect on cellular function, whereas the expression of an antisense sequence or C-terminal domain of UKp83 induced apoptosis. Fifteen healthy young adult subjects voluntarily participated in the present study. Electrocardiogram (ECG), electroencephalogram (EEG) and corrugator muscle electromyogram (EMG) were recorded and changes of them by viewing videos of dental treatments were analyzed. The subjective discomfort level was acquired by Visual Analog Scale method. Analyses of autonomic nervous activities from ECG and four emotional factors (anger/stress, joy/satisfaction, sadness/depression and relaxation) from EEG demonstrated that increases in sympathetic nervous activity reflecting stress increase and decreases in relaxation level were induced by the videos of infiltration anesthesia and cavity excavation, but not intraoral examination. The corrugator muscle activity was increased by all three images regardless of video contents. The subjective discomfort during watching infiltration anesthesia and cavity excavation was higher than intraoral examination, showing that sympathetic activities and relaxation factor of emotion changed in a manner consistent with subjective emotional changes. Seven doses of L-asp (6,000 U/m2) were administered intravenously, with one dose administered on every alternate day. Five consecutive patients were enrolled. Three patients completed the treatment. The clinical symptoms resolved in 1 patient who started the administration 8 months after the onset, being the earliest among the 5 patients. Her EBV-DNA level in whole blood markedly decreased to 0.08 times of that before treatment, and the level in plasma became undetectable. In the other 2 patients whose administration was started 3 and 3.5 years after the onset, however, a remarkable improvement was not detected. Treatment was discontinued in 2 patients because of disease progression or idiopathic dystonia. The mRNA levels of asparagine synthetase in EBV-infected cells were examined. The level from the patient who responded to L-asp treatment was low, but it did not correlate with the effects in the other patients. Liver dysfunction (grades 2 and 3) was observed in 2 patients and neutropenia (grade 3) was noted in 1 patient. This study was intended to help establish the standard indications for transoral surgery and additional therapy in patients with superficial head and neck carcinoma.</p><p><strong>Methods: </strong>Sixty-two patients with 83 superficial head and neck carcinoma underwent transoral tumor resection at the Cancer Institute Hospital between June 2006 and September 2011. We measured the tumor size and thickness, examined the gross appearance, permeation of vessels, and droplet infiltration, and analyzed the correlations between each parameter.</p><p><strong>Results: </strong>Sessile type of tumor on gross appearance showed a significantly higher incidence of thickness≥1000 µm than the other types. The JEPP aims to promote mother-infant interactions by enhancing the mother's ability to respond appropriately her child. Mothers in the JEPP group (n = 15) received support from IMH nurses in a pediatric clinic until their infants reached 12 months of age. The nurses provided positive feedback that emphasized strength of parenting, and assisted the mothers in understanding the construct of their infants. Mother-infant interactions and mother's mental health status were assessed at intake (1-3 months), and at 6, 9, and 12 months of infants' age. The JEPP group data were compared with cross-sectional data of the control group (n = 120). Although JEPP dyads were not found to be significantly different from the control group in general dyadic synchrony, both before and after intervention, JEPP mothers significantly improved their ability to understand their infant's cues and to respond promptly. In the JEPP group, unresponsiveness to infants was reduced in mothers, while infants showed reduced passiveness and enhanced responsiveness to the mother. Prospective studies were conducted twice with 13 dyads of mothers and infants who were between one (Time 1) and two years old (Time 2). Cross-sectional and longitudinal analyses were performed. The mean Developmental Quotients (DQ) significantly decreased between Time 1 and Time 2 (p < .05). Four (30.8%) infants showed DQ p < 85 at Time 1. They showed a DQ below the borderline at Time 2 and their developmental changes were different from RB infants in the normal DQ range. Infant Behavior Checklist-R (IBC-R) scores negatively correlated with DQ (p < .05). The mothers' Japanese Nursing Child Assessment Teaching Scale (JNCATS) scores were significantly higher than the normative mean. The IBC-R scores negatively correlated with mothers' JNCATS scores at Time 1 (ρ = -.66, p < .05) and positively correlated with the child domain scores on the Parenting Stress Index at Time 2 (ρ = .62, p < .05). Some RB infants showed a developmental delay. Mother-child interactions were negatively affected and mothers recognized their infants' problem.
