Cechnicki","doi":"10.5114/PPN.2018.79557","DOIUrl":"https://doi.org/10.5114/PPN.2018.79557","url":null,"abstract":"Purpose: The aim of the study was to assess the relationship between the family attitude and clinical and social outcome measures in the group of 45 people diagnosed with schizophrenia during 20 years of illness. Methods: For the evaluation of clinical and social variables, the Anamnestic and Catamnestic Questionnaire, the BPRS scale and the scale of social functioning according to the DSM-III axis V were used. The family attitude during the first hospitalisation was measured with CFI. In subsequent measurement points, a 4-stage clinical scale based on the CFI categories (warmth and kindness, criticism, hostility, emotional overinvolvement) was used, which was then analysed as a dichotomous variable: favourable/unfa- vourable attitude. Results: A significant tendency to improve the family attitude within years of illness was observed. In women, the change in the fam- ily attitude was faster than in case of men. The unfavourable family attitude was associated with more frequent relapses and re-hospitalisations after 3 and 12 years after the first hospitalisation. The severity of symptoms and the level of social functioning were not related to the family attitude at any measurement point. Conclusions: In the community treatment program, during the 20-year course of schizophrenia, the unfavourable family attitude improved among both women and men, but beneficial changes occurred faster in women. Mokros","doi":"10.5114/ppn.2018.77034","DOIUrl":"https://doi.org/10.5114/ppn.2018.77034","url":null,"abstract":"Purpose: The aim of this study was to assess the relationship between the internalised stigma of mental illness associated with the use of antipsychotic treatment and psychopathological profile among patients diagnosed with schizophrenia. Method: The study group comprised 64 patients diagnosed with schizophrenia. The Internalised Stigma of Mental Illness Inventory was used to assess the degree of self-stigmatisation in five aspects: Alienation, Stereotype Endorsement, Discrimination experience, Social withdrawal and Stigma resistance. The symptoms were assessed by a psychiatrist using the Clinical Assessment of Schizophrenic Syndromes Scale. Other variables of interest included data on current pharmacotherapy, body-mass index (BMI) and general functioning. Multivariate linear regression models were created to assess the association between variables of interest and ISMI subscale scores. Results were considered statistically significant when p < 0.05. Results: Zuclopenthixol, a long-acting injectable (LAI), was linked to greater Alienation, Stereotype endorsement and Discrimination experience, while risperidone LAI was associated with higher indices of Stereotype endorsement. Oral risperidone was linked to lower severity of Alienation and a greater level of Stigma resistance. A rise in the Lack of insight score predicted falls in Alienation, Stereotype endorsement, Discrimination experience and Social withdrawal scores. A rise in the Disturbances of Sense of Self score predicted rises in Alienation, Stereotype endorsement and Discrimination experience. Ogińska-Bulik","doi":"10.5114/ppn.2018.80884","DOIUrl":"https://doi.org/10.5114/ppn.2018.80884","url":null,"abstract":"Purpose: The cognitive processing of trauma, expressed in the form of coping strategies, aims to help individuals adapt to the new reality that emerges following an experience of trauma. The research presented here is focused on determining the role of cognitive strategies of coping with trauma and the occurrence of consequences both negative, in the form of posttraumatic stress disorders (PTSD), and positive, in the form of posttraumatic growth (PTG). Methods: A total of 340 people who experienced various traumatic events (female victims of domestic violence, victims of road accidents, parents of children with cancer, parents of physically disabled children, men with paraplegia and paramedics) were examined. The research involved the use of several standardized measurement tools, including the Cognitive Processing of Trauma Scale (CPOTS), PTSD Check List for DSM-5 (PCL-5), the Impact of Event Scale-Revised (IES-R) and the Posttraumatic Growth Inventory (PTGI). Results: Negative strategies (denial, regret) were positively related to PTSD, and negative to PTG, while positive strategies (positive cognitive restructuring, resolution acceptance, downward comparison) inversely, positively with PTG and negatively with PTSD. The strategy of regret proved to be the best predictor of PTSD, while the strategies of resolution/acceptance – the best predictor of PTG. The role of cognitive coping strategies with trauma varies depending on the type of experienced events. Conclusions: The cognitive processing of trauma plays an important role in the occurrence of negative and positive consequences of experienced events. Case description: Map of the treatment of stroke shows the sequence of the various diagnostic and therapeutic activities – having started from the call an ambulance until the patient’s discharge from the hospital. Presented in the article map shows the current status of the treatment of stroke patients in IPiN along with the most important from the point of view of management indicators and parameters. The basic source of data during the making of the map were own observations (gemba walk, i.e. the passage of an individual patient’s path) and interviews conducted with medical and administrative staff involved in the process of patient treatment. Ischemic myelopathy can also occur due to mechanical compression exerted by an osteophyte on the anterior spinal artery. So far there have been no clear guidelines for the management of such a patient. It seems that a recombinant tissue plasminogen activator (rtPA) could be used in the treatment. The available literature reports one paraplegic patient with thoracic spinal cord ischemia, who had been treated with rt-PA with a moderately favourable outcome. Case description: We present a case of a patient with cervical spinal cord ischemia, whose paresis resolved almost completely after having received combined treatment with thrombolysis and neurosurgery. Comment: We would like to emphasise that in spinal cord ischemia, and also if it is caused by mechanical compression of osteophyte on the artery, intravenous thrombolysis before surgery can prevent complete spinal cord infarction. The possibility to administer rt-PA in treating spinal cord ischemia should be included in the guidelines for the management of ischemic stroke. Additionally, the relationship between these two features and the acceptance of the disease was analysed. Methods: The following were used: Acceptance of Illness Scale (AIS, adaptation by Juczyński), Rosenberg’s Self-Esteem Scale (SES, adaptation by Dzwonkowska, et al.), The Scale of Sexual Satisfaction (SSS by Nomejko), The Kurtzke Expanded Disability Status Scale (EDSS) and original questionnaire on sociodemographic data. Results: No statistically significant differences in the level of self-esteem and sexual satisfaction among MS patients and healthy individuals were found. Verification of the relationships between variables showed a moderate negative correlation between the level of acceptance of illness and disability, and moderate positive correlation between the level of self-esteem and the level of sexual satisfaction. The level of disability of people with MS is the determinant of the acceptance of illness. Conclusions: The study shows new opportunities for exploring issues related to the process of adaptation and acceptance of MS as a chronic disease.
