Si, dans les pays occidentaux elle s’inscrit dans un parcours de soins structuré permettant un retour au sport encadré, la situation est très différente en Afrique subsaharienne. Cet article, basé sur les témoignages de quatre professionnels de santé exerçant en Guinée, au Burkina Faso, au Mali et en Gambie, met en lumière les réalités de terrain. Le diagnostic repose principalement sur l’examen clinique, en raison d’un accès limité et coûteux à l’IRM. La prise en charge est marquée par un manque d’infrastructures, de spécialistes et de protocoles standardisés. L’accès à la chirurgie, souvent non disponible localement, ainsi que les contraintes financières, retardent ou empêchent les soins appropriés. La rééducation postopératoire reste hétérogène et peu structurée, tandis que les décisions de retour au sport reposent majoritairement sur le jugement clinique, avec peu d’outils objectifs. Ces contraintes entraînent des retards de prise en charge, des reprises précoces et un risque accru de complications pouvant compromettre la carrière sportive. Toutefois, les acteurs soulignent des perspectives d’amélioration, notamment à travers la formation de professionnels, le développement d’infrastructures, la standardisation des protocoles et la promotion de la prévention. Cette problématique met en évidence la nécessité de stratégies adaptées aux contextes locaux pour améliorer durablement la prise en charge des blessures sportives dans la région.</div></div><div><div>Anterior cruciate ligament (ACL) rupture is a major injury in athletes. While in Western countries it is embedded within a structured care pathway enabling a supervised return to sport, the situation is markedly different in sub-Saharan Africa. This article, based on testimonies from four healthcare professionals practicing in Guinea, Burkina Faso, Mali, and The Gambia, highlights on-the-ground realities. Diagnosis relies primarily on clinical examination due to limited and costly access to MRI. Management is characterised by a lack of infrastructure, specialists, and standardised protocols. Access to surgery—often unavailable locally—combined with financial constraints, delays or prevents appropriate care. Post-operative rehabilitation remains heterogeneous and poorly structured, while return-to-sport decisions are largely based on clinical judgement, with limited use of objective measures. These constraints lead to delays in care, premature return to sport, and an increased risk of complications, potentially compromising athletic careers. However, stakeholders also identify avenues for improvement, including professional training, infrastructure development, standardisation of protocols, and the promotion of prevention strategies. Overall, these findings underscore the need for context-specific approaches to sustainably improve the management of sports injuries in the region.</div></di","PeriodicalId":38932,"journal":{"name":"Journal de Traumatologie du Sport","volume":"43 3","pages":"Pages 299-304"},"PeriodicalIF":0.0,"publicationDate":"2026-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148861850","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Sports injuries remain a major concern for both athlete health and performance, even though a substantial proportion of them could be prevented. While the effectiveness of evidence-based prevention programmes is now well established, their routine implementation remains limited, partly due to real-world constraints and the priority often given to performance. This article reflects on the gap between scientific knowledge and everyday sports practice, drawing on the Healthy Active project. Using a qualitative and context-sensitive approach, the project seeks to better understand how prevention is perceived by key stakeholders in sport and to translate these insights into concrete actions. It highlights how targeted communication tools, the involvement of athletes as role models, and a bottom-up approach can support the gradual integration of preventive behaviours into training routines. By emphasising adaptation to the local context and the relationship between prevention and sustainable performance, this article underlines the importance of integrated and pragmatic strategies to strengthen the impact of prevention initiatives in real-world settings.

","PeriodicalId":38932,"journal":{"name":"Journal de Traumatologie du Sport","volume":"43 2","pages":"Pages 211-215"},"PeriodicalIF":0.0,"publicationDate":"2026-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148170951","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This population is associated with specific treatment challenges. When it comes to surgical management there is a risk of growth disturbances, while conservative management increases the risk of damage to the cartilage and meniscus associated with persistent joint instability. Several surgical techniques are possible: transphyseal, epiphyseal and mixed, the final choice depending on the remaining possible growth and surgeon discretion.

This clinical case illustrates the importance of the clinical examination and the anamnesis for the diagnosis and management of acute knee trauma. A 43-year-old man presented to the emergency room of the centre hospital of Bretagne centre following pain in the right knee following a fall while climbing and jumping from a piece of apparatus. The anamnesis reported a non-contact trauma mechanism with an intense eccentric muscle contraction during a landing movement on the right leg in flexion. At this time, he felt a sharp pain in the knee area. The joint immediately “swelled” according to his terms and was associated with functional impotence. On clinical examination, the patella had moved up along the quadriceps, there was a hollow between the patella and the anterior tibial tuberosity, active extension of the leg was impossible.
