{"title":"Role of Potassium Permanganate (KMnO4) and/or vegetable oil gastric lavage in aluminium phosphide poisoning","authors":"S. We, as Emergency physicians working in this part of the world frequently encounter patients with intentional intake of wheat/ rice tablets. Due to its highly toxic potential, the intake of a single tablet is enough to cause mortality. Its ability to liberate phosphine gas, leading to early systemic toxicity in terms of cardiovascular arrhythmias, refractory hypotension and severe metabolic derangements are well observed. Despite multiple recent medical advances, there is no specific antidote thus the management remains mainly supportive. Our current practices, in managing aluminium phosphide toxicity are highly varied across the region. It is a common practice to use KMnO4 and/or vegetable oil gastric lavage in these patients which is considered to have a survival benefit. Mohan","doi":"10.5455/sajem.040207","DOIUrl":"https://doi.org/10.5455/sajem.040207","url":null,"abstract":"We were describing an interesting case of complete heart block (CHB) received in the emergency department (ED) with the cardiogenic shock in the life-threatening situation, who was successfully manage by cardiac pacing, initially using a J Tip guidewire (JTG) due to non-availability of temporary transvenous pacing catheter (TVPC) and later on using TVPC once available . TVPC placement is a potentially life-saving procedure that involves placing a catheter-based electrode within the right ventricle via central venous access, and then stimulating that electrode and the heart with an external pacing generator to optimize cardiac output. In the ED, the procedure can be performed either blindly or with ECG guidance and recently with USG guidance. Usman, Nizar Ali","doi":"10.5455/sajem.020211","DOIUrl":"https://doi.org/10.5455/sajem.020211","url":null,"abstract":"OBJECTIVE \u0000To assess the level of burnout and professional satisfaction among the doctor and nurses working in an emergency department of a \u0000tertiary care hospital in Islamabad, Pakistan. \u0000 \u0000METHODOLOGY \u0000A self-administered questionnaire-based cross-sectional study was conducted in the emergency department of Shifa international \u0000hospital, Islamabad. The questionnaire was based on a Maslach burnout inventory tool and pre-validated study. Data was collected from all the doctors and nurses who were working in this particular department for at least 3 months in any capacity. The tool consisted \u0000of 12 questions and covered 4 domains which included emotional exhaustion, depersonalization, personal accomplishment and personal satisfaction. \u0000 \u0000RESULT \u0000There were 80 participants with 40% doctors and 60% nurses and the male to female ratio was 50-50%. The results found 78.8% of the \u0000study population showing low or no burnout and 21.3% having moderate to high burn out. 17% of the study population was emotionally \u0000exhausted and depersonalization was prevalent among 18% of study population. \u0000 \u0000CONCLUSION \u0000Emotional exhaustion and depersonalization is a significant concern for the people working in the emergency department. However, similarly marketed supplements may contain ingredients that can harm the user. Our case highlights a 45-year-old male who ingested several tablets of a supplement known as vitamin B17, Amygdalin, with subsequent fatigue, mild lactic acidosis, and worsening shortness of breath. He was treated as a case of cyanide toxicity based on clinical suspicion and improved subsequently.\u0000The case describes the potential harm from unregulated substances, particularly those purchased online.
