We, as Emergency physicians working in this part of the world frequently encounter patients with intentional intake of wheat/ rice tablets. Due to its highly toxic potential, the intake of a single tablet is enough to cause mortality. Its ability to liberate phosphine gas, leading to early systemic toxicity in terms of cardiovascular arrhythmias’, refractory hypotension and severe metabolic derangements are well observed. Despite multiple recent medical advances, there is no specific antidote thus the management remains mainly supportive. Our current practices, in managing aluminium phosphide toxicity are highly varied across the region. It is a common practice to use KMnO4 and/or vegetable oil gastric lavage in these patients which is considered to have a survival benefit. In this review article, we will search for the best evidence to answer this question whether there is a role of KMnO4 or vegetable oil gastric lavage in reducing the mortality.","PeriodicalId":389251,"journal":{"name":"South Asian Journal of Emergency Medicine","volume":"223 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127921037","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The difficulty in choosing a research area can be due to the overlapping themes/clinical spectrum and the wide age range that the emergency medicine physician sees during his/her clinical practice. In this brief report we are proposing a model that can be utilized either by the trainees or the postgraduate program of the hospital in order to help them selecting research domains.","PeriodicalId":389251,"journal":{"name":"South Asian Journal of Emergency Medicine","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125579025","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

TVPC placement is a potentially life-saving procedure that involves placing a catheter-based electrode within the right ventricle via central venous access, and then stimulating that electrode and the heart with an external pacing generator to optimize cardiac output. In the ED, the procedure can be performed either blindly or with ECG guidance and recently with USG guidance. After placement, CXR perform to verify position of TVPC as well as any procedural complication.","PeriodicalId":389251,"journal":{"name":"South Asian Journal of Emergency Medicine","volume":"3 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126994748","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, similarly marketed supplements may contain ingredients that can harm the user. Our case highlights a 45-year-old male who ingested several tablets of a supplement known as vitamin B17, Amygdalin, with subsequent fatigue, mild lactic acidosis, and worsening shortness of breath. He was treated as a case of cyanide toxicity based on clinical suspicion and improved subsequently.\u0000The case describes the potential harm from unregulated substances, particularly those purchased online. Qureshi","doi":"10.5455/SAJEM.020107","DOIUrl":"https://doi.org/10.5455/SAJEM.020107","url":null,"abstract":"Although resuscitation aims to preserve life and restore health, it is sometimes at the expense of increased suffering and disability to the receiving patient. In this situation, the treating physicians often end up in the dilemma of whether resuscitation is appropriate for the patient and have discussions with the patient and/or family. This may lead to a decision to “Do Not Attempt Resuscitation” (DNAR). In western societies, the patients have an option to opt out of Cardiopulmonary Resuscitation (CPR) if they are in the situation, and at times, the medical team can make a clinical decision when the efforts are deemed futile. This is supported in the legislation of those countries, and as an example, the UK law states, There is no obligation to give treatment that is futile or burdensome.1 As the preservation of life in Muslim countries is of paramount importance due to religious reasons as well as cultures and traditions, a similar blanket approach to DNAR in Muslim countries is rarely seen. One of the earliest Fatwa (Islamic law) on this topic by Shaykh ‘Abd al-‘Azeez ibn ‘Abd-Allaah ibn Baaz and Shaykh ‘Abd al-Razzaaq ‘Afeefi in the year 1986, states “if reviving the heart and lungs is of no benefit and not appropriate because of a certain situation, according to the opinion of three trustworthy specialist doctors, then there is no need to use resuscitation equipment, and no attention should be paid to the opinions of the patient’s next of kin concerning the use of resuscitation equipment or otherwise, because this is not their specialty.” 2 This has led to physicians in Saudi Arabia being empowered to make DNAR decisions for their terminal patients and allow dignified death. The lack of similar legislation in Pakistan often places physicians in the unenviable situation of having to explain to patients and their families why CPR would be futile, and, in the absence of consensus, having to provide expensive, futile treatment, at the expense of the family, and other patients who may benefit from the resources used up. When possible, these discussions should be had by the treating speciality and the patient/families, but emergency physicians often find themselves in the awkward and undesirable situation of initiating these difficult conversations. The area to discuss and investigate here is about the ethical, religious, and legal implications of such a decision, and how an emergency physician can continue to act in the patient’s best interest while keeping themselves safe.","PeriodicalId":389251,"journal":{"name":"South Asian Journal of Emergency Medicine","volume":"303 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"1900-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114443424","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
