Mustafa, Faiza Shah, Dania Dawood","doi":"10.5455/SAJEM.20104","DOIUrl":"https://doi.org/10.5455/SAJEM.20104","url":null,"abstract":"Objective: To assess perspectives of Pakistani emergency physicians regarding the American College of Emergency Physicians Choosing Wisely Recommendations \u0000 \u0000Methodology: Physicians working in the emergency department of three hospitals in Pakistan were included in this cross-sectional study. The interviewer-administered study questionnaire was formulated as per ACEPs Choosing Wisely recommendations for emergency physicians. The study questionnaire was validated, and informed written consent was obtained from all the study participants. Ethical approval was taken from the ethical review board of Poonch Medical College, Azad Kashmir, Pakistan. Data was analyzed by SPSS v23.0 and was presented in tables and charts. \u0000 \u0000Results: Out of 70 emergency physicians, 63 responded and were included in the study (response rate = 90%). More than two-thirds of the physicians (68.3%) had never heard of Choosing Wisely recommendations and more than three-quarters (77.8%) had never discussed it with anybody. Eighteen physicians (28.6%) who knew about it had heard about it from their colleagues. Physicians beliefs about Choosing Wisely campaign were as follows: 28.6% believed it will strongly increase patient safety, 39.7% believed it will somewhat increase the quality of patient care, and 57.1% believed it will have no impact on physicians salary. The most important reason (38.1%) behind ordering a low-value investigation is due to fulfillment of patients request. \u0000 \u0000Conclusion: This study identified emergency physicians low level of familiarity with Choosing Wisely recommendations. With the ever-increasing national health cost, issues of drug resistance and hospital acquired infections, it is important to optimize the health care and expenditure. Data was collected through a validated, self-administered and structured questionnaire. \u0000 \u0000Results: Out of 126 participants, the reported prevalence of plagiarism was 57.14% (n=72) with a higher proportion among clinically oriented specialty students (65.43%, n = 53) as compared to their counterparts doing academic degrees (42.22 %; n=19). We present a case of a 45-year-old male who presented with headache, nausea, and visual impairment immediately after the descent from K-2 mountain (8611 meters) in Pakistan. His neurological exam did not reveal any findings, CT scan brain was done which showed thrombosis in the superior sagittal sinus with associated wedge-shaped hemorrhage in the frontal lobe. The outcome of a cardiac arrest improves significantly if it is promptly and properly instituted. Basic life support sessions are done regularly for the doctors to improve the knowledge and skills, but medical students are usually not mandated to have BLS updated regularly and so are not aware of the new guidelines. This study aimed to assess the impact of basic life support training on the knowledge of basic life support among under- graduate medical students. \u0000 \u0000METHODS \u0000One hundred and twenty eight medical students, mostly female (n=85, 66.4%) partici- pated in this interventional study through convenience sampling. A 16-item validated questionnaire extracted from American Heart Association (AHA) guidelines was used to assess the knowledge before and after the BLS training session. \u0000 \u0000RESULTS \u0000The pretest mean score (Standard deviation) came out to be 3.5 (±1.25) with a range of 1-6 out of a total score of 10. The post-test mean score was obtained as 6 (±1.25) with a range of 2-9. Statistically significant ( P < 0.01) difference was obtained which shows that undergraduate medical students' prior knowledge of BLS was poor which improved significantly after the commencement of the BLS training session. \u0000 \u0000CONCLUSION \u0000This study revealed that the BLS training session significantly increased the knowledge of BLS amongst the medical students.
