Eosinophil – “a good servant but a bad master”. Cardiology Lett. 2021;30(1):57–61 Abstract. Eosinophilic myocarditis (EM) is a rare form of myocarditis with a very heterogeneous etiology. It is characterized by the presence of eosinophilic infiltrate in the myocardium. The course can vary from an asymptomatic clinical picture to life-threatening conditions. We describe a case of a 29-year-old patient with a history of bronchial asthma hospitalized for chestpain and manifesting heart failure. A moderate decrease of ejection fraction of the undilated left ventricle chamber was found. High levels of troponin, NTproBNP and eosinophilia were present in the laboratory findings. By endomyocardial biopsy we confirmed the presence of an eosinophilic infiltrate with a histological pattern of myocarditis. We did not confirm clonal proliferation of eosinophils. Corticoid therapy was initiated. After a week we observed normalization of eosinophils and a decrease in cardiac specific enzymes. Different types of ventricular tachycardias, which required multiple external defibrilation, occured during treatment. An ICD was implanted in the secondary prevention of sudden cardiac death. Ventricular arrhythmias have been stabilized by a combination of pharmacotherapy and individualized programming of ICD functions. Control biopsy was without histological finding of myocarditis. Left ventricular ejection fraction improved by 10%: the patient has been asymptomatic. Eosinophilic myocarditis is a rare disease but with poor prognosis in the acute phase. With the presence of eosinophilia in peripheral blood, this potential cause of acute heart failure should also be considered. Clear diagnosis must be confirmed by endomyocardial biopsy. The basis of treatment is corticoid therapy, which positively affects survival. Objective : To monitor possible changes in the incidence and severity of erectile dysfunction (ED) in hypertensive patients in relation to the targeted intervention by a nurse aimed at adopting the correct principles of lifestyle of a hypertensive patient. Methods: In a group of randomly enrolled 200 patients aged 40 to 70 years, the questionnaire gathered data on the incidence and severity of erectile dysfunction as well as on the adherence to the correct principles of lifestyle and quality of life. Data collection was repeated after a year, while in half of the patients a nurse performed a periodic targeted intervention during the year. Results: In hypertensive patients with organ complications (previous myocardial infarction, confirmed coronary artery disease, stroke), there was a higher incidence of moderate and severe erectile dysfunction compared to hypertensive patients without organ complications. In the groups without a nurse-intervention, we observed an 18% increase in the number of patients with erectile dysfunction in the group without organ complications and 37% in the group with organ complications, after one year. Conclusion: The results confirmed the assumption of a positive effect of targeted nursing interventions aimed at the adherence to correct lifestyle on the quality of life and within it the manifestations of erectile dysfunction. Transcatheter mitral valve replacement with Tendyne valve: The first two cases in Slovakia Cardiology Lett. 2021;30(1):53–56 Transcatheter mitral valve replacement (TMVR) with the Tendyne mitral valve has emerged as a novel potential therapy for patients with severe mitral valve disease who are unsuitable candidates for conventional surgery and transcatheter mitral repair with MitraClip. In this communication we present the first two cases that were performed in Slovakia and the V4 countries. Two surgically high risk female patients 78 and 79 years old respectively (EuroScore II 7.8% and 7.4% respectively) also unsuitable candidates for transcatheter mitral repair with MitraClip, underwent transapical TMVR with the Tendyne valve. Both procedures were successful: one of the patients two months after the procedure is doing well, the other patient had a complicated postoperative course and died on postoperative day 4. Moreover we present a brief description of the valve, and of the deployment technique, and an overview of the major clinical studies. The aim of the presented proposal is to counteract the dramatic spread of the COVID-19 disease and to target reduction of its high case fatality rate by risk stratification during the early phases of the disease. Two key factors have contributed to the catastrophic extent of the disease in several countries: – Most of the known COVID-19 hotspots were locally preceded by massive gatherings of up to several hundred-thousand people (sporting events, religious pilgrimages etc.). – Widespread, mostly subclinical infection of health care workers (HCW) in the hospitals of the hotspot regions inabled a crucial dramatic virus transmission to elderly population. Replication of the former factor for virus spread is actually stoped by measures preventing people from as-sembling. However, HCW represent at least 11% of the total infected population and they still suffer from suboptimal availability of protective gear. The following targeted steps are proposed: the several biomarkers (mainly parameters of inflammation, thrombophilia, and organ damage – heart and liver) aiming at early risk stratification. Thus, this proposal represents a complementary sustainable strategy to general anti-epidemic measures it could be of help to achieve 3 crucial goals: 1. To minimize virus spread in and from health-care facilities; 2. To protect HCW; 3. To identify COVID-19 patients at high risk for adverse, frequently lethal, outcome. Atrial fibrillation (AF) is the most frequent arrhythmia in seniors and can negatively influence their quality of life. In the “Slovak audit of Atrial FIbrillation in Seniors” study (4252 consecutive patients with AF aged ≥ 65 years [mean age 80.9 years], hospitalised during 2.5 years in 4 acute geriatric departments) serum potassium level was one of the analysed parameters. We proved that both hypo- and hyperkalemia are quite frequent. This can be related to the drugs used as well (we analysed 9 drugs / drug groups with the potential known to influence kalemia). We could not prove an association between diuretics and hypokalemia. Neither did a cumulation of drugs with hyperkalemic potential lead to an increased frequency of hyperkalemia: on the contrary, they significantly decreased this risk at discharge. We proved the expected mild increase in risk of hyperkalemia under sporinolactone medication, but only at admission (OR 2.06), at discharge spironolactone was “protective” against hyperkalemia. Surprisingly we revealed an association between low molecular weight heparin (at discharge) and hyperkalemia (OR 2.01), and between non-steroidal anti-inflammatory drugs (at admission) and hyperkalemia (OR 1.76). Thus in real (hospital) life we can control the expected threats of potassium disturbances induced by diuretics and drugs influencing the renin – angiotensin – aldosterone system in an effective way. This is confirmed also by the finding that potassium dysbalance occurred in all investigated drugs / drug groups less frequently at discharge than at admission. Kalemia and its disturbances can be easily and cheaply tested and are well treatable. Appropriate attention to them should be paid in seniors (with AF) during prehospital and hospital health care delivery. Objectives: Rheological properties of blood significantly affect blood flow in large vessels. Changes in macrocirculation contribute to atherogenesis and consequently cardio-vascular diseases. However, blood flow disorders also impair the quality of microcirculation, e.g. in patients with recurrent episodes of cardiac ischemia. Since a negative correlation between plasma vitamin C levels and blood viscosity was found, we focused on the effect of vitamin C on hemorheology in more detail. Considering the important role of erythrocytes in blood rheology, we decided to simultaneously monitor changes in erythrocyte parameters. Methods: Hemorheological parameters were determined by rotational and oscillatory measurements. Regarding erythrocytes, we measured their deformability, nitric oxide production and sodium-potassium pump activity. Results: We observed a beneficial effect of increased vitamin C intake on several hemorheological parameters as well as improvement of erythrocyte deformability, increased nitric oxide production and activity of sodium-potassium pump. No change in oxidative stress and antioxidant protection parameters in plasma was observed. Conclusion: Vitamin C administration to an improvement in several hemorheological parameters in healthy young volunteers. Since improvement in hemorheology may contribute to better quality of macro- and microcirculation, our results suggest a possible benefit of vitamin C administration in patients with impaired hemodynamics.
